EE was trying to undo a flange on FM03 sample station. As the flange broke free EE's hand slipped and EE's index and middle finger were cut.

EE opened a clay bailey and material came out and got into eye.

During preparation for dismantling of motor and gearbox on slide gate EE's coworker released all bolts on gearbox which cause spinning of motor and gearbox. Coworker got caught by equipment and started spinning with it. During spinning coworker's body hit employee and caused them bruises on their leg and arm.

EE ran into a JLG man lift. EE was wearing a seatbelt.

EE slipped and fell and cut EE's right forearm. It required 6 stitches and EE was placed on modified duty.

Miner working on emission analysis equipment located in the third stage of the pyro processing tower, Miner was cleaning the sample probe, the sample probe was still hot when the miner accidentally touched it with EE's hands. Miner show minor burns in palm of EE's hands.

Was pulling a pallet jack and strained their left shoulder

On final day of EE's job, employee was descending a 15' fixed ladder with a cage when EE's foot slipped and EE fell approximately 10'. Employee broke EE's left fibula.

EE states they have back pain from driving a haul truck. EE was placed off work due to not being able to drive to and from work.

Years of driving a haul truck is taking a toll on EE's lower back.

Employee was knelling down. When they went to stand up they strained they left knee

EE was pushing a portable welder. As EE was doing so, EE strained right hamstring.

While cleaning a weight bridge plate the miner went to adjust the plate it fell onto EE's finger. The miner was wearing gloves.

EE was walking down a 22.4 degree slope and slipped on loose material and fell to same level.

Employee was walking down a slope and slipped, landing on their tail bone.

EE was performing preventative maintenance on an MCC bucket. While checking the A phase line side termination EE lifted EE's arc flash hood to align EE's screw driver correctly, as EE lowered EE's hood the screw drive made contact with the frame of the bucket causing an arc flash. EE sustained 2nd degree burns to face.

Electrician had inserted a new breaker bucket to a 480 volt panel and attempted to re-energize the panel. Upon doing so the breaker shorted causing the miner to sustain burns.

SHOVELING DEBRIS IN & AROUND CHUTE & SMASHED FINGER

Employee was working on the pre-heater tower. As EE was coming down a stairway, EE slipped/tripped on the last step. EE fell onto the mezzanine straining their back.

Employee was water blasting in the tower. As EE was holding the gun, EE pulled the hose causing the safety pedal to be pushed down. This in turn pressurized the gun causing it to strike the miner above the left eye.

EE got dust in eye. The next day EE reported irritation in the eye and was sent to the clinic.

Miner claims to have injured (R) knee climbing stairs, while fetching a fire extinguisher to extinguish a spot fire, while on watch.

EE was walking down some stairs and EE's hand slipped off the handrail. This caused EE to stumble and loose EE's balance. When trying to recover from the slip EE stepped on a curb and rolled EE's ankle.

Employee slipped and fell straining their back.

EE was removing a thermocouple from the tower using a pipe wrench. The pipe wrench slipped off the thermocouple and pinched left pinky between the pipe wrench and the handrail.

Employee was climbing into Ball Mill 2. As EE got to the end of the flights EE stepped down straining EE's left knee.

EE tested positive for covid. EE had symptoms after EE's round of day shifts.

Employee tested positive for covid on 12/1/20. I was notified by employee that was admitted to the hospital on 12/5/2020. I reported it to MSHA to err on the side of caution. Employee is not aware of how or when they got the virus.

While EE was welding a plate inside the crusher, a rock that was lodged in the rollers (overhead) became dislodged and struck EE in the back of the head, near EE's ear. Medical treatment was provided and EE was diagnosed with a concussion.

Expanded metal slid and cut EE's right forearm. Accident time unknown safety manager on vacation.

EE slipped and fell to ground.

While grinding, grinder became wedged between flapper gate causing the grinder to kick back and strike employee in area above the right knee. This caused a 6 cm laceration to that area. This resulted in 7 stitches

Employee vehicle got stuck on the plant road. As employee reached under vehicle to hook up a tow strap employee jammed left hand index finger breaking it.

Employee was carrying a box and as EE approached the door EE slipped and hit EE's forehead.

EE was walking in the middle of the mill and slipped on some material.

EE was pulling on a magnet and injured back. Not sure if it is lost time or what yet.

EE was reaching to put a cap on a drill steel and strained back.

EE was operating the loader and hit a pothole causing back to hurt. EE declined medical attention at first but asked to go to the doctor on 9/19/17.

Employee crashed vehicle on ice after shift on way home.

EE lost grip in hands.

The employee was cleaning a plugged chute and stuck ee hand into a rotary feeder.

Over the last 4 years ee noticed back was hurting when operating mobile equipment. Over time the pain became unmanageable and the employee was placed on light duty.

EE put left hand next to clay bailey to check the draft and hand was slightly pulled in the clay bailey. This caused EE to pinch finger in-between pin on screw and clay bailey.

EE was cleaning a chute and it appears EE stuck hand into a clay bailey while holding a flash light. As hand entered through the clay bailey and into the screw conveyor housing, a piece of round stock cut hand.
