The accident occurred on the conveyor belt that goes to the fine mill. The employee was cleaning the accumulated material on the magnet while the plant was out of service. Then the operator without noticing that the employee was still cleaning, turns the plant back on, this caused the employee to be trapped between the conveyor belt and the magneto.

THE OPERATOR SAYS TAHT THE LOADER MOVEMENT IS CAUSING PAINS IN HIS BACK.

THE PERSON WAS HIT BY A STONE IN HIS RIGHT HAND WHILE HE WAS WASHING NEAR A BELT CONVEYOR, THE STONE REBOUNDED IN THE BELT AND FALL OUTSIDE OF THE CONVEYOR.

INJURED PERSON WAS DRIVING LOADER. WAS FEELING PAIN IN WAIST. WHEN HE CAME DOWN OFF OF THE LOADER HE FELT A HARD APIN THAT MADE IT DIFFICULT TO WALK.

WELDER WAS WELD3NG VERTICAL SHEET PILINGS.THERE WERE A FEW SHEET PILES (15' LON) ON THE GROUND. A LOADER OPERATOR PASSED HIS EQUIPMENT CLOLSE TO THE WELDER AND PINCHED ONE OF THE PILES AT ONE END THUS MAKIN1 THE PILE ROTATE.AS THE PILE ROTATED IT HIT THE WELDER ON HIS RIGHT LEG.
