The teammate was attempting to dump the hopper. EE was unable to access the containers on the paved, North Side of the lot. As EE tried to reverse the forklift to align with the hopper, the back tire came off the pavement and became stuck in the mud. EE tried to maneuver the fork truck out of the mud with the load lifted and the forklift turned over.

While the Operator was talking he lowered his hands and the belt pulled his hand in between the free roller and the conveyor. As a result, the top of his left hand was scraped.

THE OPERATOR WAS WORKING NEAR A CONVEYOR BELT (6" WIDE) WHICH WAS CARRYING 10# BAGS OF CLAY. A BAG HUNG & AS HE REACHED FOR THE BAG HIS RIGHT INDEX FINGER STRUCK THE SIDE OF THE CONVEYOR CAUSI NG A LACERATION TO FIGHT INDEX FINGER.
THE OPERATOR WAS DRIVING A FORKLIFT THRU A WAREHOUSE DOOR (PLASTIC STRIPS HANGING FROM TOP OF DOOR). ONE OF THE PLASTIC STRIPS HIT THE OPERATOR ON LOWER LIP. SMALL LACERATION TO INSIDE OF LOWE R LIP- LOOSENED TOOTH.

OPERATOR WAS PLACING SHIPPING PAPERS INTO POCKET INTRALER, THE DRIVER PULLED THE TRAILOR FROM THE DECK AS OPEATOR WAS STANDING PARTIALLY IN TRALIOR, HE LOST HIS BALANCE AND ATTEMPTED TO BREAK HIS FALL WITH LEFT ARM AND PULLED LIGAMENTS IN LEFT FOREARM.
