Employee was using a walk behind bobcat to clean up spillage under conveyor structure. After filling bucket employee lifted bucket with controls, causing the back end of the machine to rise pinching left hand between the controls and the conveyor structure. Pinching caused laceration and abrasion.

Mostrando los 14Employee was working on snow removal with his highlift when he struck a piece of frozen ice and it jarred him in his machine causing pain in his lower back. EMPLOYEE RETURNED TO WORK ON 1/28 & 1/29/09....FIRST DAY MISSED 1/30/09
Employee was using his left hand to hold the "raise bed" switch on his loaded haul truck. His hand slipped from the switch causing his bed to free fall approx 1-2' causing the employee's body to jar.
Employee was kneeling on a catwalk and was using a 3/4" pry bar to scrape build up from the top drum of a gravity take up on the #7 conveyor, while the belt was running. The belt grabbed the bar and pulled it through the drum causing the pry bar to be pulled from his hands causing lacerations from the mushroomed end of the bar.
Employee was operating a skidsteer cleaning up at the crushing plant. When he got out of the machine he felt a pain in his lower left back. (Not reported until 1/07/08).
EMPLOYEE WAS IN A MAN LIFT TURNING ANGLE BRACKETS IN THE UPRIGHT POSITION TO SET THE CONVEYOR FRAME ONTO THE BRACKETS. THE OPERATOR HOLDING THE END OF FRAME WITH FORKLIFT MOVED AND IT CAUSED THE CONVEYOR FRAME TO MOVE AND LET CONVEYOR COME DOWN , RESTING UPON EMPLOYEE'S LEFT THUMB.
This over the road driver got out of his truck @ stockpile (restricted area) while waiting to be loaded to air up his low tire. On the ground he reaches into his truck to turn on a valve to put compressed air to his air hose. He then accidentally released his parking brake which allowed the truck to roll back and his leg ended up under the front wheel of his truck.
Employee was operating a bobcat at the crusher doing cleanup when his bucket hit something in the dirt causing him to jar in his seat. *****I realized that a form to MSHA had not been submitted. That is the reason for the delay. Any questions, please contact me.
EMPLOYEE WAS BEING LOADED WITH SHOT ROCK BY A FRONT END LOADER. THE LOADER OPERATOR DUMPED THE ROCK INTO THE TRUCK HARD CAUSING THE TRUCK TO SHAKE VIOLENTLY.
EE WAS IN HIS HAUL TRUCK WAITING TO BE LOADED BY PIT LOADER. THE LOADER DUMPED A BUCKET INTO THE BED AND WHEN HE STARTED BACKING UP THE LOADER HIT HIS TRUCK BED WITH HIS BUCKET, CAUSING THE TR UCK TO ROCK BACK AND FORTH.
EE WAS OPERATING FRONT END LOADER ON FROZEN GROUND, CLAIMING THAT EXCESSIVE BOUNCING CAUSED BACK TO ACHE.
EMPLOYEE WAS MAKING ROUNDS CHECKING GATE LOCKS. HE WENT TO GET OUT OF HIS TRUCK THINKING IT WAS IN PARK, INSTEAD IT WAS IN REVERSE. HE WAS KNOCKED TO THE GROUND BY THE DOOR, HITTING HIS HEAD O N THE GROUND, BRUISING HIS RIBS AND HIPS.
EMPLOYEE WAS STANDING ON A LARGE ROCK FLAGGING THE FRONTEND LOADER TO BUILD A RAMP FOR THE ROCK DRILL, WHEN THE ROCK HE WAS STANDING ON WAS HIT WITH THE TIRE OF THE MACHINE, CAUSING THE EMPLOY EE TO CATAPULT APPROX. 8' IN THE AIR. HE SUSTAINED A BROKEN COLLAR BONE, BROKEN RIB AND BRUISED LUNG.
EMPLOYEE WAS BACKING UP ON STOCKPILE WITH HIS WABCO HAULTRUCK TO DUMP A LOAD. THE PILE GAVE WAY CAUSING THE TRUCK TO GO OVER BACKWARD APPROX 25-30' DOWN.
