On 10/2/2012 EE came into work using crutches and stated that he had injured his back on 09/18/12 while operating a loader. On 9/18/12, he left work around 3:00AM without notifying anyone of his injury. He called-in sick on 09/19/12. He came back and worked the following days; 9/20, 9/23, 9/25, 9/26, 9/27 and 9/28. He then called in 9/30/12 and 10/1/12.

Hydraulic fluid got into the eyes of the forklift operator for the warehouse. After taking several moments to wash his eyes out with eye wash and water, he deemed it necessary to seek medical attention because of prolonged irritation.

The miner was inadvertently injured when his left shoulder struck the window latch in his designated haul truck.

Employee was cleaning under 5 pound line when a dust shield came off. He tried to put it back on and got his left hand caught in belt.

A employee was backing up forklift out of shop when other ee's motioned for him to stop, but the forklift hit employee on right side.

INJURED WAS STACKING PALLETS WHEN A PALLET FROM BEHIND FELL AND HIT LOWER BACK.

EMPLOYEE WAS REPAIRING FORKLIFT #11. EMPLOYEE HAD THE MAST AND FORKS RAISED 2 1/2 FEET WITH ANOTHER FORKLIFT #10. EMPLOYEE WAS PULLING FORKS OFFOF FORKLIFT #11 WHEN THE FORKS AND MAST SLIPPED OFF OF FORKLIFT #10, PINNING EMPLOYEE'S LEFT FOOT UNDEER FORKS. EMPLOYEE TRIED TO RAISE THE FORKS OFF HIS FOOT, CUTTING HIS FINGER AND POSSIBLY STRAINING HIS BACK. ANOTHER EMPLOYEE INTERVENED.

PARSON BELT WAS RUNNING WITH NO PRODUCT ON BELT. EMPLOYEE LEANED ON BELT. THE BELT CAUGHT HIS FINGER & CARRIED IT INTO THE SPACE BETWEEN THE TRANSFER PLANT AND BELT WHICH BECAME A PINCH POINT . EMPLOYEES FINGER WAS CUT REQUIRING 10 SUTURES.

FORKLIFT CAME DOWN AND HIT HIS LEFT SHOULDER. PUT ARM UNDER FORKS TO CLEAR BAG, WHEN THE LIFTS CAME DOWN AND HIT HIM.

HOLDING A BULK BAG FROM THE TOP & THE EE ON THE FORKLIFT WAS TRYING TO GET INTO POSITION & LIFTED THE FORKS TOO HIGH & IT PINCHED EE'S HAND IN BETWEEN FORK & TUBE.
