EE and cohorts had lowered the counterweight following a repair from the previous day. EE went up in a manlift to observe operation when they became entangled in moving machine parts of the 48-inch conveyor. EE was degloved from the shoulder to the forearm, has multiple fractures including wrist, thumb, scapula and shoulder. This injury will require skin graphing as well.

Employee was using a Front End Loader to feed material to the plants. As EE backed up to get another load of material, the employee backed into another parked front end loader. The employee received a work restriction due to a muscle strain to EE's neck and right shoulder from the impact. Employee is due back at the doctor on 1/17/2025.

While operating a Kubota RTV, the vehicle hit a pot hole and jolted the miner who sustained a back sprain.

Miner was operating a forklift moving bagged product on a pallet inside the packing warehouse and hit a structural post with the front of the pallet. The miner reported feeling back pain after the incident.

While driving a forklift the driver hit a dip in the road and caused the forklift to jolt. This created pain in the left leg and lower back of the driver.

Contract EE was standing near a moving conveyor belt. EE placed right hand on the moving belt. The conveyor caught the EEs glove and pulled right index and middle finger into the 1/4" space.

EE was digging in the hard rock portion of the muck pile when the CAT 988F loader teeth slipped out of muck pile jolting machine and the EE.

Operator picked up the boulder, dropped it and the boulder did not break. During the second attempt the boulder was lifted closer to the edge/teeth. The rear wheels of the loader lifted off the ground and the sudden loss, or shift of weight caused the rear wheels to come down hard. The operator felt pain in his lower back. Diagnosis Compression fracture to a lower vertebrae.

Employee lost control of haulage unit, ran onto berm overturning unit. No significant damage to haulage unit, however employee sustained a contusion to the left shoulder region.

Working on the drive of a screw conveyor. One employee was jogging, when another grabbed the belt and tried to manually jog. His thumb was caught by the belt and his right thumb was amputated.

Miner was operating hi-vac truck. Miner opened sight port without turning off engine, system pulsed and caused dust to go into the face of the miner. The miner was wearing safety glasses only, and dust entered his eye.

EE WAS OPENING DOORS ON RAIL CAR USING PRY BAR TO TURN DOOR OPENER, BAR SLIPPED OUT OF TURN OPENER STRIKING EE ON RIGHT SHOULDER. EE FINISHED SHIFT AND WENT HOME LATER THAT NIGHT, PAIN INCREAS ED. EE WENT DR. WHERE THE DR. SAID HE HAD A SPASAT THE POINT WHERE BAR STRUCK EE.

THE 1308 BASE BELT HAD STOPPED SOMETIME DURIN THE NIGHT, EMPLOYEES WERE IN THE PROCESS OF DIGGING THE MATERIAL FROM UNDER THE BELT THAT HAD SPILT OVER THE BELT ONTO THE CATWALK. THERE WAS A 6 MAN CREW DIGGING THE BELT OUT FROM THE MATERIAL. IT WAS THOUGHT THE MATERIAL HAD BEEN DUG ENOUGH. THE BREAKER WAS TURNED BACK ON, THE BELT WOULD NOT MOVE. HE THEN WENT BACK TO RESET THE BR

HE WAS IN THE PLANT AREA NEAR THE DISCHARGE FEEDER AND GOT DUST IN HIS EYE. IT REMAINED IRRITATED AND THREE DAYS LATER HE WENT TO THE DOCTOR, WHO GAVE HIM A PRESCRIPTION FOR SALVE TO SOOTH HI S EYE.

EMPLOYEE WAS CLEANING BEND PULLEY WITH HAMMER THE PULLEY CAUGHT THE HAMMER & PULLED EMPLOYEES HAND INTO THE PULLEY THE BELT WAS IN OPERATION

THE EMPLOYEE WA8 USING A UKE HAUL TRUCK TO EMPTY MATERIAL FROM A WASTE BIN AND DUMPING IT IN THE QUARRY AREA. THE MATERIAL WAS PILED UP AND SLOPED TO A HEIGHT OF APROX. 15 FEET. A WALL HAD B EEN CREATED FRO4 THE BACK SIDE AND HE WAS ATTEMPTING TO DUMP OVER THIS WALL WHEN HE BACKED UP TOO FAR AND THE UKE SLIPPED OFF THE EDGE OF THE WALL. THE EMPLOYEE WAS ATTEMPTING TO MOVE A BUNDLE OF SCREW CONVEYOR TROUGHS AND HAD RIGGED A CABLE SLING AROUND THE BUNDLE. AS THE BUNDLE WAS BEING LIFTED, THE LOAD SHIFTED AND HIS RIGHT HAND WAS C AUGHT BETWEEN TWO "EDGES" RESULTING IN A SCISSOR TYPE ACTION THAT CUT THE END OF HIS RIGHT LITTLE OFF.

EMPLOYEE GOT OUT OF HIS TRACTOR (SEMI) WHILE THEUNIT WAS BEING LOADED AND PROCEEDED TO GET UNDER THE TRACTOR FRAME (DIRECTLY IN FRONT OF THE TANDEMS) AND WAS BLEEDING AN AIR TANK AND TRYING TO ADJUST THE AIR TANK VALVE. THE LOAD OUT OPERATOR CAME DOWN FROM THE PLATFORM AND NOT SEEING THE DRIVER, DROVE THE TRUCK FORWARD; RUNNING OVER THE DRIVER WHO WAS UNDER THE UNIT. LEFT ARM REMOV
