VICTIM WAS OPERATING AND HAD BEEN OPERATING A CAT 988 LOADER AROUND THE PLANT AREA AND WITHIN THE PIT. FRIDAY THE LAST DAY HE WORKED NOTHING WAS REPORTED MONDAY WHEN HE DIDN'T SHOW UP FOR W ORK THE FOREMAN INQUIRED WHERE HE WAS AND IT WAS REPORTED THAT HE WAS GOING TO GO TO THE DOCTOR TO HAVE HIS BACK CHECKED OUT.
THE VICTIM WAS OPERATING A WATER TRUCK IN THE PLANT AREA AFTER FILLING UP WITH WATER HE BEGAN TO GO UP A HILL APPERENTLY HE HAD THE VEHICLE IN TO HIGH RANGE OF A GEAR AND THE TRUCK STALLED W ITH ENGINE DEAD THE BRAKES WOULD NOT HOLD SO THE TRUCK BEGAN TO ROLL BACKWARD THE DRIVER DECIDED TO TRY AND TURN THE TRUCK INTO A STOCKPILE TO STOP IT ONCE THE THE TRUCK GOT SIDEWAYS IT FLIP
VICTIM WHILE WORKING DUMPING STORAGE BINS AT THE PLANT SIRER RECEIVED SOME TYPE OF FOREIGN IRRITANT IN HIS RIGHT EYE.THIS IRRITANT MOST PROBABLY AIRBORNE DUST PARTICLES GOT UNDER HIS CONTACT L ENS CAUSING A SCRATCH TO OCCUR ON THE SURFACE OF THE EYE.
