Employee was operating a haul truck and hadn't notice a loader had stop to let him go by (both haul truck and loader were going in same direction). The haul truck's catwalk, on the driver's side, came in contact with the side of the loader bucket which pushed the hand rail of the catwalk through the driver side (open) window, which hit the employee on his nose and broke it.

IN PROCESS OF DUMPING ONTO STOCKPILE,EMPLOYEE BACKED TOO CLOSE TO EDGE OF EMBANKMENT ON TOP OF FRESH MATERIAL.AS TRUCK BED RAISED,WEIGHT OF MATERIAL TRANSFERRED TO THE REAR WHICH CAUSED EMBANK MENT UNDER THE RIGHT REAR DRIVERS TO SHIFT AND THE TRUCK TO TIP ON ITS SIDE.
EMPL CLAIMS OPERATING HEAVY EQUIP CAUSED CHRONIC BACK PAIN. EMPL FIRST BECAME AWARE OF INCIDENT ON 10/22 CLAIM IS UNDER DISPUTE. LAST DAY WORKED 10/02
