Employee was welding and some of the weld dripped onto his glove and burned through his glove onto his finger.

employee was cleaning under a shaker deck when he heard the start-up alarm go off and when he stood up to see what was going on he hit his back on a metal beam. (this injury happend on Fri. 10/3, but he did not report it until Tue. 10/7)

Employee was operating a haul truck and hadn't notice a loader had stop to let him go by (both haul truck and loader were going in same direction). The haul truck's catwalk, on the driver's side, came in contact with the side of the loader bucket which pushed the hand rail of the catwalk through the driver side (open) window, which hit the employee on his nose and broke it.

Employee was shoveling material from around a tail pulley, when a clump of material fell onto his foot, spraining his ankle.

Employee was in the Bone Yard climbing up on a piece of equipment when he slipped and fell to the ground fracturing his left femur.

Employee was removing rocks from the Jaw Crusher when he felt a pain in his shoulder.

Employee was putting a piece of metal on the back of a truck when his finger was caught between the metal and the bed of the truck. Employees finger was smashed and two blood blisters formed.

On July 28, EE was changing screens in a shaker and scratched his elbow and finger. On Aug. 8 EE complained that his finger was beginning to bother him and went to the Doctor. The Doctor requested he return on Aug. 10. On Aug. 10 the Doctor determined that his finger was infected and had his finger lanced.

Employee was walking down stairs and slipped and fell.

Employee was attempting to un-hook a com-a-long from a chute and cut his arm on the chute.

Employee had taken a sample of Wash-Concrete Sand and when he stepped off the back of his truck, he twisted his knee.

Employee noticed a piece of metal that was on a conveyor belt (that just had gone through a crusher) and went to pick it up, the metal was hot and burnt two of his fingers.

EE was replacing a starter in the MCC room. EE was removing the ground wire from the damaged starter when the wire accidentally touched the back of the starter next to it; there was a flash/spark. EE complained of seeing spots but continued to work and finished his shift. The next day EE informed us that his eyes were bothering him and he was going to see a doctor.

Employee grinding on a wedge, when employee tried to reposition himself, he slipped and the grinder wheel came in contact with his finger.

Employee was shoveling, when he slipped and fell, spraining his right thumb.

Employee was shoveling material and felt pain in his upper right arm.

Employee was opening a drawer in the lab. The drawer required additional pressure to open, employee pulled harder and felt a strain in her groin area.

Employee was operating a 50ton haul truck and had started dumping a load of sand when the haul truck leaned to one side at which time the operator retracted the bed of the truck which caused the truck to jerk. The operator complained of soreness to his back and shoulder.

Swinging a sledge hammer at a wedge in a cone crusher and missed the wedge. While swinging through, he felt a sting in his upper left shoulder.

Crusher liner fell on employee's left leg causing pain in left knee. This accident happened on 5/23/06 but employee did not go out on injury until 5/27/06.

Climbing up ladder to enter cab of Haul Truck, felt pain in lower back.

Slip/Fall. Sprain to right ankle

Driving haul truck - back pain.

Employee was cleaning out a chute that had build-up of material with a shovel. He stabbed at the build-up with the shovel and felt pain in his arm. Reported injury on 3/29/05.

EE PARKED HAUL TRUCKS SIDE BY SIDE THEN STEPPED ACROSS FROM PLATFORM TO PLATFORM. EE SLIPPED & FELL DOWNWARD BETWEEN PLATFORM CAUSING INJURY TO LEFT SIDE.

EMPLOYEE WAS STEPPING AWAY FROM WATER TRUCK CAB WHEN HE SLIPPED AND FELL TO THE GROUND, HIT HIS REAR END FIRST THEN HIS SHOULDER.

EMPLOYEE WAS DOING CLEAN-UP IN THE AREA OF #2B CONVEYOR (STAND AND CRUSHER UNDER BELT), WHEN A 4" - 8" ROCK CAME FROM ABOVE AND STRUCK HIM ON THE RIGHT FOREARM CAUSING CONSIDERABLE SWELLING AN D PAIN. HE WAS TAKEN TO DOCTOR.

EE WAS ATTEMPTING TO PRE TRIP A WATER TRUCK SERVICE THE TRUCK REQUIRED OPENING THE HOOD EE BELIVES ABOUT THREE WEEKS AGO HE FELT A PAIN IN HIS STOMACH WHILE OPENING THE HOOD.WENT TO HIS OWN DOCTOR ON 10-24-02 REPORTED TO CO. 10-25-02

WINDY CONDITIONS CAUSED WIND BLOWN DUST TO GO AROUND EYE PROTECTION & INTO EE'S EYE. INJURY WAS FIRST AID UNTIL 6/26/02 REQUEST FOR MEDICAL.

WHILE STEPPING OFF OF TRUCK ONTO A PLATFORM TO CHECK OUT SPARKS COMING FROM A CONVEYOR, EE SLIPPED AND FELL 8 FEET AND HIT BOTH KNEES.

EE WAS HELPING REPLACE SCREENS, ATTEMPTING TO REMOVE A WEDGE WITH A 4LB. SLEDGE HAMMER IN A HARD TO REACH LOCATION. HE INADVERTANTLY HIT THE RUBBER DONUT. THE HAMMER BOUNCED BACK & HIT HIS FIN GER AGAINST THE SHAKER SCREEN BASE.

EMPLOYEE WAS STEPPING OFF HIS TRUCK ONTO A PLATFORM WHICH HAD CONTROLS USED TO FILL TRUCK FROM A STORAGE BIN INTO THE TRUCK TRAILER. THERE WAS A GAP AND EMPLOYEE FELL TO THE GROUND HURTING HIS SHOULDER.

LIFTINGS PROCEDURES USED TO LIFT SCREENS, EE WAS LEANING OVER A SPRAY BAR USING HIS CHEST AS LEVERAGE TOLIFT UP SCREENS WITH BOTH HANDS ON A SET OF HOOKS ATTACHED TO THE SCREENS TO PULL IT UP WHEN EE HEARD A POP AND FELT PAIN IN HIS CHEST.
