Miner was conducting EE's plant walk facing the sand screw, which is a noisy area due to the shaker decks above, EE stated EE heard the skid steer operating, but when EE turned to EE's left to see where it was the skid steer knocked EE off EE's feet and into the bucket, causing shin and elbow injuries.

Operator of a 777F haul truck drifted into haul road berm, momentum and inertia of truck carried over into an impoundment. Operator sustained minor injuries, but taken to hospital to error on the side of caution and released from hospital after observation.

Miner was operating a 777 haul truck and hit a pothole in the haul road causing an injury to lower back. Truck was empty at the time.

Miner was in a boom lift with another miner who was operating it, miner was holding on the outside railing with right hand when the lift basket bumped into the the crusher they were working on and pinched right thumb.

Miner was operating haul truck and while being loaded a large boulder struck the inside of the bed jerking the haul truck and operator cab side to side. As a result, the miner in the haul truck cab reports headache, neck and back pain.

Tipped water truck on the side. neck strain, lumbar strain.

The miner was locking the pins in on the skid steer for another miner. The injured miner locked the pins on the skid steer and stepped back. When the operator of the skid steer moved the bucket the bucket fell off causing a contusion of the left foot.

The haul road was slick due to rain. The haul truck lost control and rolled over on right side. The operator was knocked out for 2 to 3 minutes at time of incident. After further exam at clinic is right wrist was sprained.

Tipped water truck on side. Laceration on back required stitches.

Mobile Equipment Incident.

Injured was helping on a project to fix an electrical problem and needed the man lift to reach. The area he drove equipment down was loose dirt and rather steep. He turned it too tight and lost traction thus sliding and making an abrupt stop on a dirt bank up against the gravity take up guard, throwing him around inside the basket.

While pulling wire through a conduit pipe on a 500' run with a p/u truck, the rope we used to pull the electrical wire with was tied in a square knot around a 6" C-clamp which was attached through one of the loops on the trailer hitch assembly. The clamp twisted off the hitch assembly & hit the employee above the right knee on the side of his leg leaving a very small cut.

Loader rolled down an embankment about 40' several minutes after the operator set the hydraulic brakes, the loader had the forklift assembly attached and it pinned the employee between the forks and the guard assembly surrounding the primary incline belt gravity take-up, EE sustained cuts to his right middle finger and right leg with a possible broken left ankle.

EE HAD BEEN WORKING ON A PIECE OF EQUIPMENT WHEN HE WAS DONE HE DROVE AROUND THE EQUIPMENT ONTO THE HAUL ROAD WITHOUT LOOKING AND WAS HIT BY THE HAUL TRUCK.

THE INJURED EE WAS A PASSENGER IN A PICKUP TRUCK WHICH ROLLED OVER WHILE BEING DRIVEN DOWN THE PIT LINE ROAD.

ADJUSTING THE GATE IN THE PIT 1 TUNNEL WHEN THE EMPLOYEE SAYS A ROCK CAME OFF THE BELT AND BRUISED HIS BACK. *NOTE THE BELT IS CHEST HIGH AND WE ARE UNSURE AT THIS POINT HOW THIS COULD HAVE H APPENED-HITTING HIM IN THE BACK.

EMPLOYEE WAS WASHING MATERAIL OFF OF THE CINVEYOR BELT WHE IT ROLLED BACK. THE MOVEMENT KNOCKED HIM DOWN AND TRAPPED HIM BETWEEN THE CHUTE AND THE CONVEYOR BELT.

EE WAS ATTEMPTING TO PULL A BELT WIPER BACK INTO POSITION WITH THE BELT RUNNING. THE WIPER ARM HIT HIM ON THE THUMB.
