While sitting in his 100-ton CAT haul truck employee was reaching for his lunch pail while getting loaded. When the loader dumped the shot into the truck bed the truck shook violently causing the employee's shoulder to be jarred upwards while he was reaching for his lunch pail.
No accident date. Employee devloped pain in both arms over time asa result of operatinga loader. Employee returned to work in a restricted capacity for a a period of 10 working days unless otherwise instructed by treating phsycian.
Employee complaining of pain in his right low back radiating down his leg with numbness. There is no known cause.
While operating loader, felt eye irritation. Used eye wash w/o relief, ended up going to clinc to have eye irrigated. 8/30/05: REPORTABLE - EMPLOYEE RECEIVED A PRESCRIPTION.
Was on man lift. When decending, the basket caught a pipe and then jolted employee against side, injuring his right shoulder & upper arm.
Was dumping loader into crusher and experienced pain in elbows and forearms, employee worked with this pain for a few weeks, and it never got better- he waited til the end of july to seek attention

EE CLIMBED UP AND ATTEMPTED TO REMOVE THE STONE. CLOTHING WAS CAUGHT AND EE SUFFERED BONE FRACTURE AND TENDON INJURY. Y
CLAIMED HE DEVELOPED BACK PAIN WHILE OPERATING A DUMP TRK. EE HAD PREVIOUS BACK SURGERY & WAS INVOLVED IN A MOTOR VEHICLE ACCIDENT WHILE OUT OF WORK. ONCE A NEGATIVE MRI WAS OBTAINED HE WAS RETURNED TO FULL DUTY
CLEANING YARD WITH BOBCAT, WHILE EMPTYING BUCKET, DUST/DEBRIS CAME BACK AT EMPLOYEE. A PIECE OF METAL WAS IN THE DEBRIS AND ENTERED EMPLOYEE'S EYE REQUIRING REMOVAL.

EE CLAIMS THAT PROBLEMS WITHT HE SEAT IN HIS TRUCK CAUSED HIM TO SUSTAIN A STRAIN TO HIS NECK AND BACK. EE BEGAN TO LOSE TIME FROM WORK AS OF 5/30/00.
