Miner operating a UTV too close behind a haul truck. The haul truck stopped and reversed direction. The bed of the truck struck the windshield and ROPS of the UTV. Miner injured right arm during the collision.

Mostrando los 6Haul truck operator drove over a section of uneven ground and was not wearing a seat belt. The operator bounced up and hit head on the roof and hit knee on the dash. Operator was taken to the hospital for observation. Operator has been placed on modified duty.
Preliminary Investigation: The operator turned his head to face forward while applying brakes. The braking process resulted in his nose striking the canopy.
