While lifting angle iron with a telehandler, a fork caught on a welded piece of metal on the side of the scale platform. When the weld broke, the fork sprang upward, jolting the angle iron upward, which struck the mechanic's jaw. First aid was applied, but on 5/21/25 EE reported ongoing pain. On 5/27/25, a CT scan revealed a closed fracture of the right maxilla.

As the roll was coming off the forks of the telehandler, it gained more momentum than planned and the roll pushed the employee's lower legs against the air tank reservoir behind them.
A rock came off of a conveyor and hit employee on left arm. Arm was bruised.
THE EE WAS WORKING ON A CONVEYOR WHEN HIS HANDS WERE CAUGHT IN THE CONVEYOR. BOTH HANDS WERE BRUISED AND SCRAPED.
EMPLOYEE STATES HE WAS LIFTING HOPPER WITH 980 LOADER WHEN PICKING EYE BROKE A PIECE AND HIT HIM IN THE JAW KNOCKING HIM DOWN. HE LANDED ON HIS SHOULDER ON THE GROUND.
