Mostrando los 21Employee was operating a loader, as he pushed the loader bucket into the stockpile, he stopped. During that time a rock rolled from under the bucket to under the left front wheel. As the operator backed away from the stockpile the left front tire drove over the rock, causing the tire to slam down on the ground.
Employee backed truck into ballard, causing tightness in his lower to mid-back
Employee was in manlift when the bottom portion of the cage came in contact with the bolts on the outside of the Finish Mill. This caused the manbasket to jerk, causing the employee to hit his left side on the hand rails of the manbasket. Employee was wearing harness with secured lanyard while in manbasket.

While operating a loader the employee was being bounced around in his seat and felt a pain in his lower back.
The employee stopped his truck in order for another person to get out. When the employee got out he shut the door and that activated the drivers side air bag. The air bag then made contact with his right arm burning and lacerating it.
Employee was working with the vacuum truck and when the truck (vacuum) was engaged, the sudden jerking of the hose hit his right hand that was on the truck and lacerated it. Employee was offered modified duty but elected to take a weeks vacation.
THE EE WAS PUSHING AN ELEC. CART, WHEN HE FELT A PAIN IN HIS RIGHT ELBOW.
THE EMPLOYEE WAS A PASSENGER ON AN ELECTRIC CART THAT STRUCK A POLE AT THE BOTTOM OF AN INCLINE. THE DRIVER APPLIED THE BRAKES AND THEY LOCKED UP, CAUSING THE CART TO SLIDE INTO A POLE AT THE BOTTOM. THE INJURED EMPLOYEE BRACED HIMSELF BUT THE CART STRUCK THE POLE IN THE AREA OF HIS LEFT FOOT, CAUSING A SPRAIN.
EE WAS OPERATING FORKLIFT ALONGSIDE RR TRACKS. THE AREA WHERE THE LIFT WAS BEING USED WAS UNEVEN AND THE FORKS WERE NOT AT THE PROPER HEIGHT, HITTING THE RR TIE, CAUSING EE TO BOUNCE FORWARD INTO STEERING WHEEL
EMPLOYEE WAS DRIVING A DUST TRUCK RETURNING FROMOUR DUST DUMP, WHEN HE TURNED THE TRUCK OVER ON IT'S SIDE. THE DRIVER SUFFERED MINOR ABRASIONS TO HIS LEFT ELBOW. THE DUST TRUCK WAS A TOTAL LOS S.
EMPLOYEE FELL FROMA MANLIFT ELEVATOR WHILE DESCENDING. IT IS UNCLEAR AT THIS TIME AS TO THE CAUSE OR HOW FAR HE FELL.
THE EE WAS RIDING IN THE REAR SEAT OF A TRACKMOBILE THAT WAS TRAVELING DOWN THE TRACK TO PICK UP A LOADED SET OF COAL CARS. THE OPERATOR STRUCK THE CARS AT A HIGH RATE OF SPEED THROWING THE IN JURED EE INTO THE INSIDE FRAME OF THE CAB. HE LACERATED HIS LEFT EAR.
THE EE WAS A PASSENGER IN A PICK-UP THAT STRUCK A DIRT PILE ON THE HAUL ROAD. HE STRUCK HIS HEAD CAUSING A LACERATION OVER HIS LEFT EYE.
THE EMPLOYEE WAS GREASING A BELT DR PULLEY BRG WHEN HE DECIDED TO CHECK THE BELT TENSION. HE PUT HIS LEFT THUMB ON THE BELT, THE BELT CAUGHT HIS THUMB AND PULLED IT BETWEEN THE PULLEY AND BEL T, AS A RESULT HE LOST HIS THUMB NAIL.
EE WAS HAULING A LOAD OF KILN DUST TO OUR DUMP SITE AS HE TURNED ONTO THE HAUL RD EITHER THE LOAD SHIFTED OR THE TRAILER SKIDDED ON SOME LOOSE MATERIAL AND TIPPED OVER. THIS CAUSED THE TRACTO R TO ALSO TIP OVER. THE TRACTOR WAS TRAVELING ATA NORMAL RATE OF SPEED & SPEED DID NOT APPEAR TO BE A FACTOR.
EE CLAIMS THAT THE ROUGH RIDE IN AN 85 TON HAUL PAK CAUSED A STRAIN IN HER BACK AND SHOULDER
EE WAS REBUILDING PALLETS. HE INSRUCTED ANOTHER EE OPERATING FORKLIFT TO REVERSE FORKLIFT. WHEN 2ND EE RELEASED HAND BRAKE, LIFE MOVED FORWARD. PINNING CHAVEZ' RIGHT LEG BETWEEN PALLET ON FLOO R AND PALLET ON FORKLIFT.
