Employees were installing a high voltage cabinet on the shovel carbody. A skid steer was being used to hoist the cabinet sitting on a wood pallet into position. The injured employee was on top of the carbody reaching over to align the cabinet, when the injured employee signaled to lower the cabinet it bounced pinching the fingers between the mounting ledge and the cabinet.

Haul Truck driver was being loaded when the first dipper released into the truck a large boulder struck the bed jarring the employee. EE was taken to the ER and was given time off work.

EE was jarred while operating haul truck. EE notified supervisor, but initially refused any medical treatment. EE worked her last shift on 4/18/2012 was off two days and came back for her first scheduled work shift on 4/21/12 and requested to be taken in to clinic. EE was placed on restrictions.

Employee was involved in an accident where the vehicle was tipped on it's side. Employee was transported to hospital. The injury sustained was a fracture to the arm.

EE was being loaded when the bucket of the shovel opened dropping muck on the dove tail. EE was taken to the ER and taken off work for one day.

On 10/6/10 EE notified sup and sr. sup during a meeting that he had an open Workers' Comp claim for his back. EE alleges that he injured his back on, but is unsure as to the specific date, but says that it occured on one of his dayshifts Sept 22-24, 2010. EE was also not sure as to what event caused his back pain .... This is under investigation ...

EE hit a bump while driving haul truck and the seat bottomed out, jarring EEs back. EE was taken to the ER to be observed and taken off work for one (1) day.

EE was being loaded and was jarred in the haul truck when some muck hit the dove tail. EE was taken off work.

EE was driving the TS22 Forklift to the 25 Shovel assembly site. When he turned the fan on some dust blew into his right eye. EE was taken into the clinic and referred to eye doctor. A foreign body was removed from and a presciprtion eyedrops were given. Employee was released back to work.

EE was mounting haul truck when her right foot slipped on the ladder. EE tried to catch herself and jammed her right arm in the process. EE was taken into the ER and diagnosed with a lower right arm sprain. EE was taken off work.

The ee entered the manvan at shift change and proceeded to the rear seat. The ee put his seatbelt on and the man van proceeded out of the main pit. The manvan struck a hole in the road and the ee head was jarred into the roof of the manvan. EE was checked by ERRT team to have ee checked out and transported to the ER.

EE was driving a company vehicle up an incline on the SX access road when he fell asleep at the wheel. The vehicle climbed up on the left side of the berb, traveled approx. 75 feet and came to a stop on top of the berm. EE was taken to the ER he was diagnosed with whiplash. EE was taken off of work.

EE was being loaded by the loader when a rock rolled out of the dipper into the bed of the haul truck, jarring the employee. EE alleges that when he was jarred he began to experience back pain. EE was checked by a first responder and then taken into the ER. EE was released back to work with 2 days restricted duty. EE worked his last day in the rotation on restriced duty status.

Operator of Caterpillar 735 haultruck (#30) was coming off 1 Dam to 1x Dam. Just before the ramp he hit a small pile of dirt in road which jarred his left shoulder.

EE alleges that he experienced back pain while he was being loaded. He was taken to the ER and taken off work. ....Under investigation....

Employee was loading truck with loader when the boom fell jarring the employee. He was taken into the ER and taken off work. Ongoing.

Employee was traveling to the SX on the access road when an oncoming contractor vehicle swerved to miss some deer on the road colliding head on with the employee. Employee was transported to the hospital via ambulance. Employee suffered from whiplash and contusions to her chest area. She was given pain medication and taken off work for two days.

Employee's scraper came to a sudden stop when the slack was released after the hook up. Afterwards, employee began to feel some discomfort in his neck and head. He was transported to the ER where he was checked out. He was taken off work. RTW 2/21/07.

Employee was taking banding outside to the dumpster when he waved the forklift operator down so he could get employee another bundle. As forklift operator was leaving, the banding got caught u nder the tire running over employee's left foot. On Going.

Employee was traveling to the fill loaded when he hit a low spot in the road. When employee felt pain in back. Employee was taken into the ER and was given two days off work. Employee then went on regular days off and returned on next regular scheduled shift on 8/19/06.

Employee was returning to Tyrone from Chino in the S1 and was rear ended at the corner of Hudson and Broadway, causing a cervical strain. Employee was taken to the hospital and given prescriptions for pain and release to work on 4/6/06.

While operating a 631, # 12 scraper, EE was loaded and headed to the fill. As he came out of the 8 ditch onto level ground, he lost control of the scraper and ran over a windrow on the left side of the road. After traveling about 20 yards, EE got control and crossed the windrow back onto the road to his right and pulled the scraper off the road to the right side of the road.

OPERATOR ON THE 637 SCRAPER WAS RETURNING TO BARROW PIT AFTER DUMPING HIS LOAD. THE AREA OF THE RETURN ROAD THAT OPERATOR CHOSE TO RETURN ON WAS BEAT OUT FROM PRIOR TRAFFIC. OPERATOR WENT THROUGH 3 WELL DEFINED POTHOLES, JARRING HIM. THIS RESULTED IN A FRACTURE TO THE 12TH VERTEBRAE. EMPLOYEE WAS TAKEN OFF WORK.

Operator on the 621 water wagon was crossing scraper fill are when she ran into, causing a bouncing motion which in turn caused her head to come in contact with the top of the cab. This resulted in a fracture to the 12th vertebrae. Ee was taken off work currently ongoing.

EE WAS BEING LOADED AND IS UNSURE IF THE DIPPER OR A ROCK HIT THE SIDE OF THE RIGHT SIDE OF THE BED OF THE TRUCK.

EE WAS OPERATING A LOADED HAUL TRUCK WHEN HE HIT A POT HOLE IN THE ROAD.

THE EE WAS BEING LOADED BY AN ELECTRIC SHOVEL WHEN THE TRUCK HE WAS OPERATING WAS HIT BY A LARGE BOULDER THAT FELL FROM THE DIPPER. THE SUDDEN MOVEMENT OF THE TRUCK CAUSED A STRAIN TO THE EE' S LOWER BACK.

EE WAS OPERATING 789 HAUL TRK WHEN HE WENT TO SET HIS WATER JUG ON THE PASSENGER FLOOR BOARD. HIS HAUL TRK HIT A POT HOLE IN THE SHOVEL PIT THE JARRING MOTION CAUSED HIM TO SUFFER A MUSCLE STRAIN TO HIS LOWER LEFT BACK

HE WAS RETURNING FROM THE STOCKPILE TO THE SHOVEL WHEN HE HIT A BUMP IN THE SHOVEL PIT AND STARTED COMPLAINING OF LOWER BACK PAIN. HE WAS OPERATING A CAT 789-B HAUL TRUCK.
