While scaling in 33 West, a rock fell from the back and struck the scaler boom jolting the cab causing the operator to hit the windshield with EE's forehead and right shoulder. The operator was wearing a seat belt. EE as a mild concussion and a strained A/C shoulder joint. Scaler equipped with a lap belt style seat belt.

Employee was tightening a rail car puller cable clevis attachment under a loading point. Lime dust from the conveyor above fell and got into employees eye. He was wearing RX safety glasses with side shields. The dust was embedded and he received a prescription.

Operator backed over a rock at the dump site, turning the bed of the truck on its side. Truck bed was turned upright with no damage.

EE was moving high lift through the plant when the equipment went in a hole. This caused the EE to be thrown up in the air approximately 3 feet. When he came down he landed on his ankle, spraining it.

EE WAS LOADING A RAIL CAR WHEN THE EYE USED TO HOOK THE CAR PULLER CABLE BROKE LOOSE FRTOM THE CAR. WHEN THE CAR STARTED ROLLING, EE RAN OUT ON TOP OF THE CAR, ATTEMPTED TO CLIMB DOWN THE LADD AND USE THE HAND BRAKE TO STOP THE CAR. WHEN THE SAFETY BRAKE STOPPED THE CAR, EE LOST HIS BALANCE AND FELL 13' TO THE GROUND.
