While cleaning P92A ore pass grizzly in P91 at around 21:25 of plug reversing in 1st gear, hit rib with left rear corner of loader. Upon contact with rib, low speed, felt sharp pain in front right shoulder.

Driver of Haul truck sustained cervical sprain during a rough loading event where large rock material was concealed in muck of material.

While mucking in P75, employee was cleaning off the muck from the grizzly and the bucket of the LHD hit a rock stuck in the grizzly. The loader stopped suddenly causing EE's left arm/shoulder to hit door pad. This impact caused pain in the left shoulder that got worse on Sunday morning (6/20/21).
A haul truck operator struck a shovel bucket with the truck during an attempt to spot for loading. The driver suffered spinal muscle strains.
The operator of a Cat 789 Haul truck received a back injury when the seat in the truck bottomed out while crossing a dip in the haul road. The operator was not permitted to work until seen by EE's personal physician.
Employee was operating a skid-steer loader when hit a bump and jarred back.
The employee was inspecting the wheels and breaks on a train flat at the invert in the haulage tunnel. He stepped onto the parallel tracks to check the opposite side of the flat as an approaching train with a flat clipped and twisted the employee, causing his foot to be run over by the flat wheel.
Contractor was removing a 12" vacuum line used in the process to clean HVAC unit and ducts. His right hand was pinched between the vacuum line and HVAC unit access door. The suction from the vacuum line caused his hand to be pinched against the latch on the HVAC unit access door resulting in 3 laceration on back of contractor's hand. Lacerations required stitches at hospital.
While towing a rod trailer down a decline employee lost control of Skidder it stalled. Employee dropped the blade, as it hit the ground, rod trailer caused skidder to start to jack knife he then chose to unbuckle and jump from skidder. When employee jumped, he landed on his feet with his feet slipping out from under him and then landed on his side.
While operating 994, employee felt stiffness in his neck while turning his head.

Employee reached from cab of mobile equipment to shut off a water spray valve. The mobile equipment moved, and pinched operator's forearm between rib, door, and door frame. This resulted in fractures and lost workdays.
While performing an emissions test under a pipe truck, the engine was revving and it slipped into gear rolling forward and spinning him under the truck.
Employee was riding in buggy returning to the elevator shaft. Buggy pulled to the side to allow equipment to pass. Driver released the brake to pull forward, the buggy drifted backwards, smashing the employees hand between the rail of the buggy and the rib.
The employee was in his personal vehicle leaving the property. He collided with another employee by the front gate. Severe weather conditions prevented them from seeing each other. Initially, there was not an injury. Later an injury developed from the force of the airbag coming in contact with his face. The injury became reportable as of January 29, 2008.
IN THE PROCESS OF REMOVING A ROCK WHICH HAD BECOME STUCK IN A GRIZZLY, EE STRUCK THE ROCK WITH THE LHD HE WAS OPERATING. THIS CAUSED THE LHD TO STOP IMMEDIATELY AND CAUSED EE TO STRIKE HIS FACE AGAINST THE WINDSHIELD. (CONCUSSION & CERVICAL STRAIN)

ROUNDING CORNER PAST 44 CHUTE GOING TOWARDS 33 CHUTE, HE SLOWED FOR WATER ON ROAD. TURNING LEFTTRUCK VEERED RIGHT & STRUCK RIB ON RIGHT SIDE BY34 CHUTE ENTRANCE. FLAT TIRE & BROKEN STEERING ARM.
EE STRUCK STEEL SET WITH RIGHT FRONT CORNER OF LHD BUCKET. IMPACT BROKE OUT DOOR WINDOW & CUT EE FACE RESULTING IN STITCHES
EE WAS OPERATING A LOADER UNDERGROUND IN THE MINE. THE FRONT EDGE OF THE BUCKET HIT A STEEL RIB SET WHICH STOPPED THE VEHICLE ABRUPTLY. HE HIT THE DOOR FRAME BREAKING HIS HARDHAT AND VEHICLE W INDOW. THIS RESULTED IN A LACERATION ABOVE HIS RIGHT EYE. STITCHES WERE APPLIED WHEN HE ARRIVED AT LUTHERN MEDICAL CENTER. RELEASED TO COME BACK TO WORK.
