Emp. was loading haul trucks - he had just dumped a bucket in a haul truck and was backing away, lowering the hoist at the same time, the hoist did not respond so employee moved the lever forward and the bucket dropped and struck the ground. Emp. did work that full shift and worked next two scheduled shifts - he then went to the doctor on 5/17 - and the doctor took him off work.
Employee was attempting to reach for trouble light above conveyor when his right hand was pulled into the pinch point between belt and the idler. Resulted in open wound with fractures to right hand, wrist and all five digits.
EMPLOYEE OPERATING TRUCK, LOADER WAS LOADING STONE INTO THE TRUCK. AFTER LOADING LAST BUCKET HE BACKED AWAY & A STONE FELL FROM BUCKET, HITTING TRUCK MIRROR AND PROPELLING GLASS THROUGH THE TRUCKS OPEN WINDOW. EMPLOYEE HAD WINDOW OPEN - EYE STARTED HURTING 10 MINS AFTER INCIDENT.
EE WASA OPERATING HAUL TRUCK, WAS WEARING SEAT BELT, HE WAS LOADED WITH WASH POND SALT, LOAD WAS LOOSE AND SHIFTED TO BACK OF TRUCK BED CAUSING FRONT OF TRUCK TO COME OFF GROUND, WHEN LOAD RELEASED TRUCK'S FRONT END LANDED BACK ON THE GROUND.
EE WAS OPERATING LOADER--TRAVELING ACROSS SILICA BENCH WITH A BUCKET OF STONE AND RAN OVER A LARGE ROCK. THE QUICK SID TO SID AND BUMP STRAINED HIS RIGHT SHOULDER.
EE WOKE UP THE MORNING OF 1-21-02 WITH SEVERE BACK PAIN, HE THINKS IT WAS CAUSED FROM A JARRING THAT HAPPENED WHILE RUNNING JD LOADER THE PREVIOUS WEEK. NO SPECIFIC TIME OR DATE.
EE WAS IN HAUL TRUCK, WHILE HIS TRUCK WAS BEING LOADED THE LOADER BUCKET HOOKED THE SIDE OF THE TRUCK BOX. AS THE LOADER WAS BACKING AWAY THE BUCKET WHICH THEN RELEASED THE TRUCK WHICH SNAPPED BACK TO LEVEL POSITON, JOSTLING EE.
OPERATING #37 LOADER REPAIRING ROADS WHEN HE TURNED TO LOOK BEHIND HIM TO BACK THE LOADER UP AND THEN HE HEARD HIS SHOULDER POP.
LOADER OPERATOR ACCIDENTALLY HIT THE QUICK RELEASE LEVER ON LOADER BUCKET. IT DESCENDED AND STRUCK SIDE OF HAUL TRUCK, JARRING THE OPERATOR.
