Employee was working on a 922 loader installing bottom bucket pin. The pin was struck by forklift and bushing was forced from opposite side of pin carrier and struck the employee in the face. He was knocked back from machine, lip was cut and tooth was chipped. Employee reported this incident Friday 11-13-15 at 11:15 AM to his supervisor

The loader bucket popped over the top of it when he curled it back causing loader to lurch forward hitting muck pile jarring employee. This caused a pain in his lower back and legs. MRI on 6/19/13 shows 2 herniated discs. Evaluation of injury and surgeons is taking place. Lost-time status effective 6/24/2013.
Beginning on or about 9/11/07 we had a job loading 20,000 tons of a 2 x 8 inch rock. EE said the motion of his loader while loading that material made his upper back begin to hurt. He spoke of a "whip" as the bucket came back against the stops and having to stretch to operate the scale
Employee was operating a wheel loader loading pit trucks on 3-15-06, when he felt a small pain in his back on the left side. He continued to work and didn't report anything until the morning of 3-22-06.
EE was moving shot rock in the pit area. At the end of his shift he reported to his supervisor that his lower back was sore. He had previously had lower back surgery. EE was treated for a sprain in the lower back.
PETERBUILT TRUCK TURNED OVER ON IT'S SIDE WHEN TURNING A CORNER. RAIN AND LIGHTENING CONTRIBUTED. THE DRIVER HIT HER HEAD AND LEG. SHE WAS ON RESTRICTED DUTY.
A ROLLER ON P1A CONVEYOR BELT WAS SQUEAKING YSMAEE TRIED TO STOP THE ROLLER WITH HIS HAND TO SEE IF IT WAS THE ONE MAKING THE NOISE AND IT PULLED HIS ARM IN AGAINST THE BELT AND SKIRT BOARD AN D BROKE HIS ARM.
