Employee Claims: While operating a front-end loader and digging from a muck pile, a rock was thrown from the "Surestrike" rock breaker through the right side window of the loader. The rock struck the employee on the right side of the face.

We were notified by the employee that he was having health problems on 09/03/04. The employee then notified us on 10/07/04 that the health problems were work related.
