WHEN THEY APPROACHED THE STATIONARY CARS HE STEPPED OFF OF THE RAILCAR HE WAS ON TO SIGNAL THE LOCOMOTIVE OPERATOR TO STOP IN ORDER TO CONNECT THE CARS. HE STEPPED DOWN HE SPRAINED HIS KNEE. ALTHOUGH THE INJURY OCCURED ON THE 8TH
EE OVER TURNED A HAUL TRUCK WHILE DUMPING A LOAD OF SAND OVER A 20FT WORKING STOCKPILE FACE HE FAILED TO RECOGNIZE THE SEVERITY OF THE SITUATION & FOR SOME UNKNOWN RESAON ATTEMPTED TO DUMP HIS INITIAL LOAD 6FT FROM THE EDGE OF THE PILE WITHOUT FIRST INSPECTING THE AREA WHICH IS THECOMPANIES PRACISE FORTUNATELY HE REDUCED HIS DEGREE OF INJURY BY THE USE OF HIS SEATBELT TRUCK WAS TOT
HE WAS OPERATING THE LOCAMOTIVE WHEN HE APPROACHED A SET OF CARS THE LOCAMOTIVE FAILED TO STOP IN TIME CAUSING IT TO SLAM INTO THE CARS. I NAME WAS HAULING BALLAST FROM MILL T O A STORAGE PILE IN A 35 TON HAUL TRUCK AS HE REACHED THE TOP OF THE PILE HE BEGAN TO BACK THE TRUCK UP TO DUMP IT WAS DETERMINED T HAT THE SPEED OF THE TRUCK & THE FAILURE TO POSITION THE TRUCK PROPERLY CAUSED IT TO FALL ON ITS SIDE HAD HE POSITIONED THE TRUCK ON THE FLAT AREA OF THE PILE BEFORE HE BACKED UP THE ACCIDENT WOULDNT HAVE OCCURRED
WHILE HAULING L3ME OUT OF OUR SUMP PIT HIS TRUCK BEGAN TO SLIP ON THE WET RAMP SO HE PUT ON THE BREAKS TO HOLD HIM. UPON DOING THIS THE ENGINE STALLED CAUSING HIM TO LOOSE HIS POWER STEERING. AS HE ROLLED B5CK OUT OF CONTROL THE BREAKS FAILED TO HOLD HIM WHERE AS HE ROLLED TO THE BOTTOM OF THE RAMP AND THE TRUCK TURNED OVER ON ITS RIGHT SIDE. HE WAS NOT WEARING SEAT BELT AT THE T
