EE WAS WORKING ON THE RAIL CREW CLOSING THE AIR BREAK SYSTEM WHEN HIS HAND SLIPPED WHILE TURNING A VALVE. WHEN HIS HAND SLIPPED HIS BODY PUSHED FORWARD ONTO THE KNUCKLE OF THE CAR INJURING HIS RIBS. ALTHOUGH THE INJURY OCCURRED ON THE 19TH, HE CONTINUED TO WORK THE REST OF THE WEEK. HE DIDN'T MISS WORK UNTIL THE FOLLOWING WEEK AND WDI WASN'T NOTIFIED OF THE SITUATION UNTIL 8/26/03.

Mostrando los 14EE WAS CLEANING UP STONE IN THE "OLD TUNNEL" USING A SHOVEL. WHILE SHOVELING STONE FROM THE FLOOR AND PLACING IT BACK ONTO A CONVEYOR HE FELT SOMETHING PULL IN HIS STOMACH. HE HAS RETURNED TO WORK WITHOUT MISSING ANY TIME BUT WITH RESTRICTIONS. HE WILL REQUIRE AN OPERATION TO FIX THE HERNIA IN THE FUTURE.
EE FIRST INJURED HIS LEG WHILE CHANGING ROLLERS ON A STACKING CONVEYOR WHEN HE WALKED INTO THE NEW ROLLER THAT WAS BEING MADE READY TO BE REPLACED. HE COMPOUNDED THE INJURY LATER AT 10:00 P.M. WHEN HE BUMPED THE SAME LEG ON THE C500 CRUSHER WHILE PUTTING ON SIDING.
EE WAS CUTTING A GUARD SUPPORT WITH CUTTING TORCH WHEN HE GRABBED THE HOT ANGLE SUPPORT THAT HE HAD JUST CUT WITH HIS HAND. HE WAS NOT WEARING GLOVES AT THE TIME OF THE INJURY.
SOMETIME DURING HIS SHIFT HE GOT A FLAKE OF METAL IN HIS EYE. HE WORKS OUTSIDE ON THE RAILCREW & IT WAS A VERY WINDY DAY. HE IS NOT SURE WHEN THE FLAKE BECAME LODGED. IT HAD TO HE REMOVED BY A DOCTOR.
EE WAS WORKING IN THE #2 BIN, KNEELING ON THE 304 CHUTE, LIFTING GRIPSTRUT TO PLACE UP FOR A WALKWAY. HIS SUPERVISOR WAS NOT NOTIFIED OF THE INJURY UNTIL THE NEXT DAY, JANUARY 30, HE DID NOT GO FOR MEDICAL ATTENTION UNTIL MONDAY, JANUARY 3
EE WAS WORKING IN OUR 'OLD TUNNEL' PICKING UP ROCKS THAT HAD FALLEN OFF THE TUNNEL CONVEYOR. AS HE PICKED UP A LARGE ROCK OFF OF THE FLOOR TO PLACE IT BACK ONTO THE IDOL BELT HE STRAINED HIS L OWER BACK. HE HAD HAD A PREEXISTING BACK PROBLEM THAT ORIGINATED FROM A TRAFFIC ACCIDENT IN OCT 2001. LIFTING OF THE ROCK FURTHER AGGRAVATED HIS CONDITION. LOST TIME DID NOT OCCUR TILL 12/17
A MAINTENANCE CREW WAS WORKING UNDER THE PRIMARYCRUSHER FIXING A SUPPORT BEAM WHICH REQUIRED ATTENTION. EE WAS STANDING ON A HORIZONTAL BEAM 2' OFF THE FLOOR. AS HE STEPPED DOWN OFF THE BEAM HE SCRAPPED OR LACERATED HIS LEG ON THE BEAM.
WHILE CLOSING THE DOORS ON AN OPEN-TOP HOPPER RAILCAR HE STEPPED INTO AN ANIMAL HOLE CAUSING HIM TO TRIP, LOOSE HIS BALANCE AND FALL. IN AN ATTEMPT TO BREAK HIS FALL CAUSED HIM TO SPRAIN HIS S HOULDER ON IMPACT.
EE WAS WORKING IN THE SHOP ATTEMPTING TO SEPARATE A COUPLER FOR #59 LOCOMOTIVE WITH A HAMMER AND A CHISEL. NORMALLY THIS COUPLER SEPARATES QUITE EASILY, HOWEVER, HE HAD NOT REMOVED ALL OF THE BOLTS HOLDING IT TOGETHER. HE FIRST DROVE THE CHISEL INTO THE COUPLER WITHOUT SUCCESS, THEN HE PRIED ON IT, AND AS HE MOVED AROUND THE COUPLER CHISEL, UNDER PRESSURE SHOT OUT AND HIT HIM IN TH
WHILE PERFORMING HIS REGULAR DUTY OF INSPECTING RAIL CAR DOORS, EE USED AN UNORTHODOX AND UNSAFE PROCEDURE TO CONDUCT HIS TASK. HE STATED THAT WHILE WORKING IN THE DARK, HE HAD PLACED HIS FLAS H LIGHT IN HIS MOUTH TO HOLD IT WHILE HE INSPECTED THE CAR DOORS. HE THEN SAID HE SLIPPED AND FELL, CAUSING THE FLASHLIGHT TO HIT THE CAR, DAMAGING HIS TEETH AND INJURING HIS SHOULDER AS HE A
EMPLOYEE WAS HELPING TO EXTRACT A HAUL TRUCK THAT HAD GOTTEN STUCK IN THE MUD. HE INJURED HIMSELF WHEN HE LIFTED A CABLE, THAT WAS GOING TO BE USED, THE WRONG WAY.
EE WAS DESCENDING FROM A 35 TON HAUL TRUCK; AS HE BEGAN TO TURN HIMSELF AROUND TO GO DOWN THE STEPS, HIS FEET SLIPPED OUT FROM UNDERNEATH HIM. HOLDING ONTO THE RAILING CAUSED HIS BODY TO SLAM INTO THE STEPS (WHERE THE INJURY OCCURRED) AT WHICH POINT HE LET GO OF THE HAND RAIL AND FELL TO THE GROUND.
EE WAS SHOVELING SAND OUT OF THE BOX ABOVE THE ISC-82 CRUSHER. THE SAND WAS WET AND PACKED AND BECAUSE HE WAS REACHING OVER THE GUARD RAIL TO PERFORM THIS TASK HE PULLED A MUSCLE IN HIS FOREAR M. ALTHOUGH THE INCIDENT OCCURRED ON 4/19, THE COMPANY WAS NOT NOTIFIED THAT HE HAD OBTAINED MEDICAL TREATMENT UNTIL 5/2/.
