THE VICTIM TRAMMED A SCOOP TO A PALLET OF ROCKDUST. HE POSITIONED HIMSELF BETWEEN SCOOP AND ROCKDUST SO AS TO LOAD DUST ONTO BATTERY END OF SCOOP. WHILE LANDING ROCKDUST THE SCOOP DRIFTED BACK WARDS MASHING HIS LEGS BETWEEN SCOOP AND PALLET.
EE WAS TRANSPORTING A PIECE OF SHEET METLA IN THE OUTBY END OF A TRACK OPERATED PERSONNEL CARRIER. THE METAL VIBRATED LOOSE, STRUCK COAL RIB AT A CLOSE CLEARANCE AREA, CAME AROUND AND STRUCK V ICTIM.
INJURED WAS PUTTING A NEW SET OF BOLTS ON A VIBRATING SCREEN AND AS HE WAS TURNING THE SHEEVE WHEEL WITH A PIPE WRENCH, THE BELT WENT ON THE SHEEVE WHEEL. IT TURNED BACKWARDS AND CAUGHT HIS FI NGER ON THE MECHANISM CUTTING HIS FINGER.
THE 5&6 BELTA SHUT DOWN AND VICTIM TELEPHONE OUTSIDE AND WAS TOLD PROBLEM WAS AT #5 DRIVE UPON ASSIVING AT THE #5 DRIVE HE FOUND THAT COAL AND ROCK HAD PILE UNDER THE DRIVE ACTIVATING THE GOH SWITCH THUS SHUTING DOWN THE BELTS. THE VICTIM TOOK A PIECE F DRILL STEEL AND BEGIN PUNCHING THE MATERIAL LOOSE DOWN ONTO #4 BELT WHEN SUDDENLY SOME OF THE ROCK SLID DOWN ONTO STEEL PUSHING I
EMPLOYEE STATED HE WAS TRAMMING THE #4 MBC INTO THE FACE OF #1 HEADING- THERE WAS A ROCK IN THE ENTRY THAT THE MBC TRAMMED UPON- THEREFORE CAUSING THE DOCK TO RAISE- BUMPED HIS HEAD AGAINST TH E TOP AND BACK OF HIS HEAD AGAINST THE DECK, CUTTING THE BACK OF HIS HEAD BELOW HIS HARD HAT.
