While sitting in a vehicle, it was struck by another vehicle causing the ee to fall to the floor. The incident was caused by a careless operator of the 2nd vehicle who did not observe the 1st. The injured ee was wearing the appropriate PPE but has a poor attitude. The operator was trained but received retraining in the safe operation.

Employee was injured on 9/20/2015 and worked until 10/20/2015 when doctor recommended change in treatment and pulled him off work. While riding in BT24, ride picked the switch and derailed causing employee to strike his left elbow and shoulder against the side of BT24. It did not involve compliance issues, job proficiency or attitude. All personal safety items were being used.

During a Part 50 audit and citation 9123775 being issued, this injury report is being filed. Employee stepped on miner to see over it when shuttle car came up and struck left foot resulting in a fracture and sutures. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

When the diesel motor the EE was operating derailed, the EE struck his left knee against a SCSR box. The incident was caused by the EE's failure to react to the rail conditions and maintain operating control. The EE had on the proper protective items and was compliant with rules and regulations. He was properly trained and has a positive attitude towards safety.

Employee was operating 55 jeep and struck 43 locomotive outside on the old jump track. The accident occurred because the employee wasn't paying attention. A state inspector checked the jeep after and found no compliance issues that would of resulted in accident It didn't involve job skills proficiency training or attitude. No PPE or protective devices on equipment involved.

The ee was discovered laying along the right rib when a scoop passed by him. No one saw him in the area prior to the accident. The ee claims that he does not know what happened. This occurred because the ee placed himself along a moving piece of mobile equip. The ee failed to follow a basic mining rule of not passing by a moving piece of equip. or getting the oprs. attention.

Individual was standing beside scoop bucket at the supply car location. Due to lack of concentration his foot slipped and at the same time the scoop drifted forward pinning the right leg between the scoop bucket and supply car.(Deep bruise to lower leg.) This did involve equipment and personnel safety of the individual in a Red Zone Area. Personal safety items were being utilized.

Individual was riding(back to direction of travel) in BT-55 Track Jeep when striking his head on the mine roof causing pain to the right neck area. This did not involve compliance issues, no equipment involved. This did not involve job proficiency, or attitude. All personnel safety items were being utilized.

The ee claims to have felt pain in her neck and back when the mantrip she was riding in d-railed. The accident was caused by the ee's failure to recognize the rails were slick and did not properly use the sanding devices to slow the mantrip. The ee had on the appropriate PPE.

Shuttle car canopy caught roof strap and bending it causing it to strike the left hand of the employee. Diagnosed as light contusions and abrasions. No problem with training, PPE, mining machine or compliance issues.

Driving 37 jeep out of the 60# at Sugar Run, being followed by 61 Jeep. 37 jeep stopped and 61 jeep slid into 37, knocking the injured off his seat and striking his right shoulder on the handle of the crank brake, receiving a contusion. No problems with training, mining machines, methods, PPE or compliance issues.

Riding in passenger compartment of D42, switching out on backside of East Mains switch when D42 slid into parked mantrip. Injured struck head on safety hand hold resulting in a possible lower cervical sprain. Referred to physical therapy on 09/22/2014. No problem with training, PPE, mining machines or compliance issues.

While operating a shuttle car the operator struck the rib which resulted in the operator striking his head against the shuttle car canopy. The employee thought that he was suffering from an off site injury he had experienced previously. It was determined later that it was very likely that the stated incident was the cause of his problem.

While walking onto longwall face an object(rock/coal) came out of panline and struck him on the left side of the face cheek area causing a laceration. He received six sutures.

Scooping haul road in front of feeder in 23D, with 620-2001 scoop, to level roadway. Injured was positioned behind feeder wing directing scoop onto feeder. The edge of the scoop bucket hit the feeder wing and flexed it out, striking the injured in the left hip and upper thigh resulting in a sprain and contusion. No issues with training, equipment malfunctions or mining methods.

Employee was riding in a mantrip when it derailed and came off track. The employee jarred and strained his lower back. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.

While operating a locomotive, the ee struck his head against a section of hung cable resulting in a strain - sprain of the neck. The event became reportable when the ee began missing work on 3-17,2014. The injury occurred because the ee was not paying attention to his surroundings. His history of neck problems compounded the potential for this ee to have a repeat injury.

Employee was tramming a scoop with his hand on the canopy guarding. When the scoop turned he pinched his left ring finger between the scoop and the rib. His hand was positioned in the wrong location. He was operating a 601 S&S scoop. It didn't involve job skills, proficiency, training, or attitude. No ppe was a factor. The protective guarding was in place.

Foreman Trainee following 636 Scoop traveling inby, 59-60 block 21D, with 650 batteries on it's duckbill. Foreman put hands on top of 650 batteries (unknown reason, may have tripped and tried to catch himself). Battery end of scoop went down in rut, 650 batteries raised and roofed. Pinched both hands between battery and roof.

Individual was operating # 3 Jeep towards Kuhntown Bottom when 2 motors with a trip of 4 cars came around the turn striking the # 3 Jeep. Individual jumped and struck the left shoulder ,hip, and left leg against the mine floor.

Individual was operating a 20 ton motor pulling a supply train when the train came uncoupled from the motor. The safety cable did not break and the supply train struck the motor causing the individual to be jarred by the trains impact. Individual has pain in lower back and down to the right ankle.

2 motors with rock duster & 2 cars started up 19 rt. section when loads became difficult to pull. 1 of the motors being operated by the injured & the other one by another ee. Motors were in a turn when they got off the motors to see if they had wrecked. The injured was getting back on tram & set 1 foot in the tram when he fell on his foot. His foot was pinned between rail n& motor.

Was struck by a roof bolt used to hang a run-thru check while operating a shuttle car resulting in 5 sutures to the right side of the forehead.

Truck backed into man picking up debris, twisted knee, and knocked him down. Injured right shoulder, left knee and lower back

The injured was operating No.25 man bus coming off of 5 North Mains sections when his bus collided with bus No.45. The accident occurred between brk 2-3 just as you come thru the S turn near the mouth of the section. The injured's left leg hit against the control box on the man bus causing a fracture to his left leg (femur).

Driving D41 mantrip at 30 block East Mains when a small piece of rock dust entered right eye causing irritation. Treated with antibiotic/steriod drops to eye. No issue with PPE, training, mining methods or compliance.

Operating mantrip and ran into a parked flat car along 9 South haulage resulting to an injury above the left eye needing 20 sutures.

The employee right leg got caught between the coal rib and a section of ventilation tubing when the end of the tube was struck by an on-coming shuttle car. The resultant injury was a laceration of the right leg requiring 10 sutures.

Employee was operating a battery motor when it derailed and caused him to be jarred in the operator's compartment. Employee complained of back pain and has since been diagnosed with multi level degenerative disk disease and spinal stenosis. Employee had continued to work until he was no longer able to perform his duties as of 8-13-12.

He was sitting on a jeep that was bumped by a motor and duster that had slid out of a switch with a grade. Started missing work on 5/17/12.

At 42 block Main West a 604 scoop needed reset on a move dolley. The injured was operating the scoop when the scoop fell off of cribs and slammed down on the dolley. The injured felt pain in his neck and was treated with physical therapy, no lost workdays.

Injured unhooked the chains that secured a car of gravel. He had his back to the car & the motorman & was attempting to open the page gate so the motorman could pull a car of gravel off the cage. The motorman thought the injured was signaling for him to pull forward, but the injured wasn't. The injured was pinned between the car & gate, causing a fracture to his back.

EE stated he was operating manbus & bus was struck by a tram causing the right side of his body to strike the engine compartment. The EE stated he felt pain in his neck & middle of his back. The alleged injury was reported on 10/23/11 but he did not miss any work until 10/31/11. The alleged injury is being challenged by the Co. & Co reserves the right to remove from record

A scoop traveling inby in the 3 Left heading of 17D that went into a water hole and got tangled up in ropes and cables underneath the water. This caused the loader cable to be pulled tight and come off the hangers and strike the injured in the right arm and the head. Employee received contusions and was put on restricted duty. No training, machine, mining or compliance issues.

He was driving #86 jeep past #22 jeep that was switched out. When driving by a scraper pipe on #86 jeep caught on #22 and struck him in the face causing lacerations.

Employee was viewing the #5 locomotives wheel location when the motor unexpectedly dropped off the rail striking the employee's right leg, resulting in compound fractures.

Traveling 9 South Haulage from 11D to 12D, operating the tail motor of a rock dust motor crew, the injured employee struck her hard hat on a low hanging roof strap suffering a sprain/strain to her neck. The miner missed no work, but was treated by a physical therapist. No issues with training, machinery, mining methods or compliance.

EE was operating #4 Jeep on main north haulage road at 152 block. The Jeep struck a broken rail causing the Jeep to halt and come off the track. EE struck the left side of the face and nose against the frame of the jeep. EE suffered a nasal fracture to the cartilage of the nose.

In the longwall of 16D, a duckbill scoop operator was backing up and turning. The injured was standing too close and the duckbill attachment caught his right foot between the duckbill and the floor causing a contusion. No issues with training, mining methods, equipment or compliance.

Felt neck pain while operating shuttle car. Did not hit head. Was ruled compensable and mine notified on 10/6/11.

Employee was flagging for the scoop that was pulling belt, the chain that secured the sheave wheel broke and struck him above the left eye and on the chin. Required 5 stitches.

Sitting on #55 Jeep when it was struck by a motor and he experienced a laceration to his face and a fractured knee cap.

Mantrip ran into a spur and struck a parked mantrip causing low back pain to passenger.

Operating 20 ton motor when cap light caught on roof strap, neck pain. Operator disputes the allegations set forth in Citation No.8050922 and submits this report solely to terminate the Citation. Injured was sitting in mantrip on way to Metz bottom. Driving down two track, Metz bottom, and ran into 5 spur striking another parked jeep. The switch was left thrown for the turn. Employee felt pain in back and was treated with physical therapy.

EE was standing behind BT.22 mobile which was off track in a track spurr. The mobile was struck with a 20 ton motor which caused the mobile to kick sideways pinning the ee feet beneath the mobile and the rib line.

2x4 slid down a conveyor belt and struck employee on forehead

EE was riding in an open mantrip when the loop wire that was hanging down came in contact with his lower lip causing a cut , requiring 5 sutures.

Loader was being pulled forward to prepare to rock dust. As the loader was pulled forward, the boom of the loader struck the employee.

A twenty ton motor collided with a mantrip causing an employee to receive a cut on his right bicep.
