EMP WAS USING THE CASE SKIDSTEER (BOBCAT) TO PULL AN OIL TANK TO THE PLANT. AS HE WAS BACKING TOTHE PLANT HE HIT A ROUGH PLACE CAUSING THE SKIDSTEER TO BOUNCE. HE APPARENTLY TURNED THE CONTROL & DROPPED (LOWERED) THE FORKS OF THE SKIDSTEER CATCHING HIS FOOT BETWEEN THE FORK MECHANISM AND THE OPERATOR CAB.
EMPLOYEE WAS ONA GOLF CART TRAVELING IN THE TRACK ENTRY. WENT THROUGH A CHECK SURTAIN AND RAN INTO A PARKED SUPPLY TRAILER THAT HAD BEEN BROUGHTTO THE TRACK TO BE PUT ON THE RAIL.
MAN WAS OPERATING THE REMOTE CONTINUOUS MINER. A SHUTTLE CAR CAME IN FOR A LOAD. THE CAR RAN OVER A PIECE OF FIRE CLAY. IT SHOT OUT HITTING HIM ON THE LEFT LEG BELOW THE KNEE.
WHILE DRIVING A SHUTTLE CAR MON WENT THRU A ROUGH PLACE IN THE HAUL ROAD CAUSING HIM TO BUMP HIS ELBOW ON THE SHUTTLE CAR. HE DID NOT GO TO A DR UNTIL 12-13-95 AFTER THE ELBOW CONTINUES TO PAI N.
A PIECE OF CURTAIN HAD BECOME ENTANGLED NEAR A BELT ROLLER AS THE EMPLOYEE ATTEMPTED TO REMOE THE SMALL PIECE OF CURTAIN IN THE ROLLER CAUGHT A LOOSE FINGER IN HIS GLOVE & CAUSED HIS ARM TO CO ME IN CONTACT WITH THE CONVEYOR BELT THE FINGER WAS LOOSE IN HIS GLOVE DUE TO A FINGER BEING SURGICALLY REMOVED PREVIOUSLY
THE BELT CREW HAD PUT ON (EXTENDED) THE 6T H 54IN BELT LINE THEY RAN THE BELT & THEN STOPPED IT TO SET THE FEEDER AFTER SETTING THE FEEDER THEY WERE GOING TO RUN & ALIGN T HE BELT WHEN EE STAR TED THE BELT THE BELT ANCHOR (JACK PIPE) BENT & THE TAILPIECE PULLED LOOSE HITTING HIM
MAN WAS ADJUSTING THE FUEL ON DIESEL TRACTOR WHEN HIS FOREARM CAME IN CONTACT WITH THE FAN CAUSING A LACERATION.
MAN WAS WALKING IN THE LAST OPEN CROSS CUT OF A SECTION WHEN A SHUTTLE CAR TURNED A CORNER CAUSING THE CAR CABLE TO CATCH HIS LEG BETWEEN THE CABLE AND THE COAL RIB.
THE BATTERIES WERE DISCHARGED ON HIS SCOOP, REQUIRING IT TO BE PULLED IN. AS A DIESEL SCOOP WAS PULLING HIM THROUGH AN UNDERPASS HIS SCOOP CAUGHT ON THE REAR OF THE SCOOP THAT WAS PULLING HIM, LIFTING HIS SCOOP SLIGHTLY OFF THE MINE FLOOR. WHEN IT CAME LOOSE HIS SCOOP BOUNCED CAUSING HIM TO HIT HIS HEAD ON THE CANOPY.
MAN WAS CHECKING WATCHING THE BELT FOR BAD SPLICES SOME BELT RAVELINGS STRINGS HAD WRAPPED AROUND A ROLLER AND WAS HANGING DOWN. HE PLACE HIS HAND NEAR THE ROLLER AND WHEN THE ROLLER TURNED ON E OF THE RAVELING CAUGHT HIS FINGER AND PULLED IT INTO THE ROLLER CAUSING A LACERATION.
EE STATED THAT AFTER HE GOT A LOAD FO ROCK, HE TURNE DAROUND ON CAR AND TWISTED LEFT KNEE.
MAN WAS TRAMMING A SUPPLY JEEP THROUGH A CURTAIN WHEN HE CAUGHT HIS ARM BETWEEN THE JEEP AND A SUPPLY TRAILER.
INJURED STATED HE WAS SITTING ON BACK OF A 3 WHEELER AND WHHEN IT CAME OUT OF CROSSCT HIS HEAD HIT THE ROOF CAUSING HIM TO JAM HIS NECK.
INJURED STATED THAT HIS LEFT FOOT WAS HANGING OUT OF MANTRIP AND WAS CAUGHT BETWEEN TIMBER AND SIDE OF MANTRIP WHEN MANTRIP MADE TURN.
INJURED STATED THAT HE WAS OPERATING A SCOOP WHEN HE RAN OVER A LUMP OF COAL CAUSING HIM TO JAM HIS HEAD INTO ROOF.
MAN WAS RIDING IN A DIESEL MANTRIP THAT WAS BEING BROUGHT TO BOTTOM A SCOOP OPERATOR HAD PARKED HIS SCOOP (NEAR A CURTAIN) TO HELP A SUPPLY MAN WITH HIS EMPTIES THE TRIP CAME THROUGH THE CURTA IN & HIT THE SCOOP A SCSR BOX HIT EE IN THE BACK OF THE HEAD
HE WAS BRINGING A DIESEL MANTRIP TO BOTTOM FOR DAY SHIFT. A SCOOP OPERATOR HAD PARKED NEAR A CURTAIN TO HELP A SUPPLY MAN WITH THE EMPTIES. THE INJURED AND A PASSENGER CAME THROUGH THE CUR TAIN HITTING THE SCOOP. THIS THREW INJURED INTO THE STEERING WHEEL.
BILL STATED HE WAS WORKING ON THE CHUTE ON THE BELT HEADER. HE WAS STANDING ON THE BELT ON THE BACK SIDE OF THE CHUTE WHEN HIS WORKING BUDDY STARTED THE BELT. HE FELL TO THE FELT AND WENT UN DER THE CHUTE BEFORE GETTING OFF. SAW CHIROPRACTOR FOR FIRST TIME ON 10-27-94.
EE WAS TURNING AROUND ON A SHUTTLE CAR AFTER GETTING A LOAD FROM LOADER IN #6 ENTRY AND TWISTED HIS RIGHT KNEE/ HIS FOOT CAUGHT ON THE CAR SEAT. HE DIDN'Y MISS ANY DAYS UNTIL 10-24-94 WHNE HE HAD HIS KNEE OPERATED ON.
INJURED HAD SPOTTED AN EMPTY TRAILER. HE WAS BACKING #54 TRACTOR UP TO HOOK IT TO HIS DUSTER WHEN HE HIT HIS HEAD ON A PIN BOARD. TOP WAS LOW. HE STRAINED HIS NECK.
INJURED WAS STAGGERING OUT A SHUTTLE CAR. WHILE GETTING A LOAD OF COAL A LUMP POF COAL BOUNCED OFF THE CAR FRAME AND STRUCK HIS KNEE. HE WAS HAVING TROUBLE WITH HIS KNEE DUE TO LOCKING US AN D SWELLING BEFORE THIS INCIDENT. HE WORKED UNTIL JUNE THE 7TH WHICH WAS THE FIRST DAY HE MISSED AND WENT TO THE DR WHICH SAID HE NEEDE DSURGERY.
INJURED STATED TO THE BEST OF HIS KNOWLEDGE HE WAS CLEANING ON THE E BELT . HE DECIDED TO CROSS THE BELT. AFTER THROWING HIS SHOVEL TO THE OTHER SIDE, HE STARTED TO CROSS THE BELT WHEN THE BEL T STARTED, DRAGGING HIM UNDER THE HEAD FRAME OF THE FEEDER. HE ROLLED OFF THE BELT AND WAS CAUGHT BETWEEN THE TOP AND BOTTOM BELT FOR A PERIOD OF TIME BEFORE BEING ABLE TO GET OUT.
MANW AS HELPING REPLACE A PIN (THAT HAD SHEARED) IN THE PICKBREAKER OF A RATIO FEEDER. THE BREAKER TURNED AND A PIECE OF FOREIGN MATERIAL CONCRETION THAT SHEARED THE PIN). HIT HIM ON THE RIG HT SIDE OF THE FACE.
SLACK EXCESS CABLE WAS LAYING IN THE ENTRY AFTER THE CABLE HAD BEEN REPAIRED WHEN THE CAR PICKED UP CABLE THE CABLE CAUGHT MEN BEHIND HIS LEFT KNEE. THIS CAUSED HIM TO FALL TO THE GROUND AND I NJURED HIS KNEE HE WAS NOT AWARE OF THE EXCESS CABLE.
INJURED STATED HE WAS GOING UNDER A CABLE IN THE BELT ENTRY WHEN HE RAISED UP AFTER GOING UNDER THE CABLE HE HIT HIS HEAD ON A PIN BOARD
INJURED HAD JUST LOADED HIS SHUTTLE CAR ANDWAS HEADER BACK TO FEEDER TO DUP WHEN A CABLE CAME OFF OF KNOCK OFF OF A CROSSOVER AND CUAGHT HIM ACROSS THE HEAD AND KNOCKIGN HIM OFF HIS CAR. HE R ECEIVED A CONTUSION TO THE HEAD. WAS RELEASED TO WORK NEXT DAY.
MAN'S ROOF BOLTER WAS HUNG UP. WHILE PULLING IT FREE, USING A CHAIN HOOKED TO A SCOOP JEEP, THE CHAIN BROKE AND A PIECE OF THE CHAIN HIT THE MAN ON THE RIGHT CHEEK.
INJURED STATED HE WAS TURNIGN AROUND ON A SHUTTLE CAR AT THE FEEDER WHEN HIS KNEE COLLADSES AND STARTED SWELLING. PULLED LIGAMENT A STRANED KNEE
WHILE RECLAIMING BELT, MAN WAS ATTEMPTING TO UNHOOK A GRIP LOCK THAT WAS ATTACHED FROM A CHAIN (ON THE BUCKET OF A SCOOP) TO A PIECE OF BELT. THE CHAIN WAS TIGHT AND THE GRIP LOCK COULD NOT BE UNHOOKED. THE OPERATOR, THINKING THE MAN WAS CLEAR, LET THE BUCKET OF THE SCOOP DOWN, SO THE GRIP LOCK COULD BE UNHOOKED, CATCHING THE MANS HAND.
THE MOTORMAN WAS PUTTING HIS MOTOR BACK ONTO THE TRACK AFTER IT HAD DERAILED AT A SWITCH. HE JACKED THEMOTOR UP ADN WAS TRYING TO SHIFT THROW IT BACK ONTO THE TRACK WHEN HE SLIPPED ADN THE MT OTOR CUAGHT HIS LEFT FOOT.
INJURED WAS SITTING ON A THREE WHEELER WHEN A SHUTTLE CAR DROVE BY. AT THIS TIME HE FELT HIS LEFT EYE START IRRITATING. SOMEWAY A SMALL PIECE OF METAL GOT INTO HIS EYE.
MAN WAS WORKING ON A PILLAR BLOCK BEARING ON THE STATIONARY ROLLER. THERE WAS PRESSURE ON THE BOARING REQUIRING THE TAKE UP TO BE RELEASED. HE THOUGHT HE WAS IN THE CLEAR BUT WHEN THE CARRIAGE CAME BACK IT CAUGHT HIS FOOT.
MAN WAS PUTTING A STEERING JACK ON A SHUTTLE CAR. HE HAD PLACED THE PIN IN THE BARREL END & WAS PREPARING TO PUT THE PIN IN THE STINGER END. HE WAS HOLDING THE STINGER TO ALIGN THE JACK WHEN T HE STINGER WITHDREW (DLOSED, APPARENTLY FROM AIR) CATCHING HIS HAND.
INJURED WAS TRAMMING A SCOOP TO DUMP ROCK IN THE FEEDER WHEN HE RAB OVER A PIECE OF ROCK CAUSING HIM TO JAM HIS NECK INTO THE TOP.
INJURED MINOR OPERATOR WAS ALIGNING THE CONTINUOUS MINER TO MAKE THE 2ND CUT IN A FACE. THE SHUTTLE CAR HAD STOPPED, BUT AFTER A MOMENT HE DID NOT SEE THE MINER OPERATOR AND SAW A LIGHT BESID E THE MINER. HE THOUGHT THE OPERATOR WAS READY SO HE TRAMMED FORWARD CATCHING THE OPERATOR BETWEEN THE SHUTTLE CAR AND MINER.
MAN WAS DRIVING A DAPCO PERSONNEL CARRIER WHEN THE FRONT WHEEL HIT A LUMP OF COAL. THIS CAUSED THE STEERING WHEEL TO SPIN AND THE STEERING KNOB HIT HIS HAND.

MAN STEPPED BETWEEN A JEEP AND ROCKDUSTER. THE JEEP STARTED UP MOVING AND CAUGHT THE MANS FOOT. THE JEEP OEPRATOR DID NOT SEE THE MAN.
WHILE MAN WAS GETTING A LOAD A PIECE OF MATERIAL WAS THROWN FROMTHE LOADER BOOM STRIKING HIM OVER THE RIGHT EYE.
INJ WAS CHECKING ALIGNMENT OF #3 UNIT BELT AFTER NEW BELT SET UP WAS ADDED. HE HAD HAND ON TOP OF TAILPIECE WATCHING BELT RUN WHEN LARGE LUMP OF COAL CAME DOWN BELT & STRUCK INJ ON FINGER.
MAN WAS OPERATING A SCOOP JEEP THAT HINGED IN THE MIDDLE.WHEN HE TURNED A CORNER THE SCOOP HINGED AND CAUGHT HIS FINGER.
INJ DRIVING DOWN SUPPLY ROAD,RAN OVER SOME ROCK IN ROADWAY CAUSING RIGHT KNEE TO HIT BUMPER OF JEEP.
MAN WAS DRIVING A PERSONNEL CARRIER (RUBBER TIRED) FROM THE UNIT TO BOTTOM AT THE END OF SHIFT. HE DID NOT SEE THE HOLES IN THE SUPPLY ROAD USED TO LET THE SUPPLY TRAILER WHEELS DOWN. HE HIT T HE HOLES WITH THE MANTRIP, CAUSING HIM TO BOUNCE & JAR HIS BACK.
MAN WAS DRIVING A PERSONNELL CARRIER. AS HE CAME TO A SET OF AIRDOORS, HE WAS HOLDING A CBLE HOOK THAT HAD BEEN STRAIGHTENED. A CABLE HANGER HOOK WITH A HOOK ON EACH END. AND HE ATTEMPTED T O ACTIVATE THE SWITCH WITHT HE STRAIGHTED HOOK. ONE END OF THE HOOK HUNG ON THE SWITCH AND THE OTHER HIS FINGER.
MAN WAS TRAMMING THE LOADER & PULLING THE CABLE WITH A ROPE THE ROPE BROKE & AS IT RE COILED IT HIT HIM IN THE LEFT SIDE
EMPLOYEE WAS TURNING LEFT IN THE LAST OPEN X-CUT GOING TO THE LOADER AND HIT HIS HEAD ON A CROSS-OVER CAUSING HIS SAFETY CLASSES TO CUT ABOUT ON 1 1/2 INCH ABOVE HIS RIGHT EYE BROW.
WHILE GOING THRU A PULL CURTAIN ON GOLF CART MAN WAS THROWN INTO THE STEERING WHEEL WHEN HE HIT ANOTHER GOLF CART.
INJ WAS CLEANING NEW SET UP KNOCKING TIMBERS WITH SCOOP. HE RAN OVER ONE OF TIMBERS WITH SCOOP. HE RAN OVER ONE OF TIMBERS WITH SCOOP JAMMING NECK. INJ WENT TO DR ON 7TH.
