A FALL OF ROOF WAS DISCOVERD IN THE 1ST S.E. SUBPANEL #5 ENTRY ONE CROSSCUT INBY SPAD #2211. APPROX 600 FT OUTBY THE FACE. THE DIMENSIONS ARE90 FT X 6.5 FT. NO EQUIPMENT, VENTIALTION OR PEOPLE WERE AFFECTED.
EE KNOCKED THE POWER TO THE CONTINOUS MINER CONTROL PANEL. HE REMOVED THE PANEL COVER TO HAVE ACCESS TO THE P.CT. RELAY. HE WAS LOOKING AT THE RELAY TO DETERMINE THE TOOLS NEEDED TO CHANGE THE RELAY. AS HE EXAMINED THE RELAY A FLASH OCCURRED CAUSING A BURN TO HIS LEFT HAND.
HOIST WAS DOWN FROM 7:40 AM UNTIL 10:05 AM. PROBLEM WAS BRAKE WAS GETTING HOT BECAUSE IT WAS NOTRELEASING GOOD. PROGRAM ON PLC CONTROLLER WAS CHANGED TO ALLOW FOR CHANGES IN BRAKE ADJUSTMENT F ROM DRUM HEATING UNDER NORMAL USE.
FALL OF ROOF DISCOVERED IN #8 ENTRY OF THE NE MAINS IMMEDIATELY INBY SPAD #17661. DIMENSIONS 25'L, 18'W, 8'THICK. NO EQUIPMENT, VENTILATION OR PEOPLE WERE AFFECTED.
A FALL OF ROOF OCCURRED IN THE 3RD S.W. PANEL, 4 UNIT 2 ENTRY 220 FT INBY SPAD 22745. THE FALL WAS APPROX. 50 FT LONG 20 FT WIDE AND 5 FT THICK. NO EQUIPMENT, VENTILATION OR PEOPLE WERE AFFEC TED.
THE EMPLOYEE WAS HELPING LOAD ROOF BOLTS ONTO A ROOF BOLTER. AS HE LIFTED SOME BOLTS AND TURNED TO PLACE THEM ON THE BOLTER HIS BACK STARTED HURTING. HE INFORMED HIS SUPERVISOR BUT DID NOT S EEK MEDICAL TREATMENT UNTIL 8-19-96.
EMPLOYEE WAS DRIVING A SHUTTLE CAR, WHEN HE HIT A ROUGH PLACE IN THE HAUL ROAD. THIS CAUSED HIM TO BOUNCE IN HIS SEAT AND JAR HIS BACK.
EE WAS HITTING ON WEAR RUNNER ON MINER AND A SMALL PIECE OF METAL HIT IN CENTER OF FOREHEAD CAUSING A SMALL PUNCTURE WOUND.
A FALL OF ROOF OCCURRED ON THE #1 UNIT 2ND SW PANEL#4 ROOM 120 FT FROM FACE IN LINE WITH RM#32OF THE PANEL VENTILATION, EQUIPMENT, NOR PEOPLE WERE AFFECTED.
THE WATER HOSE WAS BEING PULLED (MOVED UP) WITH A THREE WHEEL VEHICLE, THE HOSE HUNG AND WHEN IT BROKE LOOSE IT HIT THE EMPLOYEE ON THE LEFT LEG CAUSING A SMALL LACERATON. THIS ACCIDENT WAS N OT REPORTED UNTIL 8-15-96.
EMP WAS SHOVELING COAL AT A BELT DRIVE. WAS TWISTING AS HE TURNED TO THROW THE COAL ON THE BELT AND HIS BACK STARTED HURTING. WENT FOR MEDICAL TREATMENT BUT DID NOT MISS DAYS FROM WORK UNTIL 7-23-96.
WHILE TIGHTENING BOLT UNDERNEATH METAL BENCH,HAND SLIPPED OFF OF WRENCH AND HIT THE SHARP EDGE OF METAL. CUT THE LEFT INDEX FINGER ON JOINT AREANEEDING STITCHES. DR STITCHED THE CUT UP WITH 8 STITCHES.
EMPLOYEE WAS HELPING MOVE SUPPLY BINS FROM ONE SUPPLY HOUSE TO ANOTHER. WHILE HE WAS LIFTING AND CARRYING THE BINS HE STRAINED HIS GROIN.
EE WAS CHANGING A TIRE ON A GOLF CART. WHEN EE LIFTED THE TIRE & TURNED HIS BODY, HIS LEFT HIP STARTED HURTING.
FALL OF ROOF OCCURRED ON THE #5 UNIT, 1ST SW SUBPANEL, #3 ENTRY, 160' FROM THE FACE. THE FALL WAS 70'L, 19'W, 6'H. VENTILATION, EQUIPMENT NOR PEOPLE WERE AFFECTED.
EMP WAS ROOFBOLTING AN AREA OF HIGH TOP WHERE THE ROOF HAD BEEN TAKEN DOWN FOR AN OVERCAST. AFTER DRILLING A HOLE HE REACHED OUT TO GET THE AUGER AND A SMALL ROCK FELL HITTING THE CANOPY. THE ROCK BROKE INTO SMALLER PIECES HITTING HIS ARM CAUSING TWO LACERATIONS.
EMP WAS REPOSITIONING THE ROOF BOLTER FOR THE NEXT BOLT WHEN A SMALL PIECE OF DRAW FELL FROM BETWEEN THE LAST 2 ROWS OF BOLTS, HITTING HIM ON THE HEAD CAUSING HIM TO HIT HIS HEAD ON THE MACHIN E, BRUISING HIS MOUTH AND CHIPPING A TOOTH.
AFTER FILLING THE ROCKDUSTER "BULK" PODS, THE EMPLOYEE WAS TAKING THE HOSE OFF. AS HE STEPPED OFF THE SIDE OF THE PAD, HE TRIPPED ON A SAFETY ROPE CAUSING HIM TO FALL.
WHILE GETTING OFF A PERSONNEL CARRIER,EMP HUNG HIS FOOT CAUSING HIM TO FALL AND TWIST HIS KNEE.
EMPLOYEE WAS DRIVING A DIESEL VEHICLE UP AN APPROXIMATE 10% GRADE WHEN HE ENCOUNTERED A BUMP (ROUGH PLACE) APPARENTLY CAUSING HIM TO HIT HIS HEAD ON A HAND RAIL OF THE VEHICLE. THIS INCIDENT W AS INVESTIGATED BY MSHA AND KDMM.
EMP WAS USING THE CASE SKIDSTEER (BOBCAT) TO PULL AN OIL TANK TO THE PLANT. AS HE WAS BACKING TOTHE PLANT HE HIT A ROUGH PLACE CAUSING THE SKIDSTEER TO BOUNCE. HE APPARENTLY TURNED THE CONTROL & DROPPED (LOWERED) THE FORKS OF THE SKIDSTEER CATCHING HIS FOOT BETWEEN THE FORK MECHANISM AND THE OPERATOR CAB.
EMP WAS SETTING (REPOSITIONING) THE MINER FOR THE NEXT CUT. REACHED DOWN AND PULLED THE CABLE OVER AND HIS BACK STARTED HURTING. INFORMED HIS SUPERVISOR BUT DID NOT GO TO DR UNTIL 6-3-96. C-RA YS DID NOT SHOW THE PROBLEM. RETURNED TO ANOTHERDR 6-7-96 FOR MRI, WHICH IDENTIFIED THE PROBLEM.STARTED MISSING DAYS FROM WORK 6-12-96.
WHILE CHANGING A LIGHT BNULB ON A SHUTTLE CAR, THE OLD BROKEN BULB WAS PLACED ON THE MINE FLOOR WHILE THE NEW BULB WAS BEING INSTALLED. AS HE WEAS INSTALLING THE NEW BULB, THE MECHANIC KNELT D OWN ON THE OLD BULB CUTTING HIS LEG.
EMP WAS STRAIGHTENING OUT A 5' ROOF BOLT HE HAD BENT AND PLACED IN HOLE, WHEN HIS LEFT SHOULDER STARTED HURTING. WENT TO DR. BUT DID NOT RECEIVETREATMENT. CONTINUED TO HAVE PAIN AND RETURNED T O DR ON 5-30-96. STARTED MISSING WORK 5-31-96.
MAN WAS REMOVING A BOTTOM ROLLER FROM THE SLOPE BELT, AS HE LIFTED ON THE ROLLER TO BRING IT FROM UNDER THE BELT, HIS BACK STARTED HURTING.
EMP HAD MOVED THE BELT FEEDER AND WAS IN THEPROCESSOF MOVING THE CABLE. ASHE WAS HANDLING (LIFTING) SEVERAL LOOPS OF CABLE, HIS BACK STARTED HURTING.
A FALL OF ROOF OCCURRED IN THE S.E. PANEL AT SPAD #20925 IN THE #7 ENTR THE DEMINSIONS WERE 35'X19.5'X5.5'. VENTIALTION, EQUIPMENT OR PEOPLE WAS NOT AFFECTED.
THE HOIST IN THE DOTIK. IT SLOPE HAD PROBLEM WITH THE ELECTRICAL CONTROL BOX. AND THE CONTROL BOX WAS REPLACED.
WHILE TRING TO REMOVE THE BOLTS FROM A SHUTTLE CAR WHEEL UNIT, THE WRENCH SLIPPED AND HIT EMPLOYEE ON THE CHEEK RIGHT SIDE OF FACE, CAUSING A SMALL LACERATION.
EE WAS PULLING A PIECE OF DRAW SLATE WHEN HIS HAND SLIPPED CAUSING HIS ARM TO HIT THE SLATE & CAUSE A LACERATION TO HIS LEFT ARM.
A FALL OF ROOF OCCURRED ON THE 5 UNIT , 2ND S.W. SUBMAINS THE 3 ENTRY AT FACE THE FALL WAS 45' LONG 19' WIDE AND 8' THICK PEOPLE, EQUIPMENT NOR VENTILATION WAS AFFECTED.
