While traveling on a surface haul road, the injured's truck impacted the berm. He continued powering into the berm for 150' to try to get back on the road. Instead, he topped it, traveling down the rain satureated slope, sinking right front tire in the mud. Turned truck over to the right. Suffered strains from the jarring. Due to schedule w/dr, rtw delayed.

While operating his loader, the injured turned to look behind him and strained his neck.

No medical until 2/19/08, when doctor prescribed an anti-inflamitory to reduce inflamation. While pinning coal, the injured jarred his back, causing a strain. No medical until 2/19/08 and still no lost time.

While turning a corner to dump a load, one set of the injured's rear tires went over the berm and pulled the rest of the truck off. The truck and cab slid backward down the face of both sides of the highwall corner about 50'. The injured was wearing his seat belt and suffered a cut on his head requiring 3 staples to close it. ER room doctor had injured take several days off work.

While letting the bed of his end dump truck down, the bed free fell, jarring him and threw the injured into the steering wheel causing a laceration to his check.

While pinning coal with his loader in the pit, the loader movement strained his neck.

While operating the water wagon in the pit, the injured lost control and ran into a pile of shot material, straining his body and hitting his head. No injuries other than strain and headache.

REPORTED TO COMPANY AND RECEIVED FIRST TREATMENT BEYOND FIRST AID ON 11-5-03. WHILE RUNNING A WATER WAGON, THE INJURED WAS TURNING IT AROUND, HIT A HOLE AND THE BERM JARRING HIS HIP AND CAUSIN G A STRAIN. RECEIVED A PRESCRIPTION FOR MUSCLE RELAXERS.

WHILE OPERATING THE WATER WAGON ON THE COAL PAD, THE INJURED RAN OVER A LARGE CHUNK OF COAL CAUSING THE WATER WAGON TO JERK AND REACT UNEXPECTEDLY, RESULTING IN THE INJURED STRAINING HIS BACK.

WHILE OPERATING THE ELEVATING CONVEYOR, THE INJURED REACHED TO PICK UP A SMALL ROCK TO INSPECT JUST AS A LARGE PIECE OF COAL ROLLED BACK DOWN THE CONVEYOR AND SMASHED HIS HAND AGAINST ONE OF T HE CONVEYOR PADDLES RESULTING IN A TORN LIGAMENT, HAIRLINE FRACTURE AND STITCHES.

WHILE CLEANING COAL WITH HIS LOADER BUCKET, THE INJURED MADE ONE MANEUVER THAT ALLOWED HIM TO BACK INTO THE HIGHWALL STRIKING IT HARD WITH THE REAR BUMPER CAUSING A WHIPLASH (SPRAIN) INJURY TO HIS NECK.
