Positioning pump hose into sediment pond and lost footing on embankment and landed on right shoulder.

Contractor was cleaning crushing unit and lost footing resulting in a sprain to the right ankle.

Employee was assisting in the removal of access panel on bottom frame of 330C Trackhoe. The panel shifted pinning the employee's finger against the frame.

EE was climbing onto a 775 End dump truck and set lunch pail on the front bumper of truck. When EE went to reach for the lunch pail and reposition it, lost footing and fell to the ground.

Injured was running D9L dozer, when he tracked over large rock causing him to be jarred around. No doctor visit or lost time until March 18th 2015

While repositioning the dragline cable out of the way to walk, the injured caught his thumb in a strap severing the top of the thumb.

Walking on un-level ground and twisted left ankle and hip

Employee had been servicing a D9L Cat dozer. In the process he mishandled a filter and hurt his leg.

While exiting the cab, reached up to release door, lost balance falling and injured right knee.

Stepping off the front ladder of a mining truck and twisted ankle as his foot hit uneven ground

Replacing fuel filter on D10T and the access door closed with enough force to crush his fingers on his left hand.

While bending over to pick up dragline power cable, the injured strained his back.

Had been breaking down a D10 final drive with hammer and chisel. Later as he stepped to get a tool his back and leg gave way and he felt a sharp pain in his back and leg.

Unhooking hose under tank and lost footing in mud causing employee to fall. When employee fell, he extended both arms to break the fall causing strain in neck and both shoulders.

While checking for an air leak on his equipment, the injured lost his balance and placed his hand on the exhaust while trying to stabilize himself and prevent falling. He burned his hand.

1 of 3 drive belts broke, injured shut motor down, dismounted the crusher and approached the large pulley driving the crusher in the hopper. Pulley had slowed down but had not totally stopped rotating yet. Reached around from behind the guard to grab the broken belt from the back when the remaining 2 belts caught his arm between them and the pulley causing a fracture.

While dismounting from highwall drill, the injured' s foot slipped on the steps, he fell to the ground and strained his lower back area. Doctor prescribed a muscle relaxer. No other medical and no lost time.

While bending over to pick something up from the ground the EE strained his lower back.

While helping another 18 wheel over the road haul truck driver check to see if the tail gate was closing tight, the injured got his thumb caught between the tailgate and bed when the driver closed the latch before being signaled to do so.

While dismounting from an end dump truck, the injured stepped on a dirt clod or piece of shale and turned his ankle causing a strain

While going down the steps of the Scale House, the injured fell on the bottom step, landed on a small rock and cut/burst her knee requiring stitches.

While gathering and burning brush, the injured's leg became entangled in some vines. When he attempted to pull it free, he immediately felt pain in the leg from a strain.

Employee is a Long Haul Truck Driver and was off-loading product. As he was descending ladder from top of tanker, he missed the last rung causing him to fall to the ground and strike head and neck against the ground, resulting in contusion to head and neck.

While using a porta power to remove a pin from a hoe bucket, a piece of metal popped out and struck the injured in the forehead causing a laceration requiring 10 stitches. The injured is a contract welder that works intermittently for the mine.

While stepping to the ground off his end dump truck, injured strained (pulled a muscle) his leg. Note: The company did not work on the four day Thanksgiving weekend.

While standing at the ladder on the catwalk of his end dump truck. Cleaning his windshield. The injured slipped, lost his balance and his grip on the hand holds. He fractured his wrist trying to protect himself and prevent a fall to the ground.

While traveling on a surface haul road, the injured's truck impacted the berm. He continued powering into the berm for 150' to try to get back on the road. Instead, he topped it, traveling down the rain satureated slope, sinking right front tire in the mud. Turned truck over to the right. Suffered strains from the jarring. Due to schedule w/dr, rtw delayed.

While operating his loader, the injured turned to look behind him and strained his neck.

The injured has been diagnosed as having "Carpal Tunnel Syndrome". Doctors operated on 2/20/08. They believe it is from repetitive movement while operating a bulldozer.

No medical or time lost until 2/12/08, when doctor gave the injured two prescriptioins. One was an anit-inflamitory and the second a pain pill. While breaking coal chunks in the crusher box, the injured raised a metal bar and strained a muscle on his arm. He has continued to work.

No medical until 2/19/08, when doctor prescribed an anti-inflamitory to reduce inflamation. While pinning coal, the injured jarred his back, causing a strain. No medical until 2/19/08 and still no lost time.

While turning a corner to dump a load, one set of the injured's rear tires went over the berm and pulled the rest of the truck off. The truck and cab slid backward down the face of both sides of the highwall corner about 50'. The injured was wearing his seat belt and suffered a cut on his head requiring 3 staples to close it. ER room doctor had injured take several days off work.

Company was not notified until 8/22/07 of alleged injury. No explanation other than a slip & fall 16 days earlier. Last day of work 8/18/07. Company notified after calling injured after six days of no show that he claims injury of his lower back muscle, hips & tailbone. No injury report on his written timesheet for the 7th through his last day of work on the 16th.

While letting the bed of his end dump truck down, the bed free fell, jarring him and threw the injured into the steering wheel causing a laceration to his check.

While putting the dragline walk lock on the walk shelp, the lock fell on his hand and cut it requiring stitches.

While helping remove & unload a dragline swing motor, the motor shifted & pinched the injured's foot, bruising/straining it. As a precautionary measure, x-rays were taken & the injured was given a prescriptiong for pain medication, should he need it.

While pinning coal with his loader in the pit, the loader movement strained his neck.

While working on the dragline, the injured hit his finger with the handle of a hammer, smashing or bursting it open requiring 6 stitches to close it.

While operating the water wagon in the pit, the injured lost control and ran into a pile of shot material, straining his body and hitting his head. No injuries other than strain and headache.

The injured was trying to roll a rock off the haul road and strained his back.

Notified company 4/26/06 that he received a prescription that he did not fill or use. While helping the mechanic pull a rear drive out of a water wagon, the differential moved suddenly & pinched his hand causing swelling. Diagnostic X-rays on date of injury and release to return to work on his next regular shift 4/15/06. No lost time due to injury, only the prescription.

While hauling overburden, the injured was backing up for the shovel to load him and the shovel bucket hit the bed of his end dump truck, jarring and straining the injured's neck. He missed one full shift and went home early during another shift.

While hanging a strap attached to the dragline powercord to a hook on the shoe to help move the cord, the injured got his finger caught between the strap and the shoe causing a broken finger.

Installing bearing into a radiator guard on D9L. A piece of bearing broke off and embedded itself in EE's finger on right hand, between 1st and 2nd knuckle.

While steam cleaning a piece of equipment, the injured accidently squeezed the trigger of the wand and hit the right side of his right foot. The outside of the foot received 1st and 2nd degree burns.

Injured was walking on the ground, tripped and caused pain in his ankle. He did not report the injury until 5/23/2005 and was off only one day 5/24/2005. He worked his regular shift on 5/22, the date of injury, and 5/23/2005.

While attempting to cut open an oil filter to check for trash in the filter material, the injured slipped knocking the bottom ring off and hit his thumb on the vice holding the filter. He cut hit thumb on the vice, requiring 7 stitches.

WHILE SEALING THE HIGHWALL WITH THE HOE BUCKET, THE INJURED'S HOE SLIPPED ON THE WALL, JERKING THE OPERATOR & STRAINING HIS BACK.

WHILE COMPLETING A 200 HOUR SERVICE ON A 637 SCRAPER, EMPLOYEE WAS STEPPING OFF THE FRONT ENGINE USING THE STEPS MOUNTED TO THE MACHINE. HE LOST HIS FOOTING WHILE DISMOUNTING AND STUMBLED BACKWARDS FALLING INTO THE TOOLBOX DOOR OF HIS SERVICE TRUCK. HE CAME INTO CONTACT WITH THE DOOR IN HIS BUTTOCK AREA, CUTTING HIS BUTTOCK WHICH REQUIRED STITCHES.
