The injured EE was helping another EE get posts for wedges for 2nd mining. As EE was unloading wedges from a wagon the scoop being operated by the other EE accidentally bumped the wheel of the wagon which caused it to hit EE's left knee. The scoop operator thought he was far enough away from the wagon.
While winding belt on a pull with scoop, the scoop bucket was swing toward this employee striking his left foot.
EE WAS GOING FROM CROSSCUT TO CROSSCUT GATHERING SUPPLIES. HE WAS DRIVING A 3-WHEEL MANTRIP. WHEN HE PULLED TOWARD THE INTAKE STOPPING, HE OBSERVED A ROLL OF BELT WITH A METAL TUBE STICKING OUT. HE LOADED SUPPLIES ON THE MANTRIP. WHEN HE STARTED TO BACKUP, HE FORGOT ABOUT THE ROLL OF BELT AND STRUCK HIS HAND AGAINST THE METAL TUBE STICKING OUT OF THE BELT.
THE INJURED PERSON WAS ASSISTING THE SKID STEER OPERATOR WITH MOVING PALLETS. WHILE MOVING THE LAST PALLET, HE PLACED HIS HAND ON THE PALLET THE DAE WOO OPERATOR WAS SETTING DOWN ON THE PAVEMENT. HE CAUGHT HIS LEFT INDEX FINGER BETWEEN THE PALLET AND THE FORK ON THE DAE WOO.
SHUTTLE CAR OPERATOR WAS LEAVING THE MINER WITH HIS BUGGY LOADED WITH COAL. AS HE MADE A LEFT TURN, THE WHEEL UNIT ON THE DUMPING END, REEL SIDE OF SHUTTLE CAR BROKE OFF. THE BUGGY STOPPED ABRUPTLY CAUSING THE OPERATOR TO PITCH FORWARD & TO THE LEFT. THIS CAUSED HIM TO STRIKE THE LEFT SIDE OF HIS HEAD INTO THE CONTROL AREA OF THE SHUTTLE CAR.
HEAD ROLLER OF DIRTY COAL STACKER BELT HAD MUD BUILDUP ON IT. EMPLOYEE WAS INSTRUCTED TO CLEAN ROLLER WITH BELT OFF. HE DID SO FOR A FEW MINUTES THEN TURNED THE BELT ON AND USED A SLATE BAR TO SCRAPE THE ROLLER. THE BAR CAUGHT IN THE ROLLER AND STRUCK HIM ON THE CHIN.
EE STEERED TOO CLOSE TO RIB ON DRIVERS SIDE OF SHUTTLE CAR. HIS REACTION WAS TO HIT THE BRAKES & PUT HIS LT HAND OUT TO THE RIB. THE FORWARD MOTION OF THE SHUTTLE CAR PULLED HIS ARM BEHIND HIM CAUSING A STRAIN TO LT SHOULDER. SHUTTLE CAR STOPPED WITHOUT MAKING CONTACT WITH RIB. EE'S HAND ONLY TOUCHED THE RIB.
THE DATE OF THE ACCIDENT IS WHEN THE EE THINKS THE INJURY OCCURED. HOWEVER, HE DID NOT MISS ANYWORK UNTIL 2-13-02. THE EE WAS OPERATING A SHUTTLE CAR AND SAID HE FELT PAIN TO HIS LT. GROIN AREA. HE WORKED HIS REGULAR JOB WITH NO RESTRICTIONS UNTIL 2-13-02, WHICH WAS THE SCHEDULED DATE FOR OUT-PATIENT SURGERY FOR A LEFT INGUINAL HERNIA.
THE EE HAD RAISED THE LID COVERING THE S.C. RAILAND WEDGED IT TO THE MINE ROOF TO SECURE IT FROMFALLING. WHILE HE WORKED TO UNTANGLE THE MACHINES POWER CABLE THE LID LOOSENED AND FELL (LID WAS HINGED ON ONE SIDE) STRIKING HIM ON THE HEAD ANDSHOULDER AREA. THE EE DID NOT NOTIFY THE COMP. OF THE INCIDENT UNTIL 01-03-02.
EE HAD SHUTTLE CAR WHEELS TURNED THE WRONG DIRECTION WHICH MADE THE SHUTTLE CAR STRIKE THE RIB AND EE S HEAD STRUCK THE OUTHER EDGE OF THE OPERATORS DECK CANOPY.
