ACCORDING TO PERSON INJURED AS HE WAS STANDING IN FRONT OF A LOADED SCOOP BILL THE SCOOP OPERATOR MISUNDERSTOOD A DIRECTION AND MOVED THE SCOOP FORWARD I.NAME BIG TOE ON HIS FT FOOT WAS FRA CTURED
SAFETY CHAIN CONNECTED BETWEEN SUPPLY MOTOR AND SUPPLY CAR.MOTOR AND CAR WOULDNT COUPLE BECAUSE CHAIN FELL BETWEEN COUPLERS.INJURED REACHED DOWN TO PULL CHAIN OUT.SUPPLY CAR DRIFTED BACKWARDS CAUSING CHAIN TO TIGHTEN AND CATCH HIS HAND BETWEEN TOP OF COUPLER AND CHAIN.
A PRIVATELY OWNED JEEP DRIVEN BY THE INJURED, WAS TRAVELING TOWARD THE COUNTY ROAD INTERSECTION AT THE NO 1 GUARDHOUSE AND WAS STRUCK BY A RHODEWAY TRUCKING CO. MACK TRUCK. THE INJURED STATED THAT HE DIDNT SEE THE MACK TRUCK AND WAS TRAVELING AT A NORMAL RATE OF SPEED. A STOP SIGN IS POSITIONED ON THE MINE SITE SIDE OF THE INTERSECTION
THE INJURED WAS TRAMMING THE MINER BY RADIO REMOTE CONTROL TO THE FACE. HE HAD BEEN SETTING IN THE CROSSCUT WHEN THE SHUTTLE CAR EPERATOR WENT TO GET ON THE SHUTTLE CAR. THE INJURED MOVED OVER INTO THE CENTER OF THE ENTY. HIS LIGHT WAS FACING THE MINER AND THE SHUTTLE CAR OPERATOR COULD NOT SEE HIM. THE SHUTTLE CAR OPERATOR TRAMMED ONTO HIS LEG.
OPERATOR LOST CONTROL OF TRUCK. VEHICLE HEAVILY DAMAGED.
