Employee stated that he was jarred when a rock rolled from the loader bucket and struck the bed cab protector. Medical records were not available until recently.
Employee stated that he suffered a jarring injury to his neck while operating a 685 Dresser Haul truck. Medical records concerning this event were not made available until 11/23/10.
Employee jarred his neck while dumping a haul truck. Note: Employee did not begin missing work until 6/22/10.
Employee stated that his loaded haul truck slid into the ditch along the haul road during icy road conditions and he strained his lower back while holding on to the steering wheel. EE was installing boom back in saddle on the service truck, winching cable up when the hook caught on the bumper of the service truck, came loose and hit the EE in the head over top left eye.
Employee was operating a Dresser haul truck when the truck struck a hole in the road, jarring employee's lower back.
Employee struck his right index finger on a sharp metal edge inside the loader cab, resulting in a laceration.
Approx 10X12' section of haul road collapsed into an abandoned underground miner entry. The right rear tandem of EE's 730 E Dresser fell into void created by this collapse. Truck driver thinks that he cut his finger while dismounting truck afer incident occurred. Note: 3 sutures were placed in the small bliter like flap of skin.
Employee was in the process of dumping his 16th load of rock at dump area, when the truck bed fell approx 2'. The bed did not hit the truck frame but bounced back up to a fully extended position. Employee stated that the jarring motion of the truck caused a "whiplash" type injury to his neck.
WHILE HAULING COAL OVER ROUGH ROADS, EE STATED THAT HIS BACK AND NECK STARTED HURTING. EE COULDNOT RECALL ANY SUDDEN INCIDENT OR SINGLE EVENT.
WHILE LEAVING PIT AREA LOADED ROCK TRUCK STOPPED NEAR TOP OF RAMP. EMP REFUSED ASSISTANCE OF 994 LOADER AND DECIDED TO BACK DOWN RAMP ON HIS OWN. AFTER TRUCK WAS PUT INTO REVERSE AND RETARDER APPLIED THE TRUCK FREE-WHEELED DOWN RAMP. WHEN BRAKES WERE APPLIED THE FRONT END OF THE TRUCK RAISED UP AND BACK DOWN TO GROUND.
EMPLOYEE WAS PREPPING COAL NEAR QUIT TIME. AS HE WAS COMING DOWN OFF COAL RIB HE MISSED THE RAMPHE WAS JARRED AROUND IN THE LOADER ALLOWING THE RIGHT SIDE OF HIS HEAD TO STRIKE A SUPPORT INSID E THE CAB.
WHILE DRIVING #23 COAL TRUCK DOWN "SLIGHT" GRADE(NEAR COAL BIN) THE REAR OF THE TRUCK WAS BUMPED INTO BY #25 COAL TRUCK. THE SUDDEN JOLT CAUSED THE OPERATOR OF #23 TRUCK TO TWIST HIS KNEE. NOTE: OPERATOR STARTED MISSING WORK ON 2/19/02.
EE SEEN A BONE SPECIALIST ON 10-26-00 FOR LOW BACK PAIN. HE WILL "POSSIBLY" RETURN TO WORK AFTER TAKING PHYSICAL THERAPY.
AFTER UNLOADING THE TRUCK EE WENT TO THE REAR OF THE UNIT TO CLOSE THE AUGER DOOR. THINKING EVE RYTING WAS TURNED OFF HE PLACED HIS HAND IN THE AUGER DOOR OPENING TO CLEAN THE TUBE AND WAS CAU GHT BY THE MOVING AUGER.
