A fully trained employee was riding in a 5 man mantrip leaving 77 wall going inby on 2 track when he traveled past 80 wall he jumped the switch and struck another ride parked in the spur resulting in a strained back PPE was not a factor.
A trained foreman was sitting in a ride in a switch with his lights on when the ride was struck by another ride because the driver said he could not see the ride due to the height of the canopy on his ride. The collision resulted in his back being hurt, and was caused by poor communication and the poor visibility. Lack of PPE was not a factor. Area was well lit.
A fully trained roof bolter was in the operators seat of a parked diesel mantrip. The mantrip was allowing other equipment to clear when the sitting mantrip was struck by another mantrip. This caused the injured to strike his shoulder against the mantrip. No mechanical damage was found on either mantrip.
A fully trained shuttle car operator was setting in the outby side of a mantrip which was stopped to allow traffic to switch out. The stopped mantrip was struck by another mantrip. He was performing his regular job as a trained shuttle car operator. He was wearing all of his personal protective equipment. While operating the machine, he turned his head left and felt pain in his right shoulder and neck.
Employee stuck pick hammer in a hole in the belt rail and the belt caught it and threw it out striking the employee in the chin. (7 stitches required)

He was a passenger in an enclosed diesel mantrip. He was wearing all of his personal protective equipment. The mantrip operator intentionally drove into a track switch to enter the switch, the mantrip climbed the rail and went off track causing the injured to hit his hardhat on the roof causing pain in his neck and back. The operator's training was current.
He is a miner with limited experience running a Joy 10SC32 Shuttle Car. He failed to recognize the hole in his tram road due to an inadequate examination during his pre-operational check. He dropped his shuttle car into the hole in the tram road and the jarring of the tire dropping down caused him to strike his left knee against a metal bracket in the operator's deck.

Employee was cleaning from under belt, was crawling out from under belt when he was struck by a rock that had come off of the belt.

While cleaning inby #5 drive, the employee's shovel became caught between the roller and bottom belt. The shovel was pulled through the roller causing his hand/arm to contact the roller and belt abraising his arm.

Rock dust chute-switching motor around at rock dust chute started to pull pin out of the draw bar and motor jumped and pin pinched his right ring finger.
REMOVING PIN FROM DRAW BAR THAT WAS BETWEEN LOCOMOTIVE AND ROCKDUST TANK. HE NEEDED SLACK TO REMOVE PIN. OTHER LOCOMOTIVE OPERATOR MOVED TANKS A LITTLE. HOWEVER, WHEN MOVED THE VICTIM'S RIGHT INDEX FINGER WAS CAUGHT BETWEEN PIN AND BRACKET ON MOTOR.
3F-T NORTH TRACK ENTRY. MAN WAS PUTTING TOOLS AWAY ON THE END OF TRACK JEEP, A PORTAL BUS COASTED TOWARD HIM HITTING RIGHT LEG
At 12-26 along main line haulage - operating track vehicle. Trolley pole came off the trolley wire. Man slowed down and reached for the pole. He hit his head on low hanging roof strap.
5 NORTH FACE - AT FEEDER. EE WAS OPERATING SHUTTLE CAR, GOING TO FEEDER. HE REACHED OUT TO PUSH OTHER SHUTTLE CAR'S ANCHOR AWAY FROM HIS CANOPY. HE GOT HIS RIGHT HAND CAUGHT BETWEEN HIS CANOPY AND ANCHOR.
1A - 5 NORTH BELT DRIVE AREA, UNLOADING "BLOWER" FROM SCOOP. SCOOP RAM USED TO PUSH BLOWER OFF OF BUCKET. WHEN RETRACTING RAM, RIGHT MIDDLE FINGER WAS CAUGHT BETWEEN RAM AND SCOOP BUCKET.

5 NORTH HEADGATE, CENTER BOLTER HUNG UP IN CROSSCUT BESIDE AUX. FAN. USED SCOOP CAR CABLE AND HOOK AROUND POST OF CENTER BOLTER CANOPY. WHEN PULLING WITH SCOOP CAR-HOOK CAME LOOSE-SWUNG AROUND AND HIT MAN IN LEFT LOWER BACK AS HE WAS SITTING IN CENTER BOLTER OPERATOR COMPARTMENT.
2 RIGHT - 5 SOUTH LONGWALL SECTION; MAN WAS OPERATING A SCOOP CAR. AS HE WAS GOING AROUND A TURNHIS DOOR CAME OPEN. HE STARTED TO GRAB THE DOOR AND HIS LEFT HAND WAS CAUGHT BETWEEN A PIVOT JAC K AND FRAME OF THE DOOR.
CREW HAD MOVED SECTION TRANSFORMER AND PARTS CARINTO A CROSSCUT. VICTIM WAS STANDING NEAR CORNEROF CROSSCUT. A SHUTTLE CAR WAS ANCHORED ON PARTSCAR. SHUTTLE CAR PARKED OUTBY CORNER BEGAN TO RE EL HIS CABLE UP. CABLE HIT VICTIM ON LOWER LEG.
SURFACE SUPPLY YARD UNROLLING A LARGE 12 FOOT ROLL OF 72 INCH CONVEYER BELTLINE, USING A CRANE TO HOLD REEL OF BELT UP IN THE AIR. SUPPLY CARS ARE MOVED BACK AND FORTH UNDER BELTING AND BELT I S LACED ON SUPPLY CARS. LARGE REEL SHIFTED AND CAUGHT MAN'S RIGHT LITTLE FINGER BETWEEN BELTLINEAND FRAME OF CRANE.
OPERATING SCOOP, TURNING AND TRAMMING OUT OF A CROSSCUT, DOOR OF CAB CAME OPEN, MAN REACHED FOR IT. HIS LEFT HAND WAS CAUGHT BETWEEN STEERING JACK AND DOOR. ACCIDENT DID NOT INVOLE COMPLIANCE WITH RULES AND REGULATIONS.
