The employee was taking the draw bar out of an off track lowboy. The motor pulling in bumped the lowboy and caused it to roll over EE's foot before EE could get out of the way. The following were not a factor: Regs, Skills, Proficiency, Training, Equipment, Protective Devices and Clothing. Attitude and Rules may have been a factor.

The employee was operating a motor. EE was pulling up to a parked motor when EE bumped into it and claimed to feel discomfort in the lower back. Filing under protest to comply with a citation from MSHA. An employee was tramming a motor through the South Mains Dip. An unattended motor that was up the hill drifted down and caused the collision. The employee was operating a scoop car when it ran over a bolt that was buried in mud. When the tire ran it over, the bolt flipped out of the mud and struck employee in the lip. The employee was entering the #2 portal shop in a trolley jeep when the trolley pole struck EE in the arm and injured elbow. The following were not a factor: Rules, regs, mining equipment or systems, skills, proficiency, training and protective devices on equipment or clothing. Attitude may have been a factor.

Employee was on a ladder that was on the belt to set timbers during roller service. The group failed to follow company policy by confirming with the designated person that they were on the belt. The belt turned on and EE fell off the ladder to the ground injuring shoulder. Regs, skills, proficiency, training and protective devices were not a factor. Rules and attitude factored

Employee was standing between two trolley jeeps when the brake on the inby jeep was released, by another miner, and the jeep drifted back into his leg. not adhering to company rules was a factor in the incident. The employee was operating a shuttle car, hit a bump and bounced in his seat and then felt a pain in his side. The result of the injury was a strain to his lefty side ribs. The employee did not start to lose time until 06-17-2014

Riding in a trolley jeep and the trolley came towards the injured and he tried to stop the pole. Strained shoulder.

Employee was rolling up line brattice to get a scoop tractor by him. The scoop was turned off and the bucket was 6 inches off the floor. The operator touched the bucket lever and lowered the bucket on to his foot. Employee continued work until 09/18/2013 when a broken bone was found in his foot.

Employee was operating a shuttle car when he ran over a large rock. The SC bounced causing him to hit his head on the canopy. Lower back strain. Started losing time on 5/20/13

Employee was cleaning conveyor belt in a low area. The belt caught his shovel and pulled it into the structure.

EE was observing the CM load loose rock from an overcast. A roof bolt in the gob flew and hit the EE in the chin. 3 Stitches

Employee was operating # 17 shuttle car after drag rail was installed and hit a stone in roadway and got high centered causing him to strike his head on canopy. Operator was not wearing safety belt. Operator complained of pain in head area and was transported to hospital

Employee was a passenger in a Jeep (Jitney). The Came off trolley pole came off the trolley wire, struck on a roof strap and was pulled out of the base. The pole struck the employee on his left elbow.

Employee was operating #17 man bus when trolley pole came off of trolley wire and struck him in the face. Began losing time 8-16-11.

Employee was testing brakes when the machine cat 966 started over a hill. He dropped the bucket and his head hit the windshield - sutures in forehead.

The employee was driving a 777 truck at the Preparation Plant when his incident occurred. The truck shifted hard causing the operator to jar his lower back. Employee began losing time as of 03/26/09.

The employee was transporting a load of refuse from the new refuse bin to the 1050 bench site. While driving to the area, the 777 haul truck hit a hole, causing employee to bounce up and down in his seat. He began to feel pain in his lower back soon after.

Employee was operating a jeep on the track. The trolley pole came off the trolley wire. He reached out and grabbed the pole and felt something "pop" in his left forearm. Last day worked 7/2/08.

Standing in the middle of track on a turn, waiting for the scoop car to come down the track to knock out a brattice wall. When the scoop got to the end of the rail it caught the rail with the battery end of the scoop causing the rail to push over approximatly four feet catching employee's right foot. (sprain right foot)

Running scoop and drove across concrete block. The jar put strain on lower back. Note: Lost time as of 1/31/08.

Unloading supply cars off of 3 west ramp area. While turning scoop car, had left hand resting on kitchen compartment. When scoop turned completely back end of scoop (kitchen) compartment struck and lacerated left hand.

Moving scoop car from entry into a crosscut and got fingers pinched between front and rear sections of machine.

Five motor lost brakes & hit #4 motor with six loaded supply cars at 10 West switch. Everything went over hill, out of control. #4 motor went off track into xcut at bottom of hill, knocking cribs down on top of motor, covered injured with cribs, loose coal & concrete blocks.

WALKING PAST COAL HILL #2 TAIL. STONE CAME OFF OF BELT & STRUCK RIGHT SIDE OF FACE.

An employee was riding on the #3 track jeep when it became disabled, #22 mantrip which was behind the jeep struck the jeep causing him to fall out striking the right side of his body against the bottom rib.

AFTER LOADING A ROLL OF BELTING ONTO A LOWBOY, HE WAS WALKING ALONG THE TRACK ENTRY. THE LOWBOY WAS BEING PULLED BY A BUS. AS IT PASSED THE EMPLOYEE, IT STRUCK HIM ON THE LEFT ANKLE.

EMPLOYEE WAS TRAVELING TO HIS WORK LOCATION ON A TRACK MOUNTED JEEP. THE JEEP STRUCK A BROKEN RAIL AND STOPPED SUDDENLY CAUSING THE EMPLOYEE TO STRIKE HIS LEFT KNEE AGAINST THE INTERIOR COMPARTMENT OF THE JEEP.

EMPLOYEE WAS DUMPING FLY ASH AND THE TRUCK SLOWLY UPSET TO THE LEFT SIDE.

EMPLOYEE WAS RIDING ON #2 LOCOMOTIVE PUSHING A SHEILDS. CARS BACK TO THE END OF TRACK AT TOP END OF 21 WEST LONGWALL SET-UP SECTION. HE TURNED HIS BACK LOOKING THE OTHER WAY, WHEN HE STRUCK HIS HARD HAT AGAINST A 4"BEAM THAT WAS CROSSING HAULAGE.

AN EE WAS OPERATING A SHUTTLE CAR WHEN HE TRAMMED OVER A POT HOLE, AND HE BOUNCED, STRIKING HIS HEAD AGAINST THE CANOPY. HE LATER FELT PAIN IN HIS LOWER BACK AND RIGHT LEG.

THE EMPLOYEE WAS ASSISTING IN RECOVERING AN IMPACT BED ROM THE 010 24 WEST BELT TRANSFER. HE WAS HOOKING UP A CHAIN TO THE IMPACT BED. WHEN THE SCOOP TRAMMED FORWARD (TO IMPACT BED) THE BUCKET CAUGHT A TRANFER BEAM LYING ON THE BOTTOM, PUSHING IT FORWARD, PINCHING THE UPPER LEFT ARM AND CAUSING BRUISING AND LACERATION.

EE WAS OPERATING A LOCOMOTIVE ON THE A WEST TRACK. TOP WAS LOW IN THE AREA. HE HIT HIS HEAD AND SHOULDERS ON THE TOP. HE FELT PAIN IN NECK, BETWEEN SHOULDERS IN HIS BACK, LOWER RIGHT BACK AND RIGHT HIP. NO DAMAGE TO EQUIPMENT OR MINING OPERATION. NO OTHER FACTORS INVOLVED.

EE WAS STARTING TO OPERATE THE LOCOMOTIVE. HE HAD REACHED OUT WHILE STANDING IN THE OPERATOR'S COMPARTMENT TO GET THE TROLLEY POLE. AS HE PUSHED DOWN ON THE POLE, HE FELT PAIN IN HIS LEFT SHOU LDER. THIS SHOULDER HAS BEEN HURT ON A PRVIOUS INJURY. NO OTHER FACTORS WERE INVOLVED. NO DAMAGE OR IMPAIRMENT TO THE MINING EQUIPMENT OR OPERATION.

STRUCK BY TRUC WHILE TRYING TO START IT.

EE CALLED FOR CLEARANCE TO TRAVEL OUT OF THE WEST CROSSOVER TRUCK ONTO THE MAIN NORTH TRACK. ANOTHER EE WAS OPERATING A BUS BATTERY PORTAL BUS ON THE MAIN NORTH TRACK. EE FAILED TO BE SURE THE ASSIGN NORTH TRACK WAS CLEAR BEFORE DRIVING HISJEEP OUT OF THE WX0 TRACK, AND THE BUS STRUCK THE JEEP, KNOCKING THE EE OUT OF THE JEEP AND ONTO THE MINE FLOOR. FRACTURING RIBS AND BRUISING BO

EE WAS CHECKING BELT ROLLERS ALONG THE SOUTH 60 BELT. HE GOT TOO CLOSE TO A ROLLER AND CAUGHT HIS LEFT THUMB BETWEEN THE TOP ROLLER AND BELT. NO DAMAGE TO MINING EQUIPMENT OR OPERATIONS. THE E MPLOYEE SUFFERED A DISTAL FRACTURE TO HIS LEFT THUMB AND LOST HIS FINGERNAIL.

EMPLOYEE WAS RIDING IN A PORTAL BUS TOWARD HIS SECTION. THE PORTAL BUS OPERATOR WAS STOPPING FOR A LOCOMOTIVE AHEAD OF HIM BUT HE DID NOT SEE THE LOWBOY COUPLED TO THE LOCOMOTIVE. THE PORTAL B US RAN INTO THE LOWBOY AND THE EMPLOYEE RIDING IN THE LOWBOY STRAINED HIS LOWER BACK AND NECK WHEN IT SUDDENLY STOPPED.

EMPLOYEE WAS AT THE SLOPE BELT TAILPIECE AREA. HIS FOOTING SLIPPED ON A CATCH BOARD ON THE FLOOR AND HE FELL INTO THE BELT. HIS LEFT FOREARM WASPINNED BETWEEN THE MOVING TOP BELT AND A TOP ROL LER FOR THE BELT. WHEN HE PULLED HIS ARM LOOSE APIECE OF SKIN WAS TORN FROM HIS ARM. THERE WAS NO IMPAIRMENT TO THE MINING OPERATION.

EMPLOYEE WAS HOLDING UP A LOWBOY TONGUE BY HAND AS ANOTHER EMPLOYEE WAS OPERATING A LOCOMOTIVE THAT WAS PUSHING ANOTHER LOCOMOTIVE THAT WAS GOING TO BE COUPLED TO THE LOWBOY TONGUE. EMPLOYEE DROPPED THE LOWBOY TONGUE AND IT SMASHED HIS RIGHT ANKLE AGAINST THE TRACK RAIL. NO DAMAGE TO MINING OPERATION. INJURY DID NOT INVOLVE COMPLIANCE, JOB SKILLS, PROFICIENCY, TRAINING, ETC.
