This was originally a first aid and became a recordable on January 19 when he was given an oral antibiotic for an infection.

WHILE OPERATING A 777 CATERPILLAR END DUMP HAULING COAL, THE WORKER FELT A PAIN IN THE LEFT SIDE OF HIS NECK AS A REACTION TO HITTING A HAULWAY POTHOLE AT THE STOCKPILE.
ON 5-24-00 AT APPROX. 11:55 AM A 100 TON END DUMP WAS LOADING IN A PIT. AFTER BEING LOADED TO 95 TONS DRIVER STARTED TO PULL AWAY, LOAD CAUSED HAULER TO STAND ON END TRAPPING DRIVER EE IN CAB , MORE THAN 30 MINUTES. EE STATED AS HE STARTED UP RAMP FELT HE HAS SOME LOSS OF CONTROL. TRUCK REARED UP, SETTING DOWN ON END TRAPPING DRIVER IN CAB. OPERATOR WAS RESCUED BY ONSITE EMERGENCY
