EE was pulling a miner conveyor chain with a scoop and another chain. The chain attached to the scoop broke and struck EE on the left knee resulting in a cut requiring three stitches.
EE was operating a scoop hauling cable bolts. A cable bolt became fouled on the rib, sprang back and struck him on the left eye brow.
EE WAS UNHOOKING A CHAIN FROM THE TAILPIECE THAT WAS ATTACHED TO A SCOOP AND DID NOT GIVE THE SCOOP ENOUGH TIME TO COME TO A COMPLETE STOP AND GOT HIS FINGER CAUGHT IN THE CHAIN, RESULTING IN A CUT TO THE END OF HIS FINGER.
EE was operating a mantrip and a pogo stick on top of mantrip caught a stiff jack at a belt drive and sprang back and hit him on the nose.
EE was greasing the tail roller on his shuttle car and a scoop came thru a set of fly pads with his bucket up and the bottom (flat) part of the bucket struck EE and knocked him down, injuring his shoulder and hip.
EE was walking between two track motors that were parked 8' to 10' apart and one motor(with operator in deck) suddenly moved forward and pinched the back of EE leg between motors.
EE had put rock dust in a scoop bucket and laid his hand between the ejector blade and the back of the scoop bucket. He told the operator he was done and the operator started the scoop and pulled on the ejector blade lever to release the hydraulic brake, resulting in his fingers(2) on the left hand being broken.
EE was helping haul supplies to the # 1 section. As the track motor rounded a curve on a steep grade, the motor man couldn't hold the motor back and it slid down the grade and struck another motor that was sitting at the bottom of the hill. EE was in the opposite end of the motor and received bruises to the back form being jolted against the track motor frame.
EE was scooping up emergency supplies to be moved. He had his right hand on the edge of the scoop canopy to lift himself up to see the chain that he had attached to the supply sled. As he trammed the scoop forward, he steered the scoop and mashed his hand against the coal rib. Third and fourth fingers were lacerated, fifth finger was lacerated and fractured.
Employee was tramming scoop by two co-workers when the plastic stop on the center section was sticking out beyond scoop deck and struck victims's right knee.
While working on a forklift, EE took transmission out of gear and bumped started to make sure it was out of gear, employee started forklift when it turned to move to back of forklift, & it started rolling. It started rolling-hit employee knocked him down and ran over both legs, breaking ankle.
EE had repaired a broken conveyor chain on a shuttle car. He was running the chain to check it and heard it making an unusual noise. He was looking under the shuttle car to locate the source of the noise & inadvertently place his left hand on the running conveyor chain resulting in it being pulled under the chain, mashing it. This occurred in the #2 entry of the 003/004 MMU.
EE was tramming a scoop in the #6 entry, 3 breaks outby the section, and ran over a crib block causing him to be jolted in the operator's compartment, striking his head over the underside of the scoop canopy causing injury to his neck and back.
EE was operating a rail runner and entered a dip in the track where another motor was located. He struck the mantrip motor, causing injury to lower right leg. EE did not see medical attention until 9/17/05. He was diagnosed with a hairline fracture. He is presently working light duty. Warning lights were on, which indicateds that there was another motor in the dip.
MAN TRIP LOST TRACTION ON AN INCLINE AND STRUCK BELT RIDE. THIS HAPPENED ON TRACK UNDERGROUND.
EE WAS OPERATING A SCOOP IN THE #6 ENTRY 4 BREAKS OUTBY THE SECTION AND RAN OVER A CRIB BLOCK. HE WAS BOUNCED IN THE DECK OF THE SCOOP CAUSING HIM TO HIT HIS HEAD OVER THE SCOOP CANOPY & HIS BACK ON THE SCOOP FRAME OF THE DECK CAUSING INJURIES.
EE WAS RIDING A SCOOP USED TO HAUL EMPLOYEES TO THE TRACK. HE WAS IN THE SCOOP BUCKET AND SCOOP RAN OVER A ROCK CAUSING EE TO LOOSE HIS BALANCE. RIGHT LEG CAME OUT OF BUCKET AND WAS TWISTED UN DER BUCKET. EE WAS DIAGNOSED WITH A SPRAINED KNEE BUT CONDITION WORSENED AND EE WAS PLACED ON RESTRICTED DUTY ON 1-15-04. EE MISSED NO WORK AND PERFORMED REGULAR DUTIES UNTIL THEN.
EE WAS TRAMMING SCOOP DOWN ENTRY TO GET A LOAD OF BLOCK AND RAN OVER A ROCK IN ROADWAY AND WAS JOLTED IN DECK CAUSING BACK INJURY.
EE WAS WORKING ON MECHANIC BUS WHEN A SCOOP CAME AROUND CORNER OF INTERSECTION AND BUMPED HIM BELOW LEFT KNEE WITH SCOOP BUCKET. EE WAS TRANSPORTED OUTSIDE AND TAKEN TO HOSPITAL E.R. WHERE HE WAS TREATED FOR BRUISES AND STRAINED KNEE. HE WAS KEPT OVERNIGHT FOR OBSERVATION AND RELEASED ON 7-29-03.
EE WAS RIDING IN THE BUCKET OF A SCOOP USED FOR MANTRIP. EDGE OF SCOOP BUCKET CAUGHT AN OLD ROOF BOLT LYING ON MINE FLOOR, CAUSING IT TO FLIP UP & STRIKE EE ON HIS HEAD BEHIND HIS LEFT EAR. EE WORKED UNTIL 7:00 P.M., THEN CAME OUTSIDE AND TRANSPORTED TO E.R. HE WAS TREATED AND RELEASED FOR A BRUISE.
EE WAS LOADING INTO MANBUS TO COME OUTSIDE. HE REPORTED THAT MANBUS PULLED OUT BEFORE HE WAS SEATED AND CAUSED LEFT LEG TO TWIST UNDER MANBUS. WITNESSES SAID THAT HE WAS FULLY SEATED AND READY TO GO.
RIDING MANTRIP TO SURFACE. HE RAISED UP TOO SOON AT PORTAL AREA AND STRUCK NOSE ON METAL BEAM THAT HELPS SUPPORT THE MINE ROOF.
EE HAD BEEN HELPING TO ROCK DUST. WAS RIDING IN SCOOP DIPPER. SCOOP HIT A HOLE BOUNCED EE INTO MINE ROOF. EE INJURIES CONSIST OF BRUISED RIBS ADN STERNIUM AND FRACTURE TO LOWER BACK.
