Mostrando los 48Employee stepped out of the motor without getting the brake set at the #5 switch when the motor rolled over his right foot causing a distal fracture of his 3rd metatarsal. He was excused from work.
Injured employee was operating a shuttle car when he hit a rut in the roadway causing him to hit his head against the canopy. He continued to work and went to the ER later due to increased pain. Initial ER physician released him to return to work but due to continued pain he went to another ER on 01/13/12 and he was excused from work and referred to neurological services.
Injured employee was struck on the upper back and neck by a rock weighing approx. 6.3lbs that fell approx. 70 feet from above from the RC4 belt. Injured employee was treated by EMT's and EMS then airlifted to CAMC-General ED. He was admitted for observation and diagnosed with vertebrae fractures of the neck and tissue bruising.
EE was riding on the mantrip, while passing through the air lock doors one of the doors began to close, that's when the member attempted to push the door open with is foot getting his left foot caught between the door and the mantrip. He was transported to the ER and diagnosed with a sprain and swelling. Member was placed on light duty for 1 week by the ER physician.
Injured employee was removing volute liner (Cyclone Pump) using a fork lift to raise 1 side of the liner to remove the chain when the liner slid off the fork and onto his hand. He received 20 sutures.
Injured employee was riding the mantrip to the surface when the mantrip derailed. Employee struck against the inside of the mantrip injuring his left ribs causing bruising and a hematoma on his left rib cage. He went to the ER later that evening and the X-Rays were negative for fractures. He was excused from work until further notice.
Employee was operating the miner, while loading the shuttle car, the car moved causing employee to be pinched between the shuttle car's frame and the rib. He was taken to CAMC General ER were he was diagnosed with tissue bruising. He is off work awaiting to see specialized physician.
Employee exited the mantrip to throw the track switch. The mantrip operator wasen't paying attention and ran into his right foot. Employee was transported outside and then drove himself to the hospitial. He saw a specialist on 1/31/2007 and was diagnosed with soft tissue damage to his right ankle. A cast was applied to help support the ankle from further damage.
The scoop that employee was running became hung in a bad hole. The section scoop came down to help him out. EE doesn't exactly know what happened except that he hit his head on the scoop canopy.
EE was standing at the chain link fence of the R-8 belt drive. The drive chain broke hitting him on the right hand, amputating his right finger at the firsty joint.
Running shuttle car to miner. On the way back to feeder, injured hit hole in mine floor. Hitting this hole caused him to jarr his lower back.
As he was spotting shields he had his hand lying on a panline and a rock traveling down panline struck his left hand causing a laceration to the palm.
EE WAS DRIVING MANTRIP WHEN TAPE FELL FROM THE OPERATOR'S STATION. HE REACHED DOWN TO PICK IT UP WHEN HIS LEG SWUNG OUT AND STRUCK THE RIB, FRACTURING HIS BIG TOE ON HIS LEFT FOOT.
EE WAS OPERATING THE #2 15 TON BROOKVILLE LOCOMOTIVE WITH NO SUPPLY CARS AT THE TIME OF THE ACCIDENT. HE WAS TRAVELING OUTBY UNDER THE #10 BELTLINE WHEN A PIECE OF CHAIN LINK FENCE GUARDING CAUGHT UNDER THE OPERATOR'S CANOPY & CAME INTO THE OPERATOR'S DECK STRIKING HIM ON THE LEFT EAR. HE WAS TRANSPORTED TO THE SURFACE & TAKEN TO CAMC WHERE SUTURES WERE APPLIED.
EE WAS OPERATING THE MANBUS AND TRAVELING ALONG OLD 6-BELT. AS EE REPOSITIONED HIS LEGS, HIS LEFT KNEE CAUGHT THE END OF THE BELT RAIL, CUTTING HIS KNEE.
He operated shuttle car in #3 heading when the accident occurred. He struck a depression in the roadway causing him to strike his head against shuttle car canopy. Transported to surface then hospital, he was diagnosed with sprain neck, suffered headaches. Continued work but saw dr on Oct 12. Dr advise him to say away from work until MRI was completed.
Ee was pulling a dead mantrip when the chain broke under stree striking ee above the left eye.
EE was standing approx 20' out by the tailpiece at the face phone watching the tailpiece. He had his left hand on the monorail when a piece of rock fell striking his left hand. His hand was cut and he ceased to work and was transported to BMH to seek medical attention.
EE WAS BACKING SCOOP OUT ON TO THE TRACK TO MANEUVER A POWERCENTER ON A FLATCAR WHEN HE BACKED AROUND THE CORNER A CABLE BOLT FLEW UP FROM THE GROUND AND HIT HIM IN THE MOUTH. EE RECEIVED SEVE N SUTURES.
OTHER, ANOTHER, AND EE WERE UNLOADING SUPPLIES ONT HE TAILGATE SECTION WITH A SCOOP. HE WAS ON THE CAR BOOKING THE CHAIN END. OTHER WAS OPERATING THE SCOOP. WHEN HE TRAMMED FORWARD THE RAM DRO PPED PINCHING EE FOOT BETWEEN THE RAM AND THE CAR. EE WAS TREATED BY GMTS AND TRANSPORTED TO BONE MEMORIAL WHERE HE WAS DIAGNOSED WITH A CONTUSITON, RELEASE AND INSTRUCTED TO FOLLOW UP WITH A
EE AND CREW WERE RIDING IN THE MANTRIP OF THE SURFACE,AT APPROX.25 CROSSCUT ON #3 BELTLINE. OPERATOR OF THE MANTRIP WARNED THE CREW THAT THE BRAKES WERE MALFUNCTIONING AND WOULD NOT SLOW THE MANTRIP. AT SOME POINT ON #3 BELTLINE MR MCKINNE JUMPED FROM THE MANTRIP CAUSING MULTIPLE STRAINS AND CONTUSIONS. THE MANTRIP DERAILED IN CURVE AT #2 BELTDRIVE STRIKING #1 BELTLINE
EE AND #2 SECTION CREW BOARDED THE MANTRIP TO THE SURFACE. AT APPROX. 25 CROSSCUT ON #3 BELTLINEOPERATOR OF THE MANTRIP WARNED THE CREW THAT THEBRAKES WERE MALFUNCTIONING AND WOULD NOT SLOW TH E MANTRIP. AT SOME POINT ON THE #3 BELTLINE EE JUMPED FROM THE MANTRIP CAUSING MUSCLE STRAINS. THE MANTRIP THEN DERAILED IN A CURVE AT #2 BELT DRIVE STRIKING THE #1 BELTLINE.
EE RAN OVER A LARGE ROCK IN THE ROADWAY WHILE RUNNING THE SCOOP. HE HIT HIS HEAD ON THE CANOPY AND JAMMED HIS BACK WHEN HE CAME DOWN ON THE SEAT.
A CO-WORKER WAS BRINGING SUPPLIES FROM SURFACE TO SECTION #2. HE HAD CALLED FOR TRACK CLEARANCE. EE LEFT THE #1 SECTION WITH LOCOMOTIVE & A LOAD OF GARBAGE BUT FAILED TO OBTAIN CLEARANCE FROM SECTION TO SURFACE. THE 2 LOCOMOTIVES COLLIDED IN A SWAG ON #4 BELT. EE WAS TAKEN TO HOSPITAL, TREATED & RELEASED WITH ONE WEEK OFF.
INJURED WAS WATCHING ANOTHER EMPLOYEE CLEAN AFTER THE BELT HAD BEEN PULLED BACK. AS THEY WERE CLEANING THE SCOOP CAUGHT A BELT RAIL AND KICKED IT UP STRIKING INJURED IN RIBS & HEAD.
INJURED WAS MOVING A BUNDLE OF CABLE BOLTS WITH SCOOP. AS INJURED WAS TRAMMING BACK BY THE BUNDLE, HE CAUGHT A BOLT AND IT FLIPPED UP & CAUGHT INJURED ON FOREHEAD.
WHILE SITTING IN THE OPERATOR'S DECK OF 15 TON LOCOMOTIVE, INJURED STRUCK HEAD AGAINST CANOPY WHEN MOTOR WAS JARRED. TAKEN TO HOSPITAL AND DIAGNOSED WITH MUSCLE STRAIN IN NECK. ACCIDENT OCCURR ED A #10 BREAK ON #1 BELT.
EE WAS WALKING SIDE A FORKLIFT AND THE PATH NARROWED. HE STEPPED UP ON A MUDDY EMBANKMENT TO LET IT PASS. THE EMBANKMENT GAVE OUT AS THE FORKLIFT WAS PASSING CAUSING EE'S LEGS TO BE RUN OVER BY REAR TIRES OF FORKLIFT.
EMPLOYEE WAS OPERATING THE 15 TON BROOKEVILLE MOTOR WHEN HE WRECKED IN THE SWITCH AT 5 BK 3D AT THE SPUR SWITCH. HE BRUISED HIS RIGHT SHOULDER.
INJURED WAS OPERATING THE MANTRIP TAKING HIS CREW OUTSIDE WHEN THE ACCIDENT OCCURRED WHILE TRAMMING THE MANTRIP BY THE 5 HEADGATE MOTHER DRIVE HIS RIGHT ARM GOT CAUGHT BETWEEN MANTRIP AND MOTO R FOR BELT HEAD THAT WAS SETTING BY TRACK. HE WAS TAKEN TO CAME GENERAL WHERE SURGERY WAS DONE TO REPAIR INJURIES. DIAGNOSED WITH MULTIPLE INJURIES INCLUDING FRACTURES ABOVE AND BELOW ELBOW A
EE WAS HELPING THE SCOOP OPERATOR UNLOAD A POWER CENTER FROM A FLATCAR. THE OPERATOR BACKED UP SO HE COULD PICK UP A CRIB BLOCK OFF THE BOTTOM. WHEN HE BENT OVER, THE SCVOOP MOVED AND CAUGHT H IS HAND BETWEEN THE SCOOP & THE CAR.
INJURED WAS IN THE PROCESS OF CHANGING BATTERIES IN SCOOP, WHEN HE RAN OVER CRIB BLOCK. THIS CAUSED HIM TO BE JARRED INSIDE CAOPY AND HE STRUCK HIS HEAD WHEN HAT FELL OFF. TAKEN TO BOONE MEMOR IAL AND RECEIVED TWO STITCHES.
EE WAS THROWING A TRACK SWITCH HANDLE TO LET A MOTOR THROUGH. THE HANDLE WAS NOT IN THE LOCKED POSITION WHEN THE MOTOR WENT THROUGH THE SWITCH CAUSING THE HANDLE TO JUMP UP AND HIT HIS KNEE.
EE STEPPED BETWEEN THE SCOOP & THE CHARGER TO KEEP THE BATTERY CHARGING CABLE FROM BETWEEN THE SCOOP & THE CHARGER. THE OPERATOR TOUCHED THE TRAM PEDAL AND THE SCOOP MOVED BACK INSTEAD OF FORW ARD, CAUSING THE INJURY. THE DIRECTIONAL SWITCH WAS FOUND TO BE IN GOOD WORKING ORDER AT THE TIME.
EE WAS OPERATING A MANTRIP, TRAVELING TO 5 HEADGATE. MOTORMAN AND A FLAT CARWERE COMING OUT OF 5 HEADGATE. NEITHER WERE AWARE OF EACH OTHER ON THE TRACK, RESULTING IN A COLLISION. EE SUSTAINED A BROKEN ANKLE.
INJURED WAS MOVING FORKLIFT WHEN HE HIT THE BACK OF HIS HEAD ON THE CROSSBAR THAT IS MOUNTED UNDER THE DECK. HE WAS TRANSPORTED TO BOONE MEMORIAL HOSPITAL WHERE HE RECEIVED 3 STAPLES. HE THEN RETURNED BACK TO WORK.
EE HAD COMPLETED HANGING FLYPADS. AS HE WAS WALKING AWAY FROM FLYPADS, THE SHUTTLE CAR OPERATOR RANG BELL THREE TIMES AND THEN PROCEEDED THROUGH CURTAINS. THE SHUTTLE CAR HIT INJURED IN THE BA CK OF HIS LEGS AND CARRIED HIM AS IT HIT THE PARKED SCOOP. EE WAS TRANSPORTATED TO HOSPITAL. INJURY RESULTED IN BI-LATERAL AMPUTATION OF LEGS BELOW KNEES.
EE WAS WALKING BESIDE SCOOP WHEN BELT STAND CAUGHT BELT IN ROADWAY AND FLEW UP AND HIT HIM IN THE BACK OF THE HEAD.
INJURED WAS RIDING MANTRIP OUTSIDE AT END OF SHIFT. HE WAS RIDING ON THE OUTBY END OF THE SHIFT. THE MANTRIP HIT A PARKED MOTOR. THE IMPACT CAUSED HIM TO HIT HIS HEAD ON THE MANTRIP.HE WAS NOT WEARING A HARD HAT AT THETIME OF INCIDENT. HE WAS TRANSPORTED TO HOSPITAL WHERE HE RECEIVED 3 STITCHES AND 8 STAPLES.
EE WAS UNLOADING BELT STRUCTURE OUT OF A SCOOP BUCKET WHICH WAS POSITIONED BETWEEN THE CABLE CAR & THE BELT LINE. EE STEPPED BETWEEN THE BUCKET AND THE CAR. THE SCOOP OPERATOR DIDN'T SEE HIM S TANDING THERE AND REVERSED THE SCOOP WHICH CAUGHT HIS KNEE.
EE WAS BACKING THE D10 REFUSE DOZER WHEN THE LIGHTS ON THE STACKER MADE IT HARD TO SEE THE REFUSE PILE. EE BACKED INTO THE PILE & STATED HE FELTPAIN IN HIS NECK. RECOVERY TIME IS UNKNOWN
INJURED WAS ATTEMPTING TO MEASURE THE LENGTH OF A BELT ROLLER WHILE THE BELT WAS RUNNING. HE TRIED TO MEASURE THE BOTTOM ROLLER BY STRETCHING THE TAPELINE UNDERNEATH THE BELT. WHILE DOING SO, HIS ARM WAS CAUGHT BETWEEN THE ROLLER AND THE BOTTOM.
EMPLOYEE WAS ON A LADDER BESIDE THE TRACK AT SHED TAKING A CHECK SURVEY. THE MANTRIP HAD EXITED #1 SECTION AND TURNED THE CURVE. HE DID NOT SEE THE LADDER. MANTRIP SLID INTO LADDER, CAUSING LA DDER TO LEAN AGAINST BELT INJURING RIGHT KNEE. HE WAS TAKEN TO HOSPITAL WHERE X-RAYS WERE NEGATIVE.
EE WAS SITTING IN THE MANTRIP WHEN IT WAS STRUCKFROM BEHIND BY A MOTOR WHICH HAD BEGAN SLIDING. EE STRUCK HIS FOREHEAD AND LEFT HAND ON THE CANOPY OF THE MANTRIP.
EE WAS BRINGING THE MANTRIP OUTSIDE. HE CAME UPON A RIB ROLL. THE MANTRIP HIT THE RIB ROLL. THE ROCK CAME DOWN THE SIDE OF THE BUS, CATCHING HIS SCSR SHOVING HIM BAKC INTO THE OPERATOR'S COMPA RTMENT, CAUSING THE INJURY.
INJURED WAS ADJUSTING THE TIALPIECE ON THE LONGWALL SECTION WHEN A PIECE OF ROCK CAME OFF OF THE BELT AND STRUCK HIM ON THE RIGHT KNEE. EE WAS TAKEN TO THE HOPSITAL BY AMBULANCE. A BRACE WAS PLACED ON THE RIGHT KNEE DUE OT AN ACCUMULATION OF FLUID/BLOOD ON THE KNEE.
INJURED WAS ASSISTING THE BELT MOVE CREW IN THE PROCESS OF ADVANCING THE BELT ON SECTION. HE WAS OPERATING A SCOOP; USING A CHAIN TO PULL THE TAIL PIECE BACK WHEN THE CHAIN BROKE AND HIT HIM I N THE MOUTH. HE SUSTAINED A LACERATION TO HIS MOUTH AS A RESULT. HE WAS TAKEN TO THE HOSPITAL AND RECEIVED TWO STITCHES.
