Employee was operating a shuttle car and was trying to move a ventilation curtain with EE's left hand and caught EE's left ring finger between coal rib and canopy of scoop.

EE was attempting to drive onto drill bench when EE hit a bump in the road, causing EE's head to strike ceiling of truck cab. This action resulted in an injury to EE's neck.

Truck driver tried to downshift while going downhill and did not get the truck in gear, then the truck ran down hill and turned over on private property approximately 1000 feet away. The driver bumped his head requiring stitches.

Employee was operating a rock truck when he encountered rough terrain, causing the truck seat to bottom out, resulting in neck and back pain. The employee began missing work on August 5, 2013, as a result of this injury.

Employee twisted his left knee mounting a mechanic's truck.

Employee had proceeded off the job site when the brakes failed in his personal vehicle. The employee then attempted to maneuver the vehicle into a ditch to reach a stop, which resulted in the vehicle flipping several times. The employee sustained fractured ribs and vertebrae as a result of this incident.

Coal truck driver was traveling in 4th gear at approximately 10-12 mph when he heard a noise. After hearing the noise, he let off the fuel, causing the load to shift & the truck to overturn.

Employee was operating a Personnel Carrier when he ran over a rock in the roadway, causing the steering wheel to suddenly turn, striking his right hand. The employee sought medical treatment for this injury at a later late & reported to Premier Elkhorn Coal Co. on 2/6/08, that he had suffered a fracture as a result of the accident that occurred on 1/14/08.

The rock trk driver was traveling downhill from pit to the dump site on the haul road when he turned abruptly to avoid an oncoming coal truck. This maneuver caused the driver to run through an 8-ft berm & fall several ft into the pit.

Driver stalled truck & it wouldn't start back up. Employee attempted to put it in reverse to roll start it & lost control. Truck traveled a total of 100'. Truck backed into the berm & overturned.

The injured employee was operating a four-wheeler which struck a Mac 8 that was stopped in front of him. This caused the steering wheel to spin around and strike the top of the employee's right hand, resulting in a fracture.

THE EE WAS TRAVELING IN A MANTRIP WHEN IT DIPPED DOWN. THIS MOTION CAUSED THE EE'S KNEE TO STRIKE THE ROOF, RESULTING IN A LACERATION THAT REQUIRED STITCHES.

THE SCOOP BUCKET THAT THE EE WAS RIDING IN RAN OVER A ROOF BOLT ON THE MINE FLOOR. THE ROOF BOLT THEN FLIPPED UP, AND STRUCK THE EE'S HEAD AND NECK.

EE WAS OPERATING SHUTTLE CAR AND WAS RETURNING TO THE MINER. THE SHUTTLE CAR BRAKES FAILED AND EE STRUCK MINER WITH SHUTTLE CAR-EE EXPERIENCED PAIN IN BACK.

EMPLOYEE STATED THAT HIS BACK WAS SORE AND HURT AFTER HE WENT THROUGH A DIP IN THE ROAD AND HIS SEAT BOTTOMED OUT.

BODY WAS JOLTED WHEN DUMP TRUCK WAS BEING LOADEDWITH ROCK RESULTING IN STIFFNESS IN NECK.

EMPLOYEE STATED THAT HIS BACK HAD BEEN INJURED ON 4/18/00 WHILE SITTING IN HIS ROCK TRUCK BEING LOADED BY A 992C.

EMPLOYEE WAS OOPERATING LOADER BREAKING COAL IN THE PIT, WHEN HE POSITIONED LOADER TO CLOSE TO EDGE OF COAL OVERTURNING LOADER ONTO ITS RIGHT SIDE. HIS RIGHT ARM/SHOULDER HIT AGAINST RIGHT SID E OF LOADER RESULTING IN BRUISED SHOULDER.

EMPLOYEE WAS OPERATING A 320 LOADER NAD CUT HTE ENGINE TO WAIT FOR A BLAST TO BE PUT OFF, AND WHEN HE STARTED TO START THE ENGINE AGAIN, HE LOADER STARTED ROLLING DOWN THE HILL AND THE EMPLOY EE PANICKED. HE DID NOTHAVE HIS PARK BRAKE SET, AND THE LOADER WAS STILL IN GEAR. THIS IS WHY IT DIDN'T START IMMEDIATELY.

TRK DRIVER WAS HAULING COAL TO THE TIPPLE,& AS THE TRK LEFT THE PIT AREA & BEGAN TO TRAVEL UPGRADE, THE DRIVER ATTEMPTED TO STOP ON THE HILL. THE DRIVER THEN LOST CONTROL & THE TRUCK STARTED ROLLING BACKWARDS & HIT THE BERM ON THE SIDE OF THE ROAD. THE REAR TANDEMS HIT THE BERM CAUSING THE TRUCK TO ROLL OVER ON IT'S SIDE IN THE ROADWAY.
