THE EE WAS TRAVELING IN A MANTRIP WHEN IT DIPPED DOWN. THIS MOTION CAUSED THE EE'S KNEE TO STRIKE THE ROOF, RESULTING IN A LACERATION THAT REQUIRED STITCHES.

EE WAS OPERATING SHUTTLE CAR AND WAS RETURNING TO THE MINER. THE SHUTTLE CAR BRAKES FAILED AND EE STRUCK MINER WITH SHUTTLE CAR-EE EXPERIENCED PAIN IN BACK.
EMPLOYEE STATED THAT HIS BACK WAS SORE AND HURT AFTER HE WENT THROUGH A DIP IN THE ROAD AND HIS SEAT BOTTOMED OUT.
BODY WAS JOLTED WHEN DUMP TRUCK WAS BEING LOADEDWITH ROCK RESULTING IN STIFFNESS IN NECK.
EMPLOYEE STATED THAT HIS BACK HAD BEEN INJURED ON 4/18/00 WHILE SITTING IN HIS ROCK TRUCK BEING LOADED BY A 992C.
