EE was standing beside the forklift as they were loading the scoop bucket with a bag of bulk dust. He reached down to get the pallet and had his right foot there also as the forklift pulled up, when he did this, he ran over his right foot causing a contusion with the forklift.

Member had been cutting strings off of bottom hanger brackets when a belt splice caught his glove pulling his left arm into the belt. He has continued to work but did get a skin graft on 3-9-07 and is now unable to work because of the risk of infection to these areas.
No. 5 TRACKWAY 47 X-CUT. ROCK FLIPPED UP INTO OPERATOR DECK ON 12 TON MOTOR CAUSING A LACERATION AND FRACTURE TO RING FINGER ON LEFT HAND. ROCK MEASURED 6" X 12" X 2.5" THICK. THIS WAS HIS LAST SHIFT BEFORE HE STARTED HIS VACATION ON 7/3/04. HE RETURN TO WORK FROM VACATION ON 7/10/04 AND CAME BACK TO WORK.
EE WAS GOING TO LF #2 SWITCH ON THE TRACK OPERATING A MANTRIP WHEN SUPPLY MOTOR CAME INTO THE # 2 SWITCH THEY HIT FLATCAR HIT MANTRIP CAUSING EELT. WRIST TO BE THROWN ONTO THE SIDE OF MANTRIP.
WORKING BELTLINE, MUD BUILT UP ON GUARDS. THEY WENT UP TO CLEAN MUD OFF GUARDS, LOWERED GUARD USED HOE, MADICK, SHOVEL, AND HAMMER TO BREAK THE FROZEN MUD FROM GUARD. WHEN EE REACHED IN WITH THE HAMMER TO BREAK LOOSE MUD, THE ROLLER CAUGHTJACKET BETWEEN ROLLER AND BELT AND PULLED LEFT ARM THROUGH.
EMPLOYEE WAS OPERATING A MANTRIP ON ROCKHOUSE #1TRACK WHEN HIS LT ELBOW STRUCK A ROCK THAT HAD COME LOOSE FROM THE RIB CAUSING A BRUISE AND SWELLING TO HIS LT ELBOW AND WAS PLACED IN A SLI NG. THE ROCK HAS BEEN REMOVED FROM THE TRACK TRAVELWAY.
EE WAS OPERATING A SHUTTLE CAR WHEN HE RAN IN A POTHOLE IN THE HAULAGE ROAD. EE WAS BOUNCED INTOTHE CANOPY ON THE SHUTTLE CAR RESULTING IN A SPRAIN TO HIS NECK. EE WAS GIVEN A PRESCRIPTION FOR PAIN MEDICATION.
RIDING THE JOHNSON BATTERY POWERED RIDE & STARTED OVER INTO THE BELT ENTRY & NO. 5 BELT, BREAK 26 WHEN HE CAUGHT THE COAL RIB CAUSING IT TO COME OFF STRIKING HIM ON RIGHT HAND & LEFT EAR.
EE WAS A PASSENGER ON A MANTRIP THAT COLLIDED WITH A MOTOR. HE WAS THROWN INTO THE FRAME OF THE MANTRIP. HE SUSTAINED A LACERATION TO THE FOREHEAD THAT REQUIRED STITCHES.
EE WAS RIDING IN THE BUCKET OF A SCOOP. WHEN THE SCOOP ENTERED A NARROW ENTRY, EE SLIPPED AND HIS LEG WAS CAUGHT BETWEEN THE BUCKET OF THE SCOOP AND THE RIB.
EE WAS OPERATING A SCOOP, TRAVELING OUTSIDE OT PICK UP SUPPLIES. APPROX. 50' FROM THE SURFACE THE SCOOP LOST HYDRAULIC POWER AND BRAKES. EE WAS UNABLE TO CONTROL THE SCOOP AND RAN INTO A PREFA B WALL. EE WAS STRUCK BY A TIMBER. HE SUSTAINED A FRACTURED PERLVIS AND DISLOCATED LEG.
EE WAS OPERATING A MANTRIP. A TRACK TIE HAD BEEN PLACED TO CLOSE TO THE TRACK. WHEN EE PASSED BY THE TIE THE MANTRIP CAUGHT THE TIE AND THREW IT IN THE OPERATOR COMPARTMENT. EE SUSTAINED A CRA CKED VERTEBRAE IN THE NECK C6 & LOWER BACK L2.
EE WAS OPERATING A SHUTTLE CAR WHEN THE TIRE ON THE SHUTTLE CAR BLEW OUT. EE WAS JOSTLED ABOUT TIN THE OPERATOR'S COMPARTMENT. HE SUSTAINED MUSCLE STRAIN TO THE NECK AND BACK.
EE WAS OPERATING THE #4 SHUTTLE CAR. HE WAS PULLING OFF THE FEEDER, ONE BREAK INBY THE POWER CENTER AT SPAD #8316. EE HIT A POTHOLE CAUSING HIM TO BOUNCE UP OFF HIS SEAT. WHEN HE CAME BACK DOW N HIS TAILBONE STRUCK THE FRAME OF THE SEAT. EE WAS DIAGNOSED WITH A CONTUSION TO THE TAILBONE.
EE WAS HELPING LOAD BELT STRUCTURE. HIS FOOT WAS SMASHED BETWEEN TWO PIECES OF RAIL WHEN SCOOP ACCIDENTALLY RAN INTO ONE OF THE PIECES OF RAIL.
