Employee lost the balance of the doors on the forks of the skid steers and got hand caught between the door and headache rack on skid steer causing injury.

While Climbing off the machine, Employee Placed EE's right hand on the engine cover Resulting in EE's fingers contacting the cooling fan.

Employee was operating a fuel truck underground and accidentally hit a rib causing EE's knee to hit against the dash in the cab causing injury.

While conveying a load of rock from the shuttle car into the feeder breaker a rock rolled over the side rail of the conveyor from the shuttle car's throat hitting EE's right hand.

EE could see a cable could be hit by the loader bucket. EE stepped alongside the bucket to quickly move cable. The bucket slid to EE's side and struck EE's ankle. EE received a fractured ankle.

EE went to adjust the sampler brake. EE disengaged the sampler brake and then lifted the cover so EE could pull on the sampler drive belt to get it into position. EE tried to move the sampler forward the sampler did not want to move so, EE moved it in reverse. The sampler must have moved far enough to trigger the position sensor and the system turned on catching EE's right arm.

EE was using a rake to clean up the accumulations that were under the North Mains #1 Belt, the belt was approximately a foot and half off the ground. As EE put the rake under the belt the rake handle came into contact with the belt which then pushed the rake and EE's left hand into a bottom roller causing a laceration on EE's left ring finger

EE was unloading belt supplies from BB-203 at XC-7 in 6 North while unloading the supplies another EE was in BB-219 moving the truck out of the haulage way, while backing up BB-219 the rear fender caught EE's self-rescuer causing EE to get twisted between the two trucks.

Employee was hanging life line and got ankle caught between the rib and a mine vehicle causing injury. The truck backing up in motion was a contributing factor to the accident.

EE was operating controls on material hauler and placed other hand in pinch point on moving part.

While a mechanic was trouble shooting a Utility Vehicle, they accidently pulled forward and the trailer wheel smashed miners calf.

EE reached into engine compartment with motor running on skid steer to tighten nut on fuel line. Left hand was holding wrench and right hand was resting on fan shroud, when they leaned in right hand little finger contacted running fan, causing laceration.

After rock dusting activities, employee was placing the hose into the bucket of the scoop. At this point, the bucket was inadvertently lowered onto the injured's right foot. The injured was taken to the hospital where x-rays revealed no broken bones. EE was prescribed days off work.

Employee was firebossing and adjusting the air on section and a shuttle car bumped into the employee and the employee fell against the rib and felt pain in back and legs.

Loader operator dropped bucket on Employee's boot while Employee was loading a vent tube in bucket.

EE was positioned in between a parked utility vehicle (UV) and a trailer that was attached to the UV. The UV was running and lunged backward when a second employee adjusted the idle of the UV. EE was pinched between the rear bumper of the UV and the tongue of the trailer. Fracture of right leg.

Attempted to move a shuttle car. He thought the door was latched tight and started to move forward. The door swung open and he reached out and caught the door. As he was shutting the door he simultaneously let off on the tram lever, the inertia from this action caused the door to swing shut faster than anticipated subsequently catching his finger between the frame and door.

12" Drisco pipe was being pulled by utility vehicle. Employee was standing next to parked Dodge truck. As pipe cleared Dodge truck it quickly slid over and struck EE on lower left leg.

Standing on tire of Gehl bobcat forklift, resting hand on arm of bucket. When operator raised forks, his hand was pinched between the arm of the forklift and the cab lacerating his finger behind the nail of the right middle finger.

Tailgate drive cover came off. Cover came down coveyor to shearer. Cover jammed into shearer gear box and sprocket. Gear box bolts sheared sending bolts flying. One bolt hit back of employee's left hand against shield leg ram.

While clearing a setting on shield with left hand, EE placed right hand on inside of bretbe trough. At this moment, a large piece of coal coming down LW conveyor, caught right hand and caused injury.

He was standing at the feeder breaker talking to the shuttle car operator. He placed his right hand on the deck of the feeder while the car was unloading. A chunk of coal fell on his right hand from the conveyor while dumping.

Shuttle car operator was operating the shuttle car when his right rear tire dropped into a hole. He struck his head on the cab of the shuttle car straining his neck. He returned to work for one week, however, on 11-08-04 he reported off work due to a neck strain which resulted from the injury.

EMPLOYEE WAS DRIVING AN EIMCO M3 SHIELD HAULER UP THE FAULT WHEN THE RIGHT REAR TIRE BEHIND THE DRIVER'S COMPARTMENT BLEW, CAUSING THE MACHINE TO DROP TO THE GROUND AND JARRING THE EMPLOYEE'S LOWER BACK. OPERATOR'S SEAT WAS AN AIR RIDE SEAT, WHICH APPEARED TO BE WORKING. EMPLOYEE WAS WEARING HIS SEAT BELT. EMPLOYEE SUSTAINED INJURY TO LOWER BACK. HE HAS PRE-EXISTING BACK CONDITION.

WHILE OPERATING A LOW PROFILE SCOOP EE RAN OVER A ROCK HE HAD BEEN CARRYING AND HIT HIS HEAD ON CANOPY.

EE WAS IN THE BOOM TRUCK BACKING UP WITH THE MOTOR TO BE UNLOADED WHILE SOMEONE WAS GUIDING HIM. THE BOOM TRUCK OPERATOR STOPPED WHEN THE CAB OF THE BOOM TRUCK CAME TO THE HANGING DRISCO PIPE. SPOTTER TOLD OPERATOR TO CONTINUE BACKING UP & TO BRING THE PIPE OVER THE CAB TO BRING THE PUMP CLOSER TO THE STOPPING. OPERATOR WAS LEANING WITH HIS HAND ON THE CAB TOP & HAND WAS CAUGHT.

EMPLOYEE WAS ATTEMPTING TO MEASURE DISTANCE FROM TAIL PULLEY EDGE TO BEARING SUPPORT. BELT WAS RUNNING. EE STUCK STEEL TAPE MEASURE THROUGH GUARDING, WAS HOLDING TAPE MEASURE CASING WITH LEFT HAND AND FEEDING TAPE WITH RIGHT HAND. END OF TAPE TOUCHED EDGE OF TAIL PULEY AND TAPE WAS IMMEDIATELY SUCKED BETWEEN PULLEY AND BELTING. TAPE WAS PULLED THROUGH RIGHT HAND CUTTING ALL 4 FINGE

EE WAS PREPARING TO LOAD A BUNDLE OF CABLE BOLTS INTO A SHUTTLE CAR. THE SHUTTLE CAR WAS SITUATED CLOSE TO THE COAL RIB. AS HE GOT OFF THE MACHINE TO REACH FOR A CHAIN. THE UNIT ARTICULATED TO WARD THE RIB, PINCHING EE BETWEEN THE MACHINE AND THE RIB HE RECEIVED A FRACTURED PELVIS.

EE WAS TRIMMING FLOOR AND CLEANING UP THE FACE OF #2 ENTRY. HE WAS OPERATING THE JOY MINER WITH A REMOTE CONTROL AND STANDING ON THE LEFT SIDE OF THE ENTRY. HE HAD STARTED THE MINER TOWARD THE FACE ON AN ANGLE TO LEVEL THE FLOOR. THE SHUTTLE CAR FOLLOWED THE MINER INTO THE FACE ON ABOUT THE SAME ANGLE. THE SHUTTLE CAR WAS STANDARD TYPE WITH THE CAB ON THE RIGHT SIDE OF THE ENTRY.

EE HAD A HOLD OF BELT RAIL, HE WAS LOWERING IT TO TRY AND DROP FEW TOP ROLLERS. WHEN ROLLERS BEGAN TO MOVE ONE SLID DOWN AND SMASHED THE LEFT MIDDLE FINGER BETWEEN THE ROLLER AND THE RAIL. MIN OR INJURY CHECKED AT MEDICAL CLINIC.

EE WAS MOVING VENTILATION TUBES FROM WEST MAINS TO 14 LEFT. ON FOURTH TRIP LOADING TUBES USING RAM CAR, EE HAD ALREADY LOADED SIX TUBE, TALKED ABOUT LOADING ONE MORE. WALKED AROUND CAB SIDE, A ND EE PROCEEDED AROUND FRONT OF RAM CAR INTO XCUT. OPERATOR SAW THAT EE WAS CLEAR AND PROCEEDED TO PULL FORWARD, EE'S RIGHT FOOT WAS RUN OVER BY RAM CAR, DISLOCATING RIGHT ANKLE.

EE WAS WORKING ON A FEEDER BREAKER HE DID HAVE IAT LOCKED OUT BUT SOME OTHER PARTY LOCKED THE BELT OUT & WHEN THEY FINISHED & REMOVED THEIR LOCK, THE BELT STARTED UP & RUN EE THROUGH THE DISCH ARGE END OF THE FEEDER, CAUSING A FRACTURE OF THE PELVIS & THE LOWER BACK.

MEN WERE TRAINING THE CONVEYOR BELT AT THE TAIL PIECE LOCATION WHEN A BAD SPLICE CAME BY AND CAUGHT A PIECE OF WOOD OFF THE BOTTOM AND FLIPPED IT UP AND STRUCK THE MAN ON THE LEFT SIDE OF THE FACE CAUSING A SMALL WOUND THAT REQUIRED A COUPLE OF STITCHES OR SUTURING.

EE WAS HELPING ANOTHER EEREMOVE A COOKIE SHEET (PLATFORM) FROM A 936 EIMCO FORKLIFT TYPE MACHINE. THE COOKIE SHEET WAS BOUND ONTO THE FORKS AND WOULD NOT COME OFF. A CHAIN HAD BEEN HOOKED THRO UGH THE EYES OF THE COOKIE SHEET AND AROUND A CEMENT FILLED PIPE FOR ANCHORAGE. ON THE FOURTH TY AND ADJUSTMENTS AFTER EACH ATTEMPT, THE CHAIN BROKE STIKING EE ON THE INSIDE OF HIS LEFT ANKLE

BACKING UP TO A TRAILER CAUGHT ARM BETWEEN WINDOW OF TRUCK & RIB, CAUSING CHIPPED BONE & BRUISE TO LEFT ELBOW
