A contractor who was walking across the parking lot from EE's parked car to the mine's training center when EE was struck by the car of an employee. The employee was arriving at work to begin EE's shift and searching for a parking space at the time of the accident.

While operating a scoop, the injured person needed to adjust self in the seat. EE grabbed the canopy with the right hand and accidentally caught the palm of their hand between the rib and the canopy. EE was then transported to the hospital for treatment, which included surgery to set the bone in EE's pinky finger and repair the soft tissue damage.

While pulling machinery, chain that scoop winch rope was attached to broke, allowing winch rope to fly back striking the employee in the right side of the head.

While assisting a forklift operator removing block supplies from the supply car, employee placed their hand between 2 lifts of blocks. The blocks being lifted by the forklift shifted on the forks smashing EE's index and middle fingers on the left hand between the blocks.

A shuttle car operator was adjusting ventilation controls to allow equipment to run through. During this process a scoop came from the opposite direction catching the EE between the scoop and the front of the car. The EE was transported to Ruby Memorial Hospital where EE was treated for a compound fracture to the left femur and laceration of the left leg.

EE was watching the 2 inch water line hose as it was being pulled by a scoop, when the coupler on the line caught on a rib corner. The line stretched tight and the coupler came apart allowing the end of the water line to strike the injured on the right leg just below the knee resulting in a fracture to both the fibula and tibia bones.

While riding as a passenger in a rubber tired mantrip, the mantrip hit a bump in the road jarring the EE resulting in pain to the middle/upper back. This incident was reported on 6/16/2015 and the EE did not begin missing work until 7/6/2015.

Employee was standing in front of the scoop on the bucket end facing the scoop loading supplies when a supply trailer that was being pushed backwards by a diesel tractor caught both legs above the ankles between the scoop bucket (which was on the ground) and the trailer causing a fracture to his right ankle and dislocation to his left ankle.

Individual was struck by a piece of rock that slid off the loaded shuttle car and into the canopy area striking him on the right hand and causing a laceration the required 6 sutures.

Individual was unloading a trailer when the scoop he was operating slid off the trailer causing him to strike his head against the canopy receiving a laceration to the head.

While traveling down the #5 entry toward #2 section, the rubber tired mantrip EE was operating, hit a rut and caused the steering wheel knob to strike EE's right arm.

The employee was operating a battery mantrip when he leaned up hitting his head on an overcast.

The injured, along with 2 others, were installing a mechanical splice in a piece of mining belt when a scoop struck them.

While scooping the feeder, the rails on the operators side of the batteries slide forward into the cage around the operator. When this occurred, the operator had his left hand holding onto the cage, which exposed the fingers to the outside of the cage. As the rails struck the cage, the end of one amputated the tip of his left middle finger

Employee was walking in an entry where a shuttle car was parked. As he began to cross the shuttle car cable by beginning to step on the cable, the shuttle car operator simultaneously started the pump motor on the shuttle car, causing the cable to quickly come upward from the mine floor. The cable struck the bottom of the injured's foot.

The operator started to tram forward and at the same time he steered the scoop to the left, which caused the injured's lower legs to be caught between the batteries on the scoop and the batteries on the mine floor. This resulted in contusions to the calf area of both legs.

Employee was operating a scoop, scooping an idle entry when the front wheels of the scoop suddenly went down in a hole causing him to strike his head against the top of the scoop canopy which was padded causing him to have pain in the thoractic area of the back.

Guide stops for bucket unlevel, letting crosshead drift one sided. Torpedo hangs up inside crosshead, causing slack rope. Bucket dropped 12'-15', causing fractured ankle. EE was riding in bucket.

While tramming shuttlecar, injured hit a hole in the bottom causing him to bounce out of the seat and strike his head on bottom of canopy, hurting neck.

Two mantrips left the B-Mains section about 30seconds apart. The outby mantrip made an unplanned and uncommunicated stop after it went around a turn and through a switch and struck from behind by the inby mantrip.

EMPLOYEE WAS SITTING IN A STOPPED MANTRIP WHEN ANOTHER MANTRIP FAILED TO STOP, SLIDING INTO THE STOPPED TRIP. THIS CAUSED THE EMPLOYEE TO STRIKE HIS HEAD AGAINST THE CANOPY OF THE MANTRIP, CAUSING PAIN IN HIS UPPER BACK & NECK AREA.

Employee was attempting to "free" a shuttle car cable that had caught on the section power center when tension on shuttle car took up slack on cable which in turn struck him in the face causing a cut below lower lip, scraped bridge of nose and forehead.

Blocking the feeder with cribs to pull on tailpiece, clearance was close. Feeder slid off of cribs; caught left hand between feeder and coal rib. Accident occurred 1 break inby spad #3622 in the #5 entry.

WHILE OPERATING A SCOOP ON MMU-008 IN CROSSCUT 4-5, EE'S SCOOP RAN OVER A ROCK (ROCK APPROX. 1 1/2' X 1 1/2' X 10"), CAUSING HIS HEAD TO HIT THE CANOPY, RESULTING IN NECK PAIN AND NAUSEA.

EE dumped a load of coal in the section feeder. trammed shuttle car away from dumping point when shuttle car hit hole in mine floor causing ee to strike his head against canopy. this caused pain in neck, upper shoulder area.

EE STATED WHILE OPERATION A SCOOP, HIT A HOLE JARRING THE SCOOP AND TRAVELWAY. EE BECAME A LTA ON 10/29/2004.

WHILE A SHUTTLE CAR WAS HUNG UP, THE INJURED WAS OPERATING A SCOOP TO PUSH (SC)OUT. THE SCOOP WAS BOWED UP IN THE AIR & FELL TO THE BOTTOM HARD WHEN THE (SC)CAME LOOSE. THE INJURED FELT NUMBNESS IN HIS ARMS & HANDS FOR AWHILE, WITH PAIN IN HIS NECK & BACK. THE INJURED HAD TO WAIT UNTIL 9/4/04 FOR AN MRI & UNTIL 9/8/04 FOR A RESULT.

Injured had placed a crib under the deck of #1 21SC and in the bucket of the scoop in order to lift the SC to replace the wheel unit. The injured had positioned himself to the side of the scoop bucket. At the scoop lifted the SC, the crib "popped" out, striking the injured at the left eye.

Injured was removing a bag of tools from the back of a three wheeler personnel carrier during shift change when an individual from the oncoming shift boarded the machine and trammed it in reverse. The machine ran over the injured's foot, knocked him down, and ran onto his leg. The operator thought everyone was clear of the three wheeler

INJURED WAS RIDING IN OUTBY END OF 38 JEEN WHEN HE STRUCK HIS HEAR ON A X BAR AT SOUTH MAINS 14 BLK ONT HE TRACK.

INJURED WAS MOVING A 4 WHEELER PERSONNEL CARRIER WHEN THE LEFT FRONT WHEEL FELL OFF CAUSING THE STEERING WHEEL TO SPIN STRIKING HIS RIGHT WRIST.

THE INJURED WAS STANDING ALONG SIDE OF A SHUTTLE CAR, HE THEN GAVE THE CAR OPERATOR THE O.K. TO TRAM THE SHUTTLE CAR. THE INBY END OF THE CA5R SLID INTO THE INJURED STRIKING HIM ON THE LEFT LE G AND LEFT HAND. RESULTED IN A BROKEN FEMUR, ANKLE AND HAND ALL ONT HE LEFT SIDE OF HIS BODY.

INJURED WAS TRAVELING IN BY THE RAIL WHEN HE THOUGHT HE HEARD SOMETHING FALL FROM THE JEEP, WITHOUT STOPPING THE JEEP, HE TURNED TO LOOK. WHEN HE TURNED BACK TO LOOK WHERE HE WAS GOING HE "ROOFED OUT".

THE INJURED WAS TRAMMING A BATTERY POWERED SCOOPIN THE NO. 2 ENTRY OF THE E MAINS NEAR SPAD 8467HE WAS TRAMMING FROM THE SECTION TO THE SUPPLY AREA WHEN HE RANOVER AN 8" SOLID CEMENT BLOCK BOUNCING HIM IN THE OPERATOR'S SEAT RESULTING INPAIN IN HIS LOWER BACK. ORIGINAL ACCIDENT WAS 4/27/03 IT DID NOT BECOME REPORTABLE UNTIL 5/2/03

THE INJURED WAS HAULING ROOF BOLTING SUPPLIES IN THE BUCKET OF THE SCOOP TO THE BOLT MACHINE WHEN ONE OF THE FOUR FLY CHECK BOARDS 1" THICK X 6" WIDE X 16' LONG BECAME PINNED BETWEEN THE COAL RIB AND THE POST OF THE OPERATORS CANOPY, THE BOARD SLID OFF THE CANOPY POST AND STRUCK HIM ON THE LEFT EAR CAUSING A LACERATION WHICH REQUIRED 9 SUTURES.

THE INJURED WAS RIDING SEATED IN AN OPEN TOP MANTRIP, WHEN THE MANTRIP STOPPED AT THE IN BY AIRLOCK TRACK DOOR HE WAS ATTEMPTING TO GET UP AND THEN TURN AROUND ONTO HIS KNEES, AS THE MANTRIP TRAMMED FORWARD HE STRUCK HIS HEAD AGAINST THE MINE ROOF CAUSING PAIN IN HIS NECK.

PROGRESSIVE PAIN IN RT. SIDE THROUGH USE. AT END OF WORK WEEK ON 8-1-02 EE WENT TO DR COMPLAINING OF PAIN IN RT SIDE, WHEN HE RETURNED TO WORK ON 8-5-02 HE DISCUSSED HIS PAIN WITH THE MINE SUP ERINTENDENT SAID HE LIKE TO CONTINUE WORKING ANDFELT MAYBE HE COULD WORK THE PAIN OUT. HE BEGAN MISSING WORK ON 8-19-02.

THE INJURED WAS OPERATING THE TRACK MOUNTED BATTERY MANTRIP INTO THE MINE. AT 28 BLOCK ON THE MAINS TRACK THE CANOPY OF THE MANTRIP STRUCK THEMINE ROOF CAUSING A ROCK TO FALL ONTO THE INJURE DS LEFT LEG AND FOOT. THE ROCK MEASURED 10"X 12"X 4" THICK. RESULTED IN BRUISES TO THE LEFT KNEE AND LEFT FOOT.

THE INJURED WAS STANDING ALONGSIDE OF AN 18' LONG X 6" "H" BEAM AS IT WAS BEING DRAGGED IN THE TRACK ENTRY BY A SCOOP HOOKED WITH A CHAIN, WHEN THE END OF THE BEAM CAME IN CONTACT WITH A FISH PLATE, CAUSING THE OTHER END OF THE BEAM TO RAISE UP FROM THE MINE FLOOR, SWING SIDEWAYS AND STRIKE THE INJURED ON THE RIGHT SIDE OF HIS HEAD, RESULTING IN A LACERATION & KNOCKED HIM UNCONSCIO

INJURED WAS UNLOADING COAL ON A STOCKPILE. THE AREA WAS NOT PERFECTLY LEVEL AND AS THE TAILOR WAS BEING RAISED, THE MATERIAL SHIFTED TO THE LOW SIDE CAUSING THE TRUCK TO UPSET. THE OPERATOR WA S SUSPENDED IN THE SET BELT NEEDING ASSISTANCE TO GET FREE.

THE EMPLOYEE STATED THAT HE WAS WALKING BESIDE SCOOP UNLOADING ROCK DUST WHEN HIS RIGHT FOOT GOT CAUGHT UNDERNEATH THE SCOOP BUCKET.

INDIVIDUAL WAS RIDING ON THE IN BY END OF MANTRIP WHERE IT JUMPED, AT THE #3 SPUR ON 2ND TRACK. HIS LEGS WERE CROSSED AT THE TIME AND WHEN THE MANTRIP FELL FROM THE RAIL, IT BOUNCED HIM UP IN THE AIR CAUSING HIS RIGHT HAND TO STRIKE THE TOPEDGE OF THE SIDE PANEL.

THE INDIVIDUAL WAS ASSISTING A WORKMAN IN INSTALLING A 48" CRADLE ROLLER ON THE BELT ROPES OF THE MAIN WEST #2 BELT, WHEN HE FELT PAIN IN HIS RT SHOULDER JOINT. THE INJURED DID NOT BEGIN MISSI NG WORK UNTIL 1/20/99.

WHILE SITTING DOWN IN THE MANTRIP, THE INJURED STRUCK HSI RIGHT ELBOW ON THE BACK OF TH METAL SEAT.

THE INJURED HAD LOCKED THE BRAKES ON THE MOTOR, DISMOUNTED AND WENT TO UNCOUPLE THE MOTOR FROM THE SUPPLY CARS WHEN HE SLIPPED. WHEN HE CAUGHT HIMSELF FROM THE SLIP HE HAD PLACED HIS LEFT FOOT AGAINST THE RT INBY WHEEL OF THE TRANSPORT CAR AT THAT TIME THE OUTBY MOTORMAN THOUGHT THAT HE WAS FLAGGING HIM TO DROP THE CARS DOWN, WHICH IN TURN RAN OVER HIS FOOT

THE EE TWISTED HIS KNEE WHILE TURNING AROUND IN A SHUTTLE CAR DECK. THE INJURY WAS A STRAIN TO THE LEFT KNEE.

THE INDIVIDUAL HAD HAND ON BELT SCRAPPER FRAME WHEN BELT SPIKE PASSED THROUGH IT CAUSING IT TO JUMP. WHICH RESULTED IN A LACERATION TO HIS FINGER HE WAS WEARING GLOVES AT THE TIME OF ACCIDENT

THE INJ WAS STANDING ALONG SIDE THE A MAINS NO.3BELT CHECKING SPLICES IN THE BELT WHEN THE JACK'PIPES USED TO ANCHOR THE TAILPIECE LET LOOSE AND ALLOWED THE TAILPIECE TO KICK SIDEWAYS STRIKING THE INJ ON THE RIGHT KNEE RESULTING IN A LACER ATION WHICH REQUIRED 6 STITCHES.
