Employee felt back pain after hitting bump in a light duty vehicle. After emergency room visit employee was given restrictions and missed the next shift.

Employee began to feel discomfort in neck and upper back while operating a haul truck. EE was taken to the clinic for evaluation and given a full release with no restrictions or follow up appointment needed. EE did not return to their next shift upon their own decision.

Employee began to feel back pain while driving a haul truck. IE was taken to ER for evaluation and did not return to the next shift.

Task hazards not considered. Team member had some bruising to leg and ankle.

While operating haul truck employee began to feel back pain. Upon an orthopedic visit on 5-17-2024, employee was restricted and prescribed physical therapy.

IE was operating haul truck and began to complain of back discomfort after hitting a bump. The employee was seen again at the ORTHO on 01-03-2024 and was restricted no work and prescribed PT for work hardening.

Mine was struck by a Tornado. Tornado flipped our vehicle on its side , while our driver was in it. Driver was taken to the hospital , treated and released. Driver was out of service due to DOT policy. Driver allegedly claimed had PTSD from the event which placed EE out of service.

Employee experienced back discomfort and did not make their next shift.

IE hit a bump while operating a haul truck. EE began to feel discomfort in neck. EE was taken to the ER and a referral and MRI reading on 4-30-2019 revealed a neck sprain. EE was prescribed PT for 7-10 days.

IE was driving a loaded haul truck when ee hit a pot hole and felt tightness in neck. IE was seen by an ortho and an MRI was ordered. On 5/9/19 the MRI was read and it was determined the employee had a bulging disk that would require surgery.

The IE was removing mud from the transmission linkage area under a F-250 pickup when a second employee, attempting to test the motion inadvertently put it into reverse and the front left tire made contact with the IE's chest. The IE was diagnosed with simple thoracic spinous process fractures.

The IE has a history of chronic back disease which was aggravated from an incident involving a lube truck. A bone scan confirmed a degenerative illness on 08/10/2015.

The IE traveled over a rough area and began to experience pain in the low back and neck. The IE was referred and seen by an orthopedic doctor who diagnosed a whiplash type injury.

While operating a haul truck, employee ran through a soft spot in the road and began to experience upper back pain. After an ER visit, follow up Dr. visits and an MRI, it was determined that the employee has a pre-existing degenerative disc condition in the lower back.

The body of the haul truck lowered quickly causing the operator to experience back pain. The employee was seen by an orthopedic physician and was not immediately released to return to work.

The injured employee was jarred when the fuel truck he was operating hit a pot hole. On 3/27/12 an Orthopedic specialist diagnosed him with a tear in a lumbar disc and would not allow him to return to work.

The employee was jarred while operating a haul truck, upon evaluation he was returned to work. Several days later he began to experience pain in his neck and was diagnosed with pre-existing degenerative stenosis on 12/22/11. He received a restricted duty release making this illness reportable.

While operating a haul truck, employee clinched her teeth tightly cracking a crowned tooth. The dentist stated the employee suffers bone degeneration. Based on this diagnosis this is being reported as an illness.

Employee was driving a one ton pickup to the shop on a coal haul road. The pickup collided with a 240 ton haul truck. The employee suffered a sprained right ankle, fractured right clavicle and a laceration on right cheek requiring 9 stitches.

Employee was operating a haul truck and suffered a lower back muscle strain while dumping a load of overburden. She received a restricted duty work release.

A shovel oiler driving a Ford F350 pickup turned west to enter a haul truck staging area. He was momentarily unable to see due to the setting sun directly in his eyes and struck an employee walking from a parked haul truck to the lunch trailer. The injured employee suffered a broken ankle.

While emptying trash container was pushed loose striking operator causing rib fractures.

Employee was stopping the train. She was moving from the engineer's seat to the center of the cab when she bumped her shoulder on the conductor's desk.

Ready mix truck was delivering concrete on job site at mine & when finished, our employee was washing out truck & got his hand pinched between the pin & hopper of the mixer truck.

When backing under the shovel, the truck came to an abrupt stop, jarring his neck and back. The employee was diagnosed with a back and neck strain on 1/31/07.

The employee was leveling a man basket on a boom truck; the hydraulics suddenly released that jarred the man basket causing the employee to fall against the handrail resulting in 2 fractured ribs.

Loaded truck full of topsoil was traveling down a recently watered road. Truck lost traction, jack-knifed and tractor overturned, resulting in two compression fractures in back and one fractured rib.

A form 7000.1 was faxed along with a letter to MSHA on 07/27/06 concerning this incident. The Denver office requested we also submit the form electronically; therefore this submission is being made. Employee had completed his shift & left the mine site traveling on his personal motorcycle. About 1 mile past the mine gate he struck an antelope resulting in a fractured wrist.

This 7000.1 form is being submitted to abate a violation alleged in citation number 7610111 issued on 07/18/2006. The employee was off shift and had left the mine site; he was traveling home on his personal motorcycle. He lost control of the motorcycle two miles outside of the security gate.

EMPLOYEE WAS WALKING BEHIND PARKED SKIDSTEER LOADER ON PATTERN WHEN HE WAS PINNED BY A BACKING 1.5 TON DEWATERING TRUCK. EMPLOYEE WAS DIAGNOSED WITH A PELVIS FRACTURE AND CRUSHED HIP SOCKET.

Employee was operating a scraper, ran over a rock and began experiencing back pain. The employee was seen by an orthopedic doctor on 02/02/06 and was diagnosed with extensive lumbar degenerative disease with a herniation of the disc. The incident occurred on 01/25/06; we received the diagnosis on 02/02/06.

THE CONTRIBUTING CONDITIONS OF THIS ACCIDENT WERE: (1) AN EMPTY MACHINE DID NOT YIELD RIGHT OF WAY TO A LOADED MACHINE, (2) THE EMPTY MACHINE WAS RUNNING TOO FAST FOR THE CONDITIONS.

Loader with a cable spool struck the dump truck on drivers side.

DUMPING A LOAD, THE CAB WENT UP IN THE AIR WHEN THE LOAD WAS DUMPED, THE CAB THEN CAME DOWN HARD & JARRED HIS BACK.

As employee was driving a service truck along a haul road the truck encountered a large bump that caused him to bounce upwards out of his seat and strike his head on the roof of the cab. As a result of striking his head and falling back into the seat he began to experience pain in his lower and middle back. At the time of the incident he was wearing his seat belt.

Off duty employees traveling in private vehicle collided with another vehicle on mine access road. Two employees sustained injuries including multiple contusions,abrasions, brusing and fractures to leg.

EMPLOYEE HIT HIS HEAD ON CAB WINDOW AND STRUCK SOMETHING SHARP.

EMPLOYEE WAS RIDING IN A CAT 773B BEING TRAINED AND BANGED HER HEAD ON THE REAR WINDOW.

While riding in a pickup, hit rough terrain the pickup was jarred causing neck pain to the employee. The employee was prescribed physical therapy.

FINGER CUT BY FAN WHEN STUCK IN HEATER VENT REQUIRING SUTURES.

OPERATOR DROVE SCRAPER OFF AN 18" TOPSOIL EDGE AFTER DUMPING HIS LOAD. THE MOVEMENT CAUSED THE SCRAPER TO SHIFT AND THE OPERATOR BUMPED HIS ELBOW ON THE DOOR. THE OPERATOR'S ELBOW MAY HAVE A S UPPORT FRACTURE BUT IT WAS UNCLEAR AT THIS TIME.

THE TECHNICIAN WAS A PASSENGER IN A VAN THAT WAS DRIVEN OFF A HAUL ROAD, OVER A BERM AND DOWN AN EMBANKMENT. SHE WAS DIAGNOSED WITH MULTIPLE CONTUSIONS.

THE MANPOWER EE WAS A PASSENGER IN A VAN THAT WAS DRIVEN OFF A HAUL ROAD, OVER A BERM AND DOWN AN ENBANKMENT. DIAGNOSIS WAS BACK AND ARM PAIN.

THE TECHNICIAN DROVE A VAN OFF A HAUL ROAD, OVER A BERM AND DOWN AN EMBANKMENT. HE WAS DIAGNOSED WITH A BACK SPRAIN.

WHILE TROUBLE SHOOTING A SLIPPING SAMPLE CONVEYOR BELT, THE EMPLOYEE REACHED INTO THE HEAD PULLEY AREA AND CAUGHT HIS RIGHT ARM RESULTING IN INJURIES INCLUDING A FRACTURED LITTLE FINGER, FRACT URED ULNA, CHIPPED ELBOW, AND LACERATED BICEP.

SPREADER BAR SLIPPED FROM FORK LIFT FORK AND FELL APPROX 30", HITTING RIGHT LOWER LEG. EMPLOYEE HAD JUST UNLOADED A BALE OF INSULATION AND TURNED TO SET IT DOWN. WAS HIT BY SPREADER BAR WHEN B ACK WAS TO FORK LIFT CAUSING FRACTURE TO RIGHT LOWER LEG.

WHILE PLACING A STEEL WEAR PLATE ON A FLATBED TRUCK WITH A FORKLIFT, A MAINTENANCE EMPLOYEE'S HAND WAS PINCH BETWEEN THE WEAR PLATE AND THE SMALL SIDE RAILING ON THE FLATBED TRUCK. HE WAS WEAR ING WELDING GLOVES. HE LACERATED TWO FINGERS ON HIS LEFT HAND AND RECEIVED 1 SUTURE IN HIS MIDDLE FINGER AND 4 SUTURES IN HIS RING FINGER.

FOR UNKNOWN REASONS, TECHNICIAN'S RIGHT ARM WAS CAUGHT IN CONVEYOR BELT RESULTING IN THIRD DEGREE FRICTION BURNS AND A CRUSHING INJURY TO HIS RIGHT FOREARM. HOWEVER, HE DID NOT SUSTAIN ANY BRO KEN/FRACTURED BONES.
