EE stated he was tramming Eimco scoop and hit a bump or a rock in road. This caused him to slam his head into the canopy. He felt instant pain in neck and left shoulder. He did not go to the doctor until 1/30/12.

Mostrando los 13Unit 1 was traveling W on old trace road. He was approaching a paving crew which was paving the W bound lane. A flagger waved him around into E bound lane to go around the paving crew. As he was going around, EE's left side wheels dropped off the E bound shoulder. Due to the steep grade, this caused him to overturn onto its L side, coming to rest on the E bound shoulder.
Employee was operating a shuttle car. While going to feeder his tire struck a hole in the roadway causing him to be bounced up. He landed crooked, injuring his lower back.

EE WAS SITTING ON A ROOF BIT BUCKET WHILE RIDING ON THE PERSONNEL CARRIER WHEN THE OPERATOR HIT A BUMP IN THE ROADWAY CAUSING THE INJURED TO LOOSE HIS BALANCE. HE FELL FORWARD STRIKING HIS HAN D ON THE PERSONNEL CARRIER'S FOOT REST.
EE WAS TRAMMING ROADWAY WHILE OPERATING SCOOP WHEN HE GOT FOREIGN MATERIAL IN HIS RIGHT EYE.

MANTRIP WAS HEAD ON BATTERY POWER. OTHER WAS ATTEMPTING TO PUSH MANTRIP OUTSIDE WITH RAM CAR. WHEN MANTRIP RAISED OFF GROUND LEVEL AND DROPPED BACK DOWN. THE IMPACT HURT EE S BACK AREA WHILE S ETTING IN THE OPERATOR SEAT.
MECHANICAL FAILURE OF TRAM SPOOL ON VALVE BANK. EE WAS TRAMMING FEEDER. HE LET OFF TRAM LEVERS, TRAM SPOOL WOULD NOT DISENGAGE. EE WAS TASK TRAINED OPN FEEDER BEFORE HE BEGAN MOVING FEEDER AND IS EXPERIENCED AT THIS JOB.
