A millwright was moving parts with a knuckle boom when the controller was bumped resulting in an injury to the finger that required stitches.

Employee slid out of seat on bus ride out to EE's equipment. When EE slid out of EE's seat EE hit EE's left side (ribs) on a lunchbox. Positive X-ray showed fractured ribs. Was released to work and returned EE's next scheduled shift.

An employee reported that EE's back was sore from a shovel dropping a bucket of material in EE's haul truck. EE continued working for two days and then went on days off. EE did not report to work on 7/26/2024 and began missing time.

An employee with a history of back injuries began experiencing increased pain and was taken in for evaluation on 06/16/2024. An MRI was schedule with a follow up appointment on 06/20/2024. A previously bulged disc and arthritis were diagnosed, and physical therapy was recommended to relieve nerve impingement, and EE began missing time.

A haul truck operator drove into the back of a haul truck that had stopped, crushing the cab and pinning EE under the steering column for about 1 hour. EE had nerve damage to EE's legs.

The front suspension bolts sheared off and caused the front of a haul truck to hit the ground. The operator was bruised by the seatbelt.

An employee's hand slipped off a haul truck shifting handle and EE struck the trailer brake module, causing a fracture to EE's hand.

The employee reported back pain and was checked and release by a doctor at a clinic. The employee continued working, but was unable to report to work on July 1 due to back pain that increased during scheduled days off. An MRI was taken and a doctor diagnosed a chronic back condition, not related to any specific incident.

A haul truck operator backed through a berm and tipped the truck over, resulting in a back injury.

Employee made a right turn off the haul road to enter the site. Upon entering the site, the employee turned sharply to the right. The truck was unable to complete the turn and overturned onto the driver's side. Employee received a cut to the top of right hand requiring stitches and extensor tendon repair of right index finger.

While using a hoist and a forklift to position a piece of machinery, the employee got EE's finger pinched between a fork and the load.

An electrician was driving a pickup and dropped a handheld radio. While picking it up they ran up on a berm and rolled the pickup over. Employee was released back to work by the doctor, but an X-ray showed cracked ribs.

A mechanic was beginning to lift a gear case off the floor with a crane when one of the slings caught on a bolt. This caused the case to tip over, whipping a drain hose on the case towards the mechanic, fracturing left wrist.

The pickup truck's RR tire blew while driving south on the haul road causing the truck to slide into the ditch. The abrupt stop caused the driver's back to be jarred resulting in a thoracic vertebrae compression fracture.

A plant millwright was pushing on a wire rope cable to free up the feeder chain that was being pulled by the cable when his glove was pulled into a sheave, lacerating his right thumb, requiring stitches.

The teeth on the shovel bucket hooked the side of the truck bed jarring the operator in the cab of the truck. On January 28, 2015 the operator received medical treatment that makes this incident a medical reportable injury.

On October 15, 2013 a haul truck driver was jarred when dumping a load of material and the truck box bounced the truck. The technician had medical treatment on December 16, 2013 making this a reportable incident.

A haul truck pulled to the side of the road and stopped to allow another haul truck to pass.The oncoming truck continued up the ramp and contacted the stopped truck jarring the truck and the operator.The technician in the stopped truck was diagnosed with an abdominal hernia as a result of straining and bracing himself.

A millwright was in a man basket, booming down, when the boom contacted the corner of the shovel bucket. The technician continued to use the boom down control until the boom slid off the shovel bucket at which time the basket dropped and then bounced back up suddenly. This action caused the technician to be jolted in the basket resulting in a fracture to his left ankle.

Scraper operator crossed over a windrow at a high speed cause the scraper to rock side to side at which time the operators head hit the window latch causing a laceration on the top of his head.

9/21/12 a tech was jostled while riding in a crew bus.She was examined that day and seen by several doctors over the following months for increasing shoulder pain without findings while she continued to work.5/29/13 a new doctor revealed a condition that requires a procedure that makes this incident reportable when that procedure occurs.She did not report to work on 6/2/13.

Team Member was in the process of releasing a brake on a R/R car when the brake stick slipped from the brake wheel on the car allowing it to come back and strike her in the mouth.

Overburden was dumped from a haul truck when the box hit the end of the hoist cylinder stroke unusually hard. This jarred the operator which resulted in low back pain. The technician continued to work and on May 4 he received an epidural injection to relieve his back pain.

The employee was sitting in the back of the mantrip/bus when the vehicle hit a bump while transporting a crew. The bump caused her to be jostled from her seat and hit her knee. On 7/17/2012 she had a medical procedure that makes this event reportable.

A haul truck was backed into the shovel for loading when material came off the dig face landing on the tail of the truck' s box. This caused the truck to bounce, resulting in the driver hitting her nose on the steering wheel and stretching her back. On December 7th she received an epidural injection to relieve back pain.

Employee was cleaning coal out of a railroad coal car using a vacuum hose from our vac truck. The vac hose became stuck and while employee was trying to dislodge the hose, his arm got sucked into the hose from the vacuum. This resulted in a strain and contusions in his arm.

Technician bumped her head when riding in a crew bus.X-rayed and released to full duty driving haul truck.She had a MRI due to having slight pain between shoulder blades when driving truck.Diagnosis on 4/22/11 from MRI confirmed minor compression fracture in corner of T3;bulged discs and bones spurs from degeneration.Released full duty.

Employee called supervisor and complained of a sore neck. Supervisor asked what happened and the employee said nothing. No event could be determined by the employee, his neck just got sore. We transported the employee to the hospital where they found no apparent injury and called it a neck strain.

Employee was returning to regrade area with a loaded 657G Scraper to dump his load. As he entered the dump area he hit a bump that bounced him up against the seatbelt. As his body returned to the seat the scraper was bouncing up causing severe back pain.

While loading jeep & booster axles on lowboy trailer-driver thought everyone was aware he was pulling forward-employee was not clear-driver ran over right foot with rear axle tires on left side.

Employee was operating the lube truck, hit a bump and hit head on the inside roof of the cab. He was not wearing a seat belt.

While operating a Caterpillar 777 Haul Truck employee reached under the seat to see if he could feel air coming from the heater. Stuck his finger throught the guard and contacted fan blade. Laceration to the right index finger requiring stitches.

At the time of the incident ee was standing next to a truck conversing with a TBCC mechanic about a job. As they were standing there a van driven by another TBCC employee began backing up towards the truck. The TBCC employee driving the van did not see the truck parked behind the van and backed into the truck pinning ee between the van and the service truck door.

An SOS Staffing employee working at Thunder Basin Mine, Black Thunder site lost control of his haul truck, running it through the berm & tumbled 330 feet pinning the operator and causing burn injuries to the operator's right thigh and multiple contusions.The operator has been released from the hospital, but the hospital and operator are not releasing any work status.

While the bus was moving EE stood up to move her lunch box, she had one hand on the partition behind the driver and holding a cup in the other hand. The driver of the bus was making a stop when the brakes grabbed causing EE to fall forward striking a mirror bracket.

Employee was a passenger in a light-duty vehicle that overturned when the driver lost control when he encountered a slick section of roadway.The employee fractured her 7th rib on her left side. The employee was in compliance with company and mine rules and regulations. The accident did not involve the mining system, job skills or protective equipment.

HT was coming down the ramp from the dump hit a patch of ice and slid into EE's truck on the right hand side. EE's truck was stopped at the time of impact moving the truck a couple of feet. Operator reports back and neck strain.

While tying down a rail car with a brake stick it became hard to tie and employee then pulled down hard 4 times to get the brake tied. Afterwards she began to feel pain in her lower back.

Employee was riding in the back seat of the employee transport bus at the end of the shift traveling to the employee parking lot. A light snow covered the haulroad. The bus hit a bump created by haul trucks that bounced the employee into the side of the seat injuring his ribs.

Employee claims his back started hurting while operating 651 cat scraper.

Employee reported this alleged injury on 7/25/06. Running a 657 scraper and hit a rock with tire. This bounced him and he claims back pain.

HT 255 approached the berm at approximately a 45 degree angle with his right rear tires hitting the berm first. The berm measured 15ft at the base of the berm and was approximately 6.5ft in height. The truck went over and slid down the dump on its left side and came to a rest on all tires at the bottom of the dump.

Employee was riding on bus at end of shift when it hit a bump jarring his back. He continued to work his remaining rotation and on 11/22 saw a physician who put him on restricted work.

Hit a bump while driving haul truck. Jarred neck and back and got shooting pains down right leg.

Haul truck ran out of fuel while going up a ramp. Operator applied brakes but truck rolled backwards hitting spoils and tipping over on right side. Operator was wearing his seat belt but got banged around in cab bruising his shoulder. He was not released to work on his next scheduled shift.

While attempting to turn off a main haulroad in a service truck, the truck began to slide and eventually turned onto its right side. The employee was thrown across the cab and sustained injuries to his right thigh and knee. He began to loose time from work on January 3, 2005

Employee was driving a Dresser water truck. He hit a pothole and experienced back pain the following day.

The window in the haul truck shattered and employee received glass in his eye.

EE WAS HOOKING UP AN AIR COMPRESSOR TO THE PINTLE HOOK ON THE BACK OF A SERVICE TRUCK WHILE THE TRUCK WAS BACKING UP TO THE COMPRESSOR. EE RIGHGINDEX FINGER WAS CAUGHT BETWEEN A SHACKLE ON THE HITCH OF THE AIR COMPRESSOR AND THE PINTLE HOOK.CAUSING LACERATIONS AND A FRACTURE TO THE FINGER

OPERATOR WAS TRANSPORTING A CATERPILLAR D-11 DOZER WITH TRACTOR AND LOWBOY TRAILER. AS THE OPERATOR STARTED UP A RAMP HE DOWNSHIFTED THE TRACTOR TO 1ST GEAR. THE FRONT TIRES OF THE TRACTOR CAM E OFF THE GROUND AND THE TRACTOR FLIPPED OVER BACKWARD. THE OPERATOR SUSTAINED A BUMP TO THE RIGHT SIDE OF HIS HEAD AND A LACERATION TO HIS RIGHT SHOULDER REQUIRING SUTURES.
