Employee was in a scoop bucket going to the batteries first and the breaker knocked on the scoop which in turn locked up the park brake striking EE up against the ejector blade.

Employee was driving an empty shuttle car toward the miner when employee hit a hole or dip in the roadway causing employee to strain lower back, neck, and right foot.

Driver just drove out of the road and turned the truck upon its side. Driver was loaded driving only 10 miles per hour. They drove into the ditch and turned truck upon its side.

Employee was riding in a scoop going to the section due to the super steer being down. The scoop hit a hole in the roadway causing employee's back to be jerked.

Employee was traveling down the roadway when he ran over a rib that had rolled out causing him to hit his head on the canopy. Employee started missing 08/24/15.

Employee was struck by a water line being pulled by equipment.

Employee ran over a bump in the road and hit his head on the canopy. Employee stated that he was sick to his stomach.

Employee riding in a mantrip with his hand on the edge of the fender and as the mantrip was backing up it hit on offside in the rib catching his hand between the mantrip and rib severing his fingers.

Employee was riding in scoop when it hit a rock in the roadway.

Under investigation. Employee started missing 04-02-12.

Employee was riding in the mantrip on the inby end facing in on the rib side, when the mantrip struck something, causing it to fly up and strike the employee on the left forearm.

Employee was crawling down the belt on lolo and his foot must have slipped off the side. They were in the process of setting bits in the miner and when they backed the miner up to roll the heads it hit the Mobile Bridge Carrier (MBC) causing it to twist and rubbed employees foot.

Employee was installing a draw head on his RRD motor before coupling to the rock duster. A second supply motor was coming up the track and the operator did not notice that the switch was thrown toward the side track where the employee was working. The second motor hit the RRD motor and knocked it into employee.

Employee was going up track when his rail runner hit the guard with the canopy letting the guard fall down and pin him in the operators compartment.

Lotrac operator was going through scoop doors at the end of the track. Lotrack operator saw employee standing next to the door and when the operator went through the door he heard employee make a grunting noise. He then looked around and saw employee on top of the lotrac bent over. The lotrac operator had to back lotrac up so that the employee could get off the lotrac.

Employee encountered smoke coming from face drive. When he went to check it out, he got into the smoke and it caused him to have difficulty breathing.

Employee got his right thumb caught between fender of scoop and tire. Injury required 13 stitches.

Employee was running the miner when a ramcar came to get loaded a piece of coal flew out from under the tire and struck him in the right eye. Injury required stitches.

Employee was running fork lift and when he raised the fork lift up to set over, he hit his head against the roof. Employee started missing 04-17-09.

Employee was helping to install a new canopy on scoop. Employees finger got caught between lo-trac fork and canopy as they were trying to line the canopy post up. Injury required stitches.

Employee was traveling down #7 entry on 001 intake on the Johnson Industries super car when the front wheel struck a rock causing him to lose control of the steering wheel. The steering wheel struck his hand/finger. Injury resulted in a fracture.

EE was tramming a double head roof bolter up a hill and the roof bolter had to be pushed by a scoop. He accidently stuck his foot outside the operator's compartment and the scoop bucket hit his foot. The injury to his left foot required that the ends of his 3rd and 4th toes be amputated and the 2nd and 5th toe was fractured.

Employees back started to hurt prior to him coming to work on 05-02-08 due to a re-occurrance of an old injury which occurred on 02-12-08.

Employee was walking behind the roof bolter in the #1 entry and a scoop came up the #1 entry and hit employee in the legs with the scoop bucket below the knee. Employee did not hear scoop due to noise from the roof bolter.

Employee was getting bits off ram car when he got pinned by another ram car.

Employee was putting power box in a break when the back of a scoop caught the end of a piece of track rail causing it to swing around and hit him on his left leg.

Employee was riding mantrip to section when a rock came in the mantrip and struck his left leg below the knee.

Employee was riding mantrip buggy from the face to the track when a piece of belt came in contact with his left knee/leg. Injury resulted in a fracture.

Employee was standing beside back bridge when bridge began to move, it slid a little sideways pinning his right foot under it.

The miner was mining in 3 right pillar, made 2nd run and backed out to timber off when employee started yelling, co-workers ran back to him they found the mbc track was on top of employee's left foot. Accident resulted in fractures.

Employee was tramming ram car down roadway when he hit a hole in the roadway and felt pain in back. Employee started missing 06-25-05.

Employee was struck by a scoop while working on another scoop which was stuck/broke down in an intersection. Employee was down in the deck of scoop #1 exchanging a hydraulic hose and had his feet sticking out when another scoop came along and struck his left foot (heel) requiring approximately 30 stitches.

Employee attempted to remove a bag of rock dust from scoop while scoop was in motion and was caught between batteries on scoop and mine roof. Accident resulted in 5 stitches behind right ear and a fractured collar bone.

Supply car de-railed in a waterhole, car had been jacked up and pushed over on rail. Employee couldn't see rail to make sure car was on track because of water. Employee stuck hand under car to feel the rail and wheel to see if it was on the track, the car slid over catching his hand. (Fractured index finger on right hand and 15 stitches)

Scoop operator was attempting to move large loop of belt, scoop moved through under belt too far causing end of belt loop to come into operator compartment with operator. Employee started missing 05-03-04.

Employee was helping check empty train cars when he somehow fell under the moving train car and damaged his left leg to the point in which it had to be amputated 6"-8" below the knee.

INJURED EMPLOYEE HAD BACKED HIS TRUCK UP TO UNLOAD IN BIN. WITH BED UP A PC OF COAL GOT THROWN FROM THE BED HITTING THE EMPLOYEE ON TOP OF HIS HEAD CAUSING CONTUSION LACERATIONS.

EMPLOYEE WAS HIT WITH SCOOP, BREAKING HIS LEFT ARM. STARTED MISSING 10/3/02.

EMPLOYEE WAS DRIVING PERSONNEL CARRIER, WHEN FRONT WHEEL DROPPED INTO A GROOVE IN ROADWAY, CAUSING STEERING WHEEL TO SPIN LOOSE FROM OPERATORS GRIP CATCHING THUMB ON LEFT HAND. A HAIRLINE FRAC TURE TO THE LEFT THUMB.
