Employee was building a crib back, when a piece of draw rock struck him on the back of his right hand. Injury required stitches.

Employee was packing an eight foot timber when he felt a sharp pain in his lower back. Employee started missing 02-06-12.

Employee was walking by the Power Center when a piece of draw rock fell striking his right leg.

Employee was installing a roof bolt when the bolt kicked out and struck his arm.

Employee was cutting end of dust hose off when he inadvertently cut himself.

Employee was attempting to remove a double tire off ram car with a 3/4 ratchet and cheater bar. The ratchet slipped off and struck his left leg/knee. Employee started missing 01-25-11.

Employee was shoveling 3A belt when the rib rolled out and struck him on the left foot. Employee started missing 09-30-10.

Employee was walking beside bolt machine when he fell on his left foot.

Employee was bolting on the operator side of the working place when the rib rolled out and struck him on the back and leg. Employee started missing 09-13-10.

Employee was going up track when his rail runner hit the guard with the canopy letting the guard fall down and pin him in the operators compartment.

Employee tripped on hoses in the floor and put hand out to catch himself. Hand was cut on metal panning.

Rock fall occurred in 1 East Mains in the #5 entry at break #49. Fall measured approximately 20' wide x 30' long x 5' thick.

Lotrac operator was going through scoop doors at the end of the track. Lotrack operator saw employee standing next to the door and when the operator went through the door he heard employee make a grunting noise. He then looked around and saw employee on top of the lotrac bent over. The lotrac operator had to back lotrac up so that the employee could get off the lotrac.

002 section was running coal when they cut into a core hole.

Rock fall occurred on 1 north main between BK71 to 72 #2 entry. Fall measured approximately 20' wide by 60' long and approximately 4' to 6' thick.

Employee was making up setup belt, he was cutting a conveyor belt with a knife, the knife slipped cutting his right leg. Injury requiring stitches.

Employee was changing tire on Lo Track. New tire was laying on rib/ He was lifting tire up when tire slipped and turned around mashing fingers against rock behind tire. Injury required stitches.

Employee was getting off loader stepped on a rock and twisted left knee. Started missing work on 07-19-10. Returned 07-26-10 light duty.

Employee was attempting to jack up a wrecked car when the car fell sideways and caused the bar to strike him across his nose.

Employee had his hand on top of the roof bolter canopy and was going to swing boom out when a piece of draw rock fell out and hit him on the hand.

Employee said his left knee had been hurting for the last two weeks. He stated that he didn't know what caused it but that he thought he hurt it working on an overcast and that he was on his knees a lot. Employee started missing 05-05-10.

Employee was walking through #2 right entry when a piece of waterline busted, knocked his feet out from under him, causing him to fall and twist his knee.

Employee went to get a nail board that was leaning up on the rib when it fell and struck him on the leg. Injury required 11 stitches.

Employee was attempting to tie beams to the top when a jack slipped out causing the beam to fall. As the beam fell, it struck him and caused him to fall back into the scoop bucket injuring his back.

Employee was running the miner when a ramcar came to get loaded a piece of coal flew out from under the tire and struck him in the right eye. Injury required stitches.

Employee was handling a timber (lifting and setting) when he experienced pain in his lower right side of stomach. Employee is scheduled to have hernia surgically repaired.

Continuous miner was in the #6 entry and was mining cut #30 when a rock approximately 7 feet thick x 18 feet wide x an undetermined length fell on the continuous miner. The continuous miner was unable to tram out from underneath the rock and a retriever was utilized to pull the miner out from under the rock.

Continuous miner was cutting in the #4 entry in left side of pillar, when the rib rolled off and went into deck of the miner. Rib roll pushed E-stop in and miner wouldn't start back. The roof bolter was being utilized to bolt up to the miner, when a rock 3' thick fell on the head of the miner. A retriever was utilized to pull the miner out.

Hoist was down due to an electrical conductor had burnt off.

Employee was repairing a broken track on the continuous miner, when the cribbing that was in place failed.

A rock fall occurred in 1 North Mains in the #4 entry at break #103 extending to break #104. Fall measured approximately 20' wide x 70' long x 5' thick.

A rock fall occurred in the 3 East Mains return air course 1 break outby survey station #4189 at break #28 in the #4 entry. Fall measured approximately 20' wide x 30' long x 6'-8' thick.

Employee was pulling water line out from under the belt structure and when it came out it sprung upward and caught his hand between the belt structure and the water line.

Rock fall #1 occurred on 1 East Mains in the #1 crosscut between #3 and #5 entry. Fall measured approximately 20' wide x 60' long x 4'-5' thick. Rock fall #2 occurred on 1 East Mains in the #2 crosscut between #6 and #7 entry. Fall measured approximately 20' wide x 20' long x 3'-4' thick.

Employee was shoveling belt when he stated that he felt pain in his lower back and his right leg was hurting.

Employee was traveling down #7 entry on 001 intake on the Johnson Industries super car when the front wheel struck a rock causing him to lose control of the steering wheel. The steering wheel struck his hand/finger. Injury resulted in a fracture.

Rock fall occurred on 1 North Mains in the #2 entry at break #25. Fall measured approximately 20' wide x 90'-100' long x 4'-6' thick.

Rock fall occurred on 3 East Mains at break 31 in the #2 entry. Fall measured approximately 20' wide x 30' long x 5'-7' thick.

Employee was operating the continuous miner in the #7 heading when a piece of drawrock fell out between bolts and struck his right hand. Injury resulted in a chipped fracture.

Rock fall occurred in return at bottom of slope in the #1 entry along the solid wall 3 breaks from fan. Fall measured approximately 20' wide x 30' long x 3'-5' thick.

Employee was setting roof jack and beams in the track entry beside #7 conveyor belt at break #36. Employee was lifting beam up on lo-trac when he dropped it on his hand mashing small and ring finger on left hand. Injury resulted in a fracture.

Employee was setting jacks on belt when another employee's hammer slipped and struck him in the mouth. Injury resulted in stitches and loosened teeth.

Rock fall occurred on 1 East Mains at break #14 between the #4 and #5 heading and extending over to the edge of the #6 heading. The rock fall was approximately 20' wide x 90'-100' long and 6'-8' thick.

Employee was unloading stands out of ram car for belt set up when he got his finger mashed between two stands. Injury resulted in a fracture.

Rock fall occurred on 1 East Mains on #4 belt at break #45. Fall measured approximately 20' wide x 90' long x 5'-6' thick.

Hoist was down due to changing idler shaft roller.

Employee strained his lower back while shoveling belt. Employee started missing 09-23-08.

Employee turned the conveyor belt off and was crossing over the belt, when he stepped off other side he twisted his knee. Employee required surgery and started missing 12-04-08.

Employee was cutting a band on a pallet of rollers when the band broke it struck his right arm causing a laceration. Injury required 7 stitches.

Employee was operating a roof bolter in the #3 entry and he hit his left arm on a roof bolt plate. Injury resulted in 9 stitches.

Employee was walking behind the roof bolter in the #1 entry and a scoop came up the #1 entry and hit employee in the legs with the scoop bucket below the knee. Employee did not hear scoop due to noise from the roof bolter.
