The employee's left shoulder was injured when the left side of the loader bucket on the caterpillar 992G EE was operating caught a large rock. This caused the employee's upper body to twist to the left causing pain in EE's shoulder.

Caterpillar 777F Truck driver claims EE's lower back was injured after a large rock rolled out of the caterpillar 992 loader bucket and jarred EE's truck.

When a 992G loader was cleaning the base of a highwall. The bucket of loader knocked a section of rock lose that fell onto the bucket. The loader was jolted around causing the employee to hit their right side on the arm rest. Seat belt was being used.

The employee was cleaning loose material away from the highwall in the #8 coal pit when the bucket on the loader hit a rock. This caused the loader to abruptly swivel hard to the right sending a jolt through the employees back.

The employee was injured during routine haulage operations while he was operating a caterpillar 40 ton articulating truck. The employee suffered from a cervical strain that occurred when the first bucket of material was loaded onto his truck.

While EE was prying at a rock with his loader bucket, the bucket slipped, causing a quick jerk to EE's back. Diagnosis is pending.

At end of shift, while tramming Cat. 992K to parking lot, EE drove through a dip in the road, causing a quick jerk to his back. Diagnosis is pending an MRI.

As EE was desending the haul road in his loaded coal truck, his brakes failed at the bottom of the haul road, resulting in his losing control and going over a 25' embankment. Prelimenary reports are that his left arm is broken at the wrist.

EE WAS IN THE PROCESS OF PULLING A LOAD OF WATER UP THE ROAD WHEN SOMETHING BROKE ON THE TRUCK CAUSING THE TRUCK TO GO BACKWARDS APPROX 40-50 FEET. THE TRUCK CAB TURNED OVER ON ITS LEFT SIDE. EMPLOYEE RECEIVED 5 STITCHES TO THE LEFT SIDE OF HIS HEAD. HE WAS TAKEN TO HAZARD HRH WHERE HE WAS TREATED AND RELEASED.

Ee was in the process of dumping a load of rock when he backed over a 2 foot trench. He complained of a lower back injury, his diagnosis is pending.

EE WAS TRYING TO BREAK A LARGE ROCK WHEN THE TEETH OF THE LOADER SLIPPED, CAUSING THE LOADER TO JAR, AS A RESULT EE'S LEFT KNEE STRUCK THE CONSOLE, HE DROVE HIMSELF TO THE DOCTOR. HE REMAINS OFF WORK, HIS DIAGNOSIS IS PENDING.
