EE WAS DRIVING FRONT END LOADER. EE HAD TO GET OUT AND CLIMBED DOWN THE LADDER. EE MISSED THE BOTTOM STEP AND TWISTED LEFT ANKLE.
EMPLOYEE WAS GETTING OFF THE LOADER. WHILE MAINTAINING 3-POINT OF CONTACT ON THE LADDER, EMPOLYOEE LOST BALANCE ON THE LAST 2 STEPS TO THE GROUND AND FELL BETWEEN THE FRONT TIRE AND THE STEEL OF THE MACHINE.
Water pipeline blew a hole near booster pump approx. 5' deep and 4' wide. Employee was standing near break in pipe while waiting water pressure to decrease to make repair. Bank of hole sluffed off and employee fell into hole resulting in injury to their right ankle.
The employee was washing under the low washer stepped over a pipe and slipped on wet ground falling on left side.
The employee was working in the locked out clarifier when the employee slipped and struck their back on the rake. The employee initially thought they were okay. We recorded the fall. The next workday the employee said their back hurt and the employee went to the clinic and was told they had strained back.
EE was exsiting excavator and jumped off the last step twisting an ankle on a rock.
Employee parked haul truck to take a break. Employee stated slipped and fell while walking across the haul truck parking area. With all the rain we were getting the area may have been slippery.
EE was cleaning material off of the conveyor. EE used a pipe wrench to turn the pulley. EE lost EE's balance and tripped and fell on the conveyor. EE cut EE's leg and was treated with 1st Aid. EE later complained of leg hurting and was taken to the clinic and left leg treated for a laceration. EE was given antibiotics and light duty.
Freezing rain and Cold condition contributed to the incident. While walking toward pump to fuel unit in pit. Slipped due to freezing rain injuring left hand. Employee taking for treatment and was diagnosed with a hairline fracture of the left hand and is on restricted duty till further notice
EE was climbing up the ladder on the haul truck. EE's right foot was on the bottom rung which swung under the bumper. EE's right shin and knee hit the face of the bumper, pushing EE's foot off the step. EE's foot hit the ground, all of EE's body weight landed on EE's right foot. EE was taken to the doctor and being treated for a spraining EE's right ankle
On Thursday morning EE was on way to grease shed to pickup supplies to service haul truck. We had heavy rain the night before and into that morning. From all the rain there must've been some wash out around the shed. EE walked into puddle and twisted left ankle.
Tripped while stepping over elevated power line. Strained tended in right arm around elbow.
On Tuesday 2/14 around 7:10 am EE was rolling up a 24 inch wide conveyor belt when EE slipped and fell, injuring lower back. This incident was not reported until Wednesday 2/15 by text messaging Supervisor at 2:27 am. Supervisor called and informed me about the incident.
EE slipped on patch of ice causing him to lose his balance and fall backwards on his right ankle. Fibular Fracture - surgery needed.
The miner was dismounting a 400D John Deere Haul truck, when he caught his left foot on the top step. He did have ahold of the hand rail and was going down backward. He lost his footing and fell 6 feet to the ground landing on his back. He hit his right arm on a hydraulic cylinder, breaking his arm just above the wrist. There were no other injuries.
Employee was stepping down from scales platform in office and strained right knee.
Employee climbed down from loader and stepped into a small hole and twisted his right ankle
EE was exiting the plant control house and slipped on the steps hitting his right knee and lower back.
EE came down stairs from small shaker. When he got on the concrete he slipped on an old metal pad hitting his head, back and arm.
EE was washing green algae from concrete. He slipped down hitting right shoulder on concrete and scraped side of his face.
EE climbed portable ladder onto control house roof. He was walking across it and slipped off of the roof and fell approx. 10 feet onto the tounge of welding trailer sitting next to control house. He had compound fracture to lower right leg.
