On 6/3/2010, after employee was suspended, he alledged that he was carrying his tool box and slipped on the track rail. He was seen by a doctor on 6/15/2010, but did not address when employee could work, but tests were scheduled. The company does not have any information as to whether this is a reportable event.

Injured was with crew loading belt structure into scoop bucket. When EE twisted his body his foot remained stuck in approx. 8" of mud, twisting his left knee.

EE was changing batteries on scoop, fell and hit his shoulder. This report is being generated under protest to terminate citation #9006074.

EE got out of scoop to check pumps, lost footing on slick bottom. Injury resulted in knee immobilizer and crutches. This report is being generated under protest to terminate citation #9006032.

EE was hanging miner cable. Jumped up to hang cable, came down and turned his ankle. Injury resulted in an aircast on his left ankle. This report is being generated under protest to terminate citation #9006073.

EE was walking around feeder and tripped on feeder cable hitting his forearm on the feeder. Injury resulted in application of sling. This report is being generated under protest to terminate citation #9006094.

EMPLOYEE TRIPPED OVER UNKNOWN OBJECT STATES HIT STOMACH. EMPLOYEE NOTIFIED COMPANY ON 12/18/09

Employee stepped from ladder on top of tank when the tank collapsed. He grabbed the edge the tank, cutting his fingers and resulting in three stitches.

Employee was helping install a silt fence, when he stepped in a drop off and fell approximately 4 feet. He was complaining with neck pain and was transported to the hospital and kept overnight for observation as a precautionary measure.

Employee was outside loading top structure into scoop when he tripped on a cable and his back.

Employee was installing mirror on hydroseeder door when he lost his balance and grabbed for the door bracket to catch himself straining his wrist.

Employee was getting out of foreman's truck when he tripped on a flashlight wire and fell on his left side on ground straining his left leg.

Employee was hanging curtain then turned around and stepped in hole twisting his left knee.

Ground frozen and covered with approx. 1 inch ice and 1/4 inch snow. Driver got out to check tailgate and slipped - spraining left ankle.

Employee slipped and fell in mud straining his hip.

EE was walking around faces in #4 entry, stepped over gob into a hole and twisted his knee. This report is being generated under protest to terminate citation #9006063.

Employee was crossing the hood of a rock truck and tripped on a shovel and fell hurting his back.

EE was changing oil in a dozer, when he slipped on the track. He tried to jump from the track and his left hip hit the open door on the tool box on his grease truck, causing a puncture wound that required stitches.

Truck driver was hooking lines onto trailer behind tractor cab. Started to dismount tractor deck, missed step and fell on left shoulder. Driver declined squad but agreed to be taken to ER to be checked out. Driver broke left collar bone, cracked or broke multiple ribs on left side and received scraps to left forearm.

Employee was climbing off drill and slipped or missed bottom step causing him to fall and strain his back. Physician did not take employee off work until 9/29/08.

EE was operating the miner and started to turn around. He was standing in a muddy arae and his boot stuck in the mud, causing him to twist his knee. He finished his shift, continued to work until his appointment with an orthopedic surgeon. All X-rays were negative, but due to pain in squatting, was placed on restricted duty on 9/8/08.

Employee was walking down along a concrete retaining wall and fell over the wall to the concrete floor below. The employee received 12 stitches in his left forearm near the elbow.

EE had been helping grease his shuttle car and was walking back to the operator's deck. He slipped and fell and his arm went in between the discharge roller and the rear brace. He bruised his ribs and stained a neck muscle. EE continued to work through 9/11/08 and his physician took him off work effective 9/12/08.

While moving a miner out of a crosscut, he stepped backwards into a hole and twisted his right knee. Employee had outpatient surgery on 9/26/08 to repair damaged knee.

EE was roof bolting and stepped in a hole, twisting his knee. EE had a put patient medical prodecure performed on his knee on 4/23/08.

Employee was walking up # 5 entry when he slipped and twisted his left knee. Employee has continued to work, but his physician placed him on restricted duty effective 9/17/08.

Stepping up on dozer track and felt something pop in the back of his left calf. Physician did not take him off work until 2/22/08.

Crossing push arm with foot on idler, when his foot slipped, straining his back. EE did not miss work until 2/10/08.

Top rail of handrail was removed to provide service truck boom access to replace crusher parts. As the boom was being lowered at job completion victim stood and stepped back; striking middle rail of handrail. He fell approx 11 feet landing on his feet, and then right side. Injury occurred to Rt. ankle, Rt. elbow and Rt. shoulder.

Employee slipped in mud in parking lot and tried to catch himself as he fell causing his right elbow to become dislocated. Physician took him off work effective 1/17/08

Building road, employee got off of dozer to determine exact path of gas line when he tripped and fell fracturing hand.

Employee was climbing up 10 ft berm. When he grabbed limb for support it gave way causing him to lose balance and fall injuring his back. He began missing work due to this injury on 7/14/07.

Employee was helping mechanic to loosen bolt, when he slipped and twisted his left knee.

Slipped on lowboy deck while unloading augers twisted knee. (EE did not lose any days of work until surgery on 11/16/07)

Employee finished fueling excavator and was climbing off when he slipped and fell off machine dislocating his left ankle. Taken off work 4/15/07.

Dozer was being serviced with blade safety stands under push arms. Operator was climbing back up on dozer when he slipped & fell fracturing left wrist.

HE WAS STANDING ON THE CAT WALK OF A BULLDOZER, WHEN THE BLADE WAS LET DOWN IT JARRED HIM AND HE FELL DOWN ONTO THE DOZER TRACKS.

Employee was in process of making belt splices. When pin was pulled out, several pin windings stayed in clips. As he attempted to extract windings, the belt separated, and he fell, striking right cheek against belt bed rails. 6 stitches.
