Injured was building new regulator in southwest area of mine. The air pressures sucked injured thru the open regulator causing him to fall and strike his head on the regulator panel. This resulted in a laceration requiring 4 stitches.

Employee was standing on ladder in bottom of oil change pit removing a hydraulic pump from a locomotive. Pump came loose suddenly striking him and causing him to fall from ladder. This resulted in back strain.

INJURED WAS WALKING THRU THE CUT THRU TO THE SLOPE BELT. INJURED'S FOOT SLIPPED CAUSING HIM TO FALL BACKWARD. THIS RESULTED IN A FRACTURE TO BONE IN THE LEG. (INJURED FINISHED SHIFT AND DID NOT SEEK ANY MEDICAL ATTENTION UNTIL 2/12/07)

injured stepped onto longwall shield pontoon. injured's foot slipped off the pontoon and his toe caught the relay bar causing him to trip. this cause injured's left foot to twist resulting in fracture to small bone above the ankle. injured finished shift before seeking medical attention.

Injured was walking belt slope when he slipped and twisted his foot. (this acident was not reported to mgt until 01/17/07)

injured was geting off the elevator at the elevator bottom when he slipped and fell. Injured struck his left hand on the elevator door when he fell. This resulted in a fractured left hand.

Running away from roof fall, employee slipped and fell straining his groin.

INURED WAS WORKING ON BELT LINE. AS INJURED TURNED AROUND TO TURN OFF BELT SWITCH, HIS FOOT SLIPPED CAUSING HIM TO STRIKE HIS LEFT ARM AGAINST THE BELT STRUCTURE.

Injured was going to the section parts car to get parts. When he stepped off of the ride, he stepped in a water hole and twisted his right ankle.

Injured was using a air hose to blow wet coal off of the dryer bed. When he turned on the air he lost his balance causing him to turn his ankle.

INJURED RIGHT FOOT BECAME STUCK IN DEEP MUD. INJURED TRIED TO FREE HIS FOOT AND TWISTED HIS RIGHT ANKLE.

Injured was working on longwall recovery. Injured was recovering a drill steel from a "z" drill. As injured turned to lay drill steel onto the pan line, he lost his footing. Injured fell and struck his head against a shield resulting in a neck strain.

injured was walking on a board in a wet area when his foot slipped causing him to fall.

INJURED WAS HELPING TO TAKE CHAIN OFF A SHIELD CARRIER. HIS FOOT SLIPPED OFF THE CARRIER CAUSING HIM TO TWIST HIS RIGHT ANKLE.

Injured stepped down from a flat car into a tire rut in wet uneven ground twisting his left knee.

Injured fell thru a hole in the floor on the gate floor of the dump. Injured did not request medical treatment until 01/24/06 and recieved same day.

Injured and co-worker were installing a set of double door frames. Frame work was stood up and when they lifted up a leg to install some additional blocking the frame fell and pulled the injured to the ground. Injured fell on top of the frame work and her co-worker fell on top of her.

INJURED WAS WALKING TO PUT UP TOOL IN TOOL BOX WHEN HIS FOOT CAUGHT IN SOME SCREEN WIRE LAYING ON THE MINE FLOOR. THIS CAUSED INJURED TO FALL AND BRUISE HIS LEFT LEG.

injured had taken down a scoop charger cable and was was walking with the plug when his left knee gave out. injured fell and twisted his left knee.

Injured was crossing behind the belt drive area. When he stepped in mud and strained his left knee.

INJURED WAS OPERATING A LOCOMOTIVE. HE STEPPED OFF THE LOCOMOTIVE AND HIS FOOT SLIPPED, CAUSING HIM TO SET DOWN ON THE MINE FLOOR. THIS RESULTED IN A CONTUSION TO HIS HIPS.

INJURED WAS WALKING FIREBOSS ROUND. INJURED STEPPED OVER A PIECE OF OLD BELT STRUCTURE. HIS RIGHT FOOT CAUGHT ON THE STRUCTURE. HIS LEFT FOOT SLIPPED AT THE SAME TIME. THE SLIP CAUSED HIS RIGHT LEG TO TWIST AND BREAK A BONE.

Injured was walking with a new air compressor and stepped across a ditch as he stepped across his left knee gave out causing him to fall forward and landing on his hands and knees.

INJURED WAS STANDING ON PUMP MOTOR ATTEMPTING TO HANG COME-A-LONGS IN TROLLEY. INJURED FOOT SLIPPED CAUSING HIM TO FALL AND STRIKE A PROTRUDING GREASE FITTING ON THE PUMP MOTOR.

A subcontractor's employee was improperly crossing bracing when he lost his grip and fell. He suffered a fracture in his lower left leg.

Injured stepped down off of a dozer. As he stepped down off dozer arm his foot slipped causing him to fall backwards. As he fell he tried to catch himself with his left hand.

Injured was standing between the handles of a wheel barrow while she was removing dust from it. As the last bag was removed the wheel barrow tipped over causing her to fall and pull a muscle in the back of her left leg.

Injured was changing bits out on the head when he climbed back across the plow motor to get out of the way while the operator bumped the plow. He stepped on the motor, slipped and fell on the motor, landing on his tail bone.

Injured was crossing over a dollie tongue when his boot got caught on the dollie tongue. He fell forward landing on his knee and catching himself with his hands. The fall caused him to twist or jerk his back .

Employee was walking from his car to the bath house and slipped on ice in the parking lot. Injured fell to the ground injurying his lower back.

Injured was trying to step over a rock to get into the convyor when he slipped. He caught his body weight by placing his left hand on the pan. His left foot hit the plow guide door and closed it on his thumb.

Injured stood on a bucket to reach the top of a stopping. Injured lost his balance and fell off the bucket into the rib falling on his rescurer injuring his ribs on his right side.

Injured was traveling from #3 entry at the dump point to the power center. His left foot slipped out from under him causing him to fall and striking his knee against the mine floor.

INJURED WAS STANDING ON A RAM CAR TO HANG A PIECE OF CABLE. WHEN HE STEPPED DOWN HE TWISTED HIS ANKLE.

foeman walking from tail to head along the longwall walkway. walkway had incline. slippery mine floor at #104 shield caused injured foot to slip. injured attempted to catch himself with left forearm. injured struck his forearm against the spillboard resulting a fractured elbow.

INJURED WAS CROSSING UNDER THE SECTION MONO-RAIL WHEN HE SLIPPED AND FELL STRIKING HIS RIGHT KNEE ON A TRACK RAIL.

Injured was walking in the entry when he stepped in a "hole" in the mine floor with his right foot. His knee buckled and he fell down.

EMPLOYEE WAS EXITING A 988 LOADER. WHILE CLIMBING DOWN HE MISSED THE LAST STEP. HE FELT A PUL ON THE LEFT HAND SIDE OF HIS LOWER BACK.

The injured employee had been washing out a screen from the inside. As the injured climbed from the screen, his foot slipped causing him to twist his left knee.

INJURED WAS WALKING DOWN DIRT ROAD. HE SLIPPED ON SNOW & ICE, FEET SLID & FELL BACKWARDS.

INJURED WAS WALKING FROM THE MANTRIP TO HIS TOOLBOX. INJURED STEPPED INTO A HOLE CAUSING HIM TO TWIST HIS RIGHT ANKLE.

INJURED TURNED WATER HOSE ON PRESSURE CAUSED INJURED TO SLIP AND FALL. INJURED STRUCK THE COAL RIB RESULTING IN A KNEE STRAIN.

INJURED WAS STEPPING OFF THE BACK OF THE ROOF BOLTER WITH SEVERAL TOOLS IN HIS HANDS. THE ROOF BOLTER HAS A WALKWAY IN THE CENTER. AS INJURED STEPPED DOWN, HE FELL TO HIS KNEES, STRIKING HIS K NEE ON A ROCK. THE HEIGHT OF THE STEP WAS APPROXIMATELY 20".

INJURED STEPPED ON A ROCK BETWEEN THE TRACK RAILS AND FELL STRIKING HIS RIGHT HIP ON THE RAIL.

INJURED STEPPED OFF OF A BELT LEDGE AND FELT A PAIN IN HIS RIGHT FOOT.
