ONE LAY IN ONE HOIST ROPE ON THE OTIS ELEVATOR SERPARATED AND BECAME ENTANGLED IN A ROPE GUARD. THIS CAUSED THE ELEVATOR TO STOP BEFORE REACHING THE BOTTOM. NO PERSONS WERE RIDING THE ELEVATOR. ALL ELEVATOR HOIST ROPES WERE REPLACED BY KONE ELEVATOR PERSONNEL. KONE ELEVATOR PERSONNEL PERFORMANCE TESTED THE ELEVATOR AND FOUND IT TO BE OPERATING PROPERLY.
Injured was in the process of changing a bolter tire and could not get it off. As he was trying to beat the wheel off with a hammer it bounced back and caused him to strain his wrist.
Employee was removing bolts from equipment. While removing, he was using an impact gun in one hand and with the other hand was applying pressure to the bolt head. Bolt came free from the employee's grip and cut the knuckle of the ring finger on the left hand, requiring stitches.
Employee was standing on ladder in bottom of oil change pit removing a hydraulic pump from a locomotive. Pump came loose suddenly striking him and causing him to fall from ladder. This resulted in back strain.
Injured was changing a fuse out on a portal bus when he bumped the pole causing it to come in contact with the trolley wire. This resulted in a burn to his right hand.
Injured was in the belt slope drive house preparing the take a conveyor belt roller down the belt slope. As injured placed the roller end on a stair step in the drive house, he caught his finger between the roller end and the metal step. This resulted in a tip fracture and laceration to the 1st finger on his left hand. Injured was wearing gloves.
Injured was picking up a crib block when a piece of rock 6 inches by 6 inches and 0 to .5 inches thick fell and struck the injured on the right ear. The injury required 4 stitches to close the laceration.

INJURED WAS APPARENTLY UNCONSCIOUS FOR APPROX 2 MINUTES. INJURED WAS TRANSPORTED TO HOPSITAL AND RELEASED SAME NIGHT WITH NO INJURIES.
Building braces for belt line. I-beam slipped, pinning left hand between two I-beams.
Injured was dropping high voltage cable, as he was lifting the cable out of a hanger a rail located directly behind him rolled over and struck his left heel.

Employee claims numbness, swelling and pain in both hands caused by drilling. Employee was seeing a doctor on his own. He started going to a company authorized doctor on March 2, 2007. He is currently being treated and has not missed any time from work.
INJURED WAS USING A UTILITY KNIFE TO CUT OFF A TORN PIECE OF CONVEYOR BELT FROM THE SECTION BELT. THE KNIFE SLIPPED AND CUT HIS RIGHT LEG REQUIRING 4 STITCHES.

Injured was bolting when a piece of rock pushed the rock shield in and caught him between the machine and rock shield.
Injured was struck in the head by a clevis pin which broke while trying to extract a continuous miner from a pillar fall. Injured was unconscious for about 30 seconds.
